The last annual visit was conducted on 09/24/2024. This facility has a compliance history of 99% as of 09/16/2025. Upon arrival in space 2, I was greeted by J. Kasek, lead teacher, Ms. A. Foster was subbing for Ms. Felecia Bone, teacher assistant. I observed children playing outside on playground, transitioning back indoors participating in large group time, transitioning to free play in centers in the classroom, followed by lunch then rest time. In Space 1, I was greeted by Ms. V. Bogle sub and Mr. J. Milner sub for Ms. N. Mack, lead teacher, and M. Williams, teacher assistant, were both out the day of the visit. I observed children playing in a variety of centers, cleaning up and participating in group time on the carpet. I also observed them resting on cots after lunch. Required items were posted and current in both licensed spaces. A sanitation inspection was completed on 4/30/24 and the facility received a Superior classification. I monitored the outdoor space where centers were set up on the concert area with activities available for large gross motor skills. Four violations were cited during today’s visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. 10A NCAC 09 .0604(c) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. All staff did not have updated health questionnaire's in their files. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. New staff member did not have emergency information form in file. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. Two sub one in space one classroom and one in space two class did not have criminal background checks completed. G.S. 110-90.2(b) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. This North Carolina Pre-Kindergarten program currently operated by Guilford County School System earned a Four-Star License issued 12/18/2017. The last Sanitation Inspection was conducted on January 23,2025 and the facility earned a “superior” classification. The last Fire Inspection occurred on 10/28/24 and the program was approved for Daytime Care only. The last Annual Compliance visit was conducted on 09/24/2024. A partial assessment of applicable child care requirements was conducted during today’s visit. There are two North Carolina Pre-Kindergarten Programs located at this Guilford County Public School. N. Mack, N. Davis, J. Kasek, M. Williams, and twenty-five children were present during this visit. Upon arrival in space 2, I was greeted by J. Kasek and T. Watkins. I observed children having small group time with each of the teachers. They participated in art crafts for valentine day. I also observed children having free play in different centers. In Space 1, I was greeted by N. Mack and M. Williams. I observed children playing in a variety of centers, cleaning up and participating in group time on the carpet. There were two violations of Child Care requirements observed during this visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One electrical outlet was uncovered in one of the classrooms. This violation was corrected during the visit. 10A NCAC 09 .0604(c) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. There was only one shelter-in-place done this school year back on October. .0604(u);.0302(d)(8) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a letter of correction by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by February 28, 2025. Include the following information in your correction letter: 1.Facility Name 2.Facility ID 3.Date of the letter 4.The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. Reminders: Please to remember to check your CPR and First Aid training expiration dates a few of your CPR and First Aid training is expiring this August 2025. Please remember to do shelter-in-place or lock down drills every three months. Please remember to check electric outlet to make sure they are covered.
