    Name of Operation: TRIAD CHILD DEVELOPMENT CENTER Facility ID: 4155254 Consultant: ERICKA BAILEY Operation Type: Center Case Number: Visit Date: 5/15/2025 Number Present: 50 Completed Date: 5/15/2025 Age: From 1 To 5 Total Minutes: 105 Time In: 10:15 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of this routine unannounced visit is to assess compliance with applicable child care requirements. This center currently operates with a Five Star-Star License effective December 13, 2018. I was greeted by Ms. Kimberly Thomas, the director. An approved fire inspection was conducted on April 21, 2024, and I received a copy. The latest sanitation inspection was conducted on January 7, 2025, with a Superior score. I observed ratios/children. I observed the one and two year old's eating lunch and transitioning to naptime. One of the NC PRE-K classes was outside playing on the playground, and the three and four year classroom was playing on the playground. The other two NC PRE-K classrooms were participating in various centers in their classrooms, engaging in music and movement, followed by transitioning for lunch and rest time. The playground has plenty of shade and activities for the children. There is room for running and vigorous play. New mulch has been added in areas with climbing equipment. The fence where the 3 and 4 years old play on the playground was closed off due to the recent storm causing tree branches to hit the fence further damaging the tension mental wires causing them to stick out the ground. Ms. Kimberly has had someone already come and begin repairs, this area of the playground is closed off till the reminding repairs are completed. I reviewed the Staff and Training worksheets; staff have a current CBC on file and special trainings are current. A partial assessment of applicable child care requirements was conducted during today’s visit. There were two items of noncompliance documented during today’s visit, and both were corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the one and two year old room one plug outlet was uncovered. 10A NCAC 09 .0604(c) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. In the two year old classroom foam rubber blocks were in the block center on the shelf. .0604(q) Please continue to visit DCDEE’s website to get the latest information for child care at https://ncchildcare.ncdhhs.gov/. MOODLE has training courses that are free and will provide additional in-service hours. Technical Assistance Criminal Background Checks (CBC) •Start Early: Encourage staff to begin the process at least six months in advance to account for any delays. •Use ABCMS Portals: Regularly check the ABCMS Portal to track when staff qualification letters are due. •Set Reminders: Implement a system of automated reminders for upcoming CBC renewals. •Maintain Compliance: Ensure all staff meet the NC Child care Law (General Statute 110-90.2), which mandates CBCs for all child care workers. Field Trips Remember: •Permission slips: ensure that permission slips are signed with detailed information about the trip, give a copy to parents, keep one in the office, and post one in the classroom. •Roster & Attendance: Keep an updated roster at the center and conduct frequent headcounts. •Emergency Contact/ Emergency Medicines: Make sure each teacher has all the emergency info, and all the emergency medicines students need. •Transportation Safety: Always double- check that no child is left behind. •Supervision: Maintain required ratios and never lose sight of the kids. I was greeted by Ms. Kimberly Thomas, the center director. This center currently operates with a Five-Star license, effective December 20, 2018. A fire inspection was conducted on August 28, 2024, and I received a copy. The latest sanitation inspection was conducted on January 7, 2025, with a Superior score. The compliance history is 95% as of March 13, 2025. A full assessment of applicable child care requirements was conducted, I used the Annual Monitoring Checklist during today’s visit. I walked through the center and the playground. I observed the children and gathered attendance. Children were participating in group activities, individual play, and coming back inside from a short walk on the playground. The classrooms are supplied with many activities and toys for the children. The playground has plenty of shade, also there is room for vigorous play. There is plenty of mulch under climbing equipment. NCPK # 1 was playing outside one playground. NCPK # 3 was in the classroom participating in a teacher lead activity. NCPK # 2 was participating in teacher lead activities. I saw reused/recycled cardboard in the NCPK classrooms, children and staff had created dollhouses, pizza oven, and robots. The toddlers were playing in centers in the classroom. The school age children were in school during the visit. I reviewed a sampling of staff and children files. Program files were current and in order. Your signature on these forms will serve as verification that the information provided is accurate and complete. Ms. Thomas and I reviewed the staff and training worksheet. All staff have current Health and Safety training. Currently there aren’t any children requiring any medications. This center does provide transportation for after school pick-up. I monitored the bus; inspection, license and insurance are up to date. The notebook is current as well. I observed two items of noncompliance during the visit today. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the school age room one of the plug outlets were not covered. This violation was corrected during the visit. 10A NCAC 09 .0604(c) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The director at the center did not complete their ITS-SIDS training for the three year period. .1102(f) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by March 27, 2025. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. This center is in Cohort Number 1 for the rated license assessment; your preparation year is July 1, 2023 – June 30, 2024, and your implementation year is July 1, 2024 – June 30, 2025. Ms. Thomas mention that is wants to start the process for the ERS assessment this school year. I suggest using the NCRLAP website for guidance. Their website address is https://www.ncrlap.org. Kimberly Thomas, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Thomas and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. A staff member failed to accurately account for the arrival and departure times for enrolled children. 10A NCAC 09 .0302(d)(4) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. A staff member failed to complete all required components of an incident report documenting an injury occurring on 8-9-24. .0802 (e) 853 Incident logs were not completed and maintained as required. A staff member failed to log an incident report of an injury that occurred on 8-9-24. .0802(g)(1-6) 904 Child was handled roughly. A staff member handled a three-year-old child roughly by grabbing and yanking the child’s right arm which resulted in an injury. .1803(a)(1) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member failed to renew their First Aid certification prior to expiration in April 2024. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member failed to renew their CPR certification prior to expiration in April 2024. .1102(d) 1911 An incident report was not completed and mailed to a Division representative within seven days after the incident when medical treatment was required. A staff member failed to notify the Division within seven days of a three-year-old receiving medical attention as a result of an injury. .0802(f) Violations must be corrected immediately.
