I used a checklist to note the requirements I monitored today. This NCPK currently operates with a Five-Star license, effective June 22, 2017. This elementary school has one NCPK classroom. I was greeted by Ms. A. Nissan, LT and Ms. Andrea Gomez, TA when I arrived to the classroom. Today thirteen children were present. I observed the NCPK classroom and gathered attendance. Children were participating in free play, individual time with teachers, and group activities. The children moved freely around the classroom and outside. Ms. Gomez, the TA took half of the children outdoors for play time on the covered porch due to the rain and when she returned, she took the other half of the children outdoors for play time. All the interactions I observed were warm and caring. A Fire Inspection was conducted on April 10, 2024. The latest Sanitation Inspection was conducted January 16, 2025, with 4 demerits and a Superior score. I reviewed sampling of children’s files. I reviewed staff files and the staff and training worksheet. Your signature on these forms will serve as verification that the information provided is accurate and complete. The center’s compliance history is 98% as of February 12, 2025. I observed two violations of childcare rules during the visit today. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Two electrical outlets in the classroom did have covers over them. This violation was corrected during the visit. 10A NCAC 09 .0604(c) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The NCPK program only had one shelter-in-place done in September 2024 this school year. .0604(u);.0302(d)(8) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a letter of correction by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by February 26, 2025. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. Reminder: Ms. Gomez your CPR and First Aid expiration is coming up at the end of August 2025. Please remember that Shelter-in- Place or Lockdown Drills must be completed every three months. You were provided with an opportunity to ask questions regarding child care requirements. Please continue to visit DCDEE’s website for any added information or forms pertaining to early education. Ericka Bailey, a childcare consultant accompanied me during today’s visit. Upon our arrival, we were greeted by one of the office staff. The office staff walked us to the Prek classroom. When we entered the classroom, Ms. A. Gomez, the teacher assistant greeted us. Ms. Gomez informed us that Ms. A. Nissen, the lead teacher was out today. We introduced ourselves informed Ms. Gomez the purpose of today’s visit. Ms. Gomez gave me the necessary paperwork I needed to complete today’s visit. We observed and documented the attendance. Sixteen (16) children were present during today’s visit. They were observed building in blocks, fixing puzzles, playing with playdough, playing in housekeeping and drawing. Later I observed then cleaning up and playing outside on the playground. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions; CPR and First Aid Certification. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. The substitute's file was not made available for review. G.S. 110-91( 9) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Prior to the expiration date of the qualification letter, the principal did not submit required forms to complete a criminal background check. The qualification letter expired September 16,2024 G.S. 110-90.2(b) & .2703(n)&(o) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. An asthma inhaler and EpiPen was not labeled with the one child's name, and/or written instructions did not include the required information. 10A NCAC 09 .0803(2)(b)(i-v) The above violations must be corrected immediately, and a letter must be email to me no later than October 23,2024 stating how each item was corrected.
