Name of Operation: THE NEST SCHOOL Facility ID: 41002387 Consultant: ERICKA BAILEY Operation Type: Center Case Number: Visit Date: 1/8/2025 Number Present: 89 Completed Date: 1/8/2025 Age: From 0 To 5 Total Minutes: 385 Time In: 09:20 AM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to check compliance during an annual compliance visit. Today I was accompanied by Samantha Mcleod child care license consultant. We were greeted by Ms. Jen Newsom the director and Ms. Rachel Briggs, the assistant director. This center currently operates with a Three-Star license, effective October 2, 2019. An approved fire inspection was conducted on July 30, 2024, and I received a copy. The latest sanitation inspection was conducted on August 13, 2024, with six demerits and a superior score. The compliance history is 98% as of January 7, 2025. A full assessment of applicable child care requirements was conducted, We used the Annual Monitoring Checklist during today’s visit. We conducted a walk-through of the center and gathered attendance. We observed the children, staff, ratios, and supervision. The infants were playing on the floor, and the teachers were providing care. The classroom with the older two-year-old's and three-year-old's were playing in centers, Teachers were engaging with the children during play. The young two-year-old's were having story time. The one-year-olds were playing and the teachers were sitting on the floor with the children. The three- and four-year-olds were doing group activities. The three-year olds were having story time. We observed teacher interaction with children and all observations we saw were warm and nurturing. We observed the playground. The red picnic table was rusted with shape edges and hazards. Some areas of the fence on the playground had areas of entrapment and protrusions. The swings are not being used and are out of the reach of the children. There wasn’t a significant amount of shade provided on the playground that had the mulch. We strongly suggested shade is provided on the playground, there were only some shades is provided on the playground equipment for only two children on that playground with the mulch. This center recently received mulch. We also suggested on the schoolager playground that the pole by the basketball goal have some type of protection of your choice to prevent future injuries from happening as a precaution. We reviewed a sampling of staff and children files. We also reviewed any new staff's files. Program files were current and in order. Your signature on these forms will serve as verification that the information provided is accurate and complete. Ms. Newsom has completed the Staff and Training Worksheet. All items are up to date and training is current, and special training is also current. The buses are clean, we also saw current registrations, insurance forms and current license tags. They have the notebooks to check children on the bus in the afternoons. Two violations were observed during the visit. Violation Number Comment Rule 218 Outdoor play area did not provide an area that is shaded. There wasn’t a significant amount of shade provided on the playground that had the mulch on it. 10A NCAC 09 .1402(d) 721 All equipment and furnishings were not in good repair. A red picnic table on the middle pavement playground had sharp edges around the top of the table and on the seated area. On middle playground the fence were in poor condition. G.S. 110-91(6); .0601(b) We discussed with Ms. Newsom about her recent request regarding the floor plan on room #2210 and discussing how it could be modified to include infants, if there is an approved food prep area by sanitation. Jennifer mentioned having a bottle warmer and a mini fridge, we emphasize the need for a dedicated sink for food prep. We explained that a written approval from the sanitation inspector has to take place before we can approve changes in the age range for room # 2210. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future.
