I used a checklist to note the requirements we monitored today. This elementary school has two NCPK classrooms and currently operates with a Four-Star license, effective May 3, 2017. I was greeted by Ms. D’Toya Dove, LT, and Ms. Marie Price subbing for Ms. T’Erica McCall, TA, Ms. T’ Erica joined the classroom during lunchtime and in the other NCPK classroom I was greeted by Tanya Curry, LT and Ms. Monique Short, TA. Today twenty- five children were present. I observed both NCPK classrooms and gathered attendance. Children were participating in free play, story time, group time, and having lunch. All teachers and teacher assistants were involved with the children and guiding the children with their activities. All interactions I observed were warm and caring. Ms. Dove also had a one on one aid for a child in the classroom name Mr. Douglas. The NCPK’s have their own playground area. No violations were noted on the playground during today's visit. A Fire Inspection was conducted on January 08/2025. The latest Sanitation Inspection was conducted September 22, 2025, with a Superior score. I reviewed sampling of children’s files. We reviewed staff files and the staff and training worksheet. Your signature on these forms will serve as verification that the information provided is accurate and complete. The center’s compliance history is 93% as of December 10, 2025. I observed one violation of childcare rules during the visit today and the violation was corrected during the visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. The one on one aid in one of the NCPK classrooms did not have their Criminal Background Check completed. G.S. 110-90.2(b) The above violations must be corrected immediately, and a letter must be email to me no later than December 25, 2025, stating how the one item was corrected. If I do not receive your letter by December 25, 2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. This DPI preschool program currently operates with a Four-Star License effective May 3, 2017. This site has two NC Pre-K classrooms. I was greeted by Ms. Dove, the lead teacher in the first NCPK classroom. I gathered the attendance and observed children. There were thirteen children present today. The children were participating in a large group activity followed by going outside to play on the playground. After playing outside the children transition into the classroom to wash hands and use restroom, followed by doing music and movement. In the second NCPK classroom I was greeted by Ms. Curry, she assisted me with attendance, and I gathered attendance and observed the children. There were eleven children present today. The children were participating in free play in the centers in the classrooms, all centers were open, the children then transitioned to outdoor play. A partial assessment of applicable child care requirements was conducted during today’s visit. Three violations were observed and documented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In one of the NCPK classrooms there were six electrical outlets that did not have covers on them. This violation was corrected during the visit. 10A NCAC 09 .0604(c) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member qualifying letter was expired. The staff member has begun the process to complete the requirements. G.S. 110-90.2(b) & .2703(n)&(o) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last Shelter-in-place / lockdown drills were completed in November. .0604(u);.0302(d)(8) :In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. I must receive a letter stating how the violation was corrected and how you plan to maintain compliance in the future by May 20, 2025. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. 5. Wytina Blackmon, child care consultant, accompanied me during today's visit. This NCPK currently operates with a Four-Star license, effective May 3, 2017. This elementary school has two NCPK classrooms. We were greeted by Ms. D. Dove, LT and Ms. T. McCall, TA, in the first classroom we visited twelve children were present. We explained to the teachers that we were here to follow-up the staff/Child ratio violation from last visit. We observed the NCPK classroom and gathered attendance. Children were participating in group time, free play and individual guide activity time with teachers. Ms. M. Short and Ms. L. Price, a sub. In the second classroom there were fourteen children. The were participating in free play. All the interactions we observed in both classrooms were warm and caring. The last fire inspection was done on January 08, 2025. The latest Sanitation Inspection was conducted on October 10, 2024, with six demerits and a Superior score. We reviewed the staff and training worksheet. We observed four violations of childcare rules during the visit today. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Two staff and one sub did not have update health questionnaires on file. This violation was corrected during the visit. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Two staff members and one sub did not have updated Emergency Information updated on file. This violation was corrected during the visit. .0701(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Wytina Blackmon, child care consultant accompanied during today's visit. We used a checklist to note the requirements we monitored today. This NCPK currently operates with a Four-Star license, effective May 3, 2017. This elementary school has two NCPK classrooms. We were greeted by Ms. T. McCall, TA in the first classroom we visited. Ms. D. Dove was absent today. Today twelve children were present with one teacher. We explained to the teacher that this was a violation to staff/Child ratio. Ms. McCall moved three children across the hallway and this violation was corrected during the visit. We observed the NCPK classroom and gathered attendance. Children were participating in free play, individual time with teachers, and group activities. The children moved freely around the classroom. Ms. McCall were involved with the children and guiding the children with their activities. Both classrooms had lunch and after lunch we visited the second classroom. Ms. M. Short and Ms. L. Price, a sub were doing a movement activity. In this classroom thirteen children were attending. All interactions we observe in both classrooms were warm and caring. We were not able to monitored playground due to the playground still covered with snow. The last fire inspection was done January 08, 2025. The latest Sanitation Inspection was conducted on October 10, 2024 with six demerits and a Superior score. We reviewed a sampling of children’s files. The staff files were not accessible. Your signature on these forms will serve as verification that the information provided is accurate and complete. The center’s compliance history is 98% as of January 6, 2025. We observed four violations of childcare rules during the visit today. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There were no documentations of fire drills from February 2024 thru December 2024. .0604(t); .0302(d)(5) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. There were no documentations for monthly playground inspections for the months of February 2024 thru December 2024. .0605(q) 1043 All staff records, except financial records, were not made available for review. The staff records were not accessible during today's visit. G.S. 110-91( 9) 1775 NC Pre-K program staff/child ratios and group sizes were not met.
