Name of Operation: Little Reasons Academy Facility ID: 41002518 Consultant: MARY MCMILLAN Operation Type: Family CC Home Case Number: Visit Date: 5/27/2025 Number Present: 4 Completed Date: 5/27/2025 Age: From 0 To 4 Total Minutes: 75 Time In: 10:30 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a temporary time period visit. During today's visit I met with Ms. Valencia Turner, owner/operator. Ericka Bailey, Child Care Consultant, accompanied me during today's visit. The children were observed during free play and a teacher directed art activity. The infant was sleeping during the visit. The following items were monitored today: Supervision, capacity, approved space, safe environment, program and children's records and the children were playing with age-appropriate activities, required items posted and restrictions followed. I reviewed your 3-month self-study that you have completed, and you have met the requirements with improvements on the playground. Your last fire drill was recorded on 5/14/25 at 3:46pm with 1 minute and 10 seconds evacuation time. Your last emergency drill was recorded on 5/23/25. The following violations were cited during today's visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment, for 1 preschool child. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1803 Safe sleep policy did not contain the required information as indicated in rule .1724(a)(1-12). G.S. 110-91(15)(a)(b) & .1724(a)(1-12) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. In the bathroom there were topical items and lotions stored on the counter and within reach of the children. .1719(a)(6) During today's visit we discussed the following items of technical assistance due to the items of non-compliance: -Making sure that your ITS-SIDS policy is fully completed with the specific items that your facility does. -All preschool children's physicals and immunization records must be on file within 30 days of their enrollment. -All lotions/topical items for you and your household must be stored out of reach of the children in the bathroom, due to them being potential hazards for the children. We discussed the following items of consultation during today's visit: -Rated license information: Program: Once all violations are corrected and you are meeting minimum requirements you can earn 2 points Education: Your Associates Degree in ECE has been verified by our WORKS unit and you will earn 7 points based on education and 30+ years of experience. Quality: 1 point: You have chosen to serve no more than 2 infants under 1 year of age Total: It appears that you will receive 10 points for a 4 star family child care home license We discussed that once your playground is completed, you will need to complete your monthly playground inspections. Your son, D. Scott, that lives with you, is your substitute caregiver. You must correct all violations immediately and submit a compliance letter, stating how the violations are corrected by 6/10/25.
