Name of Operation: MONTLIEU ELEMENTARY NC PRE-K PROGRAM Facility ID: 41002157 Consultant: ERICKA BAILEY Operation Type: Center Case Number: Visit Date: 12/10/2025 Number Present: 16 Completed Date: 12/10/2025 Age: From 4 To 5 Total Minutes: 90 Time In: 09:15 AM Time Out: 10:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit. I used a checklist to note the requirements we monitored today. This NCPK currently operates with a Four-Star license, effective February 20, 2018. There is one NCPK classroom located in this elementary school. I was greeted by Ms. Stacey Pate, LT and Ms. Amesha Tolman, Sub. Today sixteen children were present. I observed the NCPK classroom and gathered attendance. Children participated in large group time, then they transitioned to small group time with one group with Ms. Pate and the other children participated in small group time, Ms. Tolman. After small group time Ms. Pate gathered the children for reading time and then they washed hands and transitioned to lunchtime. All interactions I observed were warm and caring. The NCPK has their own playground area, the teachers bring playground toys outside. The area is shaded, with room for gross motor play and vigorous play. A Fire Inspection was conducted on November 20, 2025. The latest Sanitation Inspection was conducted September 15,2025, with 8 demerits and a Superior score. I reviewed a sampling of children’s files. I reviewed staff files and the staff and training worksheet. Your signature on these forms will serve as verification that the information provided is accurate and complete. The center’s compliance history is 96% as of December 10, 2025. I observed one violation of childcare rules during the visit today. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. The sub in the classroom did not have their Criminal Background Check completed. G.S. 110-90.2(b) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by December 24, 2025. Included the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. If I do not receive your letter by December 24, 2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Today Wytina Blackmon accompanied me during the visit. We used a checklist to note the requirements we monitored today. All interactions we observed were warm and caring. The NCPK has their own playground area, the teachers bring playground toys outside.
