    Name of Operation: PARKVIEW ELEMENTARY NC PRE-K PROGRAM Facility ID: 41002096 Consultant: ERICKA BAILEY Operation Type: Center Case Number: Visit Date: 3/6/2025 Number Present: 17 Completed Date: 3/6/2025 Age: From 4 To 5 Total Minutes: 190 Time In: 10:10 AM Time Out: 01:20 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit. I used a checklist to note the requirements I monitored today. This NCPK currently operates with a Five-Star license, effective May 24, 2017. This elementary school has one NCPK classroom. I was greeted by the school office staff. I then was greeted by Ms. Lisa Billy, LT of the Pre-K classroom and Ms. Keisha L, a sub filling in for Ms. Tonya Morgan, TA in the classroom. Today seventeen children were present. I observed the NCPK classroom and gathered attendance. When the children came back in, they went to the play area outside the classroom and transitioned to bathroom breaks. Child had a group time and then move to the cafeteria for lunch. The children moved freely around the classroom. Ms. Billy & Ms. Lagrant were involved with the children and guiding the children with their activities. All interaction I observed were warm and caring. The playground is outside the door of the classroom, they have many toys and activities to take outside. The area has room for gross motor play and vigorous play. The latest Sanitation Inspection was conducted January 14, 2025, with a Superior score. I reviewed a sampling of children’s files. I reviewed staff files and the staff and training worksheet. Your signature on these forms will serve as verification that the information provided is accurate and complete. The center’s compliance history is 95% as of March 06, 2025. I observed five violations of childcare rules during the visit today. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Two plug outlets by the teacher desk were uncovered. This violation was corrected during the visit. 10A NCAC 09 .0604(c) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The playground inspection was not completed during the month of February 2025. This violation was corrected during the visit. .0605(q) 1054 Documentation of staff's on-going training was not on file and/or was not current. One of the teachers did not have the required amount of training hours on file. 10A NCAC 09 .1106(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last shelter-in-place was documented on October 23, 2024, there was no documentation of any other shelter-in-place after that date. .0604(u);.0302(d)(8) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One teacher did not have Recognizing and Responding to Suspicions of Child Maltreatment training on file. .1102(g) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by March 20, 2025. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. All staff need to have a copy of CPR and First Aid in file or someone in classroom. All staff also needs to have a copy of Health and Safety training Certificates in file.
