    Name of Operation: THE NEST SCHOOL Facility ID: 41002389 Consultant: ERICKA BAILEY Operation Type: Center Case Number: Visit Date: 3/27/2025 Number Present: 48 Completed Date: 3/27/2025 Age: From 0 To 5 Total Minutes: 95 Time In: 09:50 AM Time Out: 11:25 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of this routine unannounced visit is to assess compliance with applicable childcare requirements. This center earned a Four-Star license on October 2, 2021. The last annual compliance visit was conducted on December 12, 2024. I was greeted by Ms. Ashely Joyce, the director, she accompanied me on a walk-though of the center and the outdoor area. There were children playing outside and other children were playing inside. Lunch was served during the visit. Infants were playing on the floor; teachers were providing routine care. Attendance was gathered and staff/child ratios within compliance. The playground has plenty of shade. There is a lot of room for vigorous play and many activities for the children outside. The latest Sanitation Inspection was conducted February 11, 2025, with a Superior score and 8 demerits. A Fire Inspection was conducted on July 29, 2024, and I have a copy. A partial assessment of applicable child care requirements was conducted during today’s visit. I observed three violations of childcare rules during the visit today. These three items were corrected during the visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In the three- year- old classroom the current lesson plan was not posted. This violation was corrected during the visit. GS 110-91(12); .0508(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the two-year- old and three-year-old combined classroom one electrical outlet was not covered. This violation was corrected during the visit. 10A NCAC 09 .0604(c) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In the two year old classroom there were zip lock bags in the activity center within reach in of the children. This violation was corrected during the visit. .0604(q) Four new teachers have been hired, and they have the required information for new hires in their files. Please check the DCDEE for new forms and up to date information. A full assessment of applicable child care requirements was conducted, we used the annual monitoring checklist during today’s visit. This center currently operates with a Four-Star license, effective October 2, 2021. The facility has a compliance history of 100% as of December 1, 2024. I was accompanied by Ericka Bailey, Child Care Consultant on this visit. Upon arrival, we were greeted by Wendy Douglass and Ashley Joyce, Administrators. We conducted a walk-through of the center and noted attendance. We also completed a walkthrough of the playground. The playground has shade and equipment for outdoor play. We observed infants playing and napping in cribs, and teachers providing routine care. Toddlers and preschool children were observed playing in centers, having group time and playing outside. Teacher interactions with children were warm and nurturing. We reviewed six children’s files, six new staff members files, and CPR/FA, IT’s SIDS and annual requirements for other staff. An approved fire inspection was conducted on 7/29/24 and we received a copy. The latest sanitation inspection was conducted on 3/13/24, with a superior score. Playground inspections and emergency drills were documented and current. Required items were also posted. We monitored the van used to transport school age children. The insurance and registration was current, attendance records were current and a fire extinguisher and first aid kit was on the van. Three items of non-compliance were observed during the visit and the corresponding violations were cited. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. A portion of the fence facing the street below 4 feet measuring 3 feet 6 inches and 3 feet 7 inches in multiple places along the fence. GS 110-91(6); .0605((i) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Orientation due within six weeks of employment was not documented for R. Coleman, M. Menting, C Primus and S. Wolf. .1101(a) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed.
