    Name of Operation: KINDERNOGGIN Facility ID: 41002208 Consultant: JENNIFER JONES Operation Type: Center Case Number: Visit Date: 5/12/2026 Number Present: 42 Completed Date: 5/12/2026 Age: From 1 To 5 Total Minutes: 265 Time In: 10:05 AM Time Out: 02:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for compliance during an annual compliance visit. A checklist was used to note the requirements I monitored today. This facility currently operates with a five-star license issued 10/25/18. This facility has an 87% compliance score as of 5/11/26. The last annual compliance visit was on 6/5/25. As of 5/11/26 the facility was current and active according to the Secretary of State website. The last sanitation visit was 3/11/26 and resulted in a Superior rating with six (6) demerits. The facility tested negative for lead in water 5/2/24. The facility needs to contact Clean Classrooms regarding lead in paint testing and asbestos testing. Upon my arrival, I was greeted by Deborah Brooks, Administrator and Wanda Wolfe, Owner/Administrator. I informed Ms. Brooks and Ms. Wolfe of the purpose of today’s visit. Ms. Brooks and Ms. Wolfe gave me the necessary paperwork I needed to complete today’s visit. The last fire drill was conducted 4/21/26. The last emergency drill(Shelter-in-place) was conducted on 3/17/26. The last playground inspection was 5/1/26. The last fire inspection was 3/11/26 and is attached to this report. Forty-two (42) children were present during today’s visit. I observed the indoor and outdoor spaces. I observed both staff/child ratio and supervision and found both to be in compliance. During today’s visit, I observed children playing outside, playing in centers, and at group time. The outdoor environment has a wide variety of materials and activities for the children to participate in. There needs to be more mulch around the large climber. All medication and hazardous items were out of the children's reach. There were two diaper creams in the Twos room that did not have current paperwork. All posted items were posted in the classrooms and lobby. Staff and children’s files were monitored during today’s visit. Six children files were reviewed. The staff and training worksheets were reviewed during today’s visit. Your signature on these forms will serve as verification of the information provided is accurate and complete. One staff member needs an updated criminal background check and one staff member’s CBC will expire on 6/16/26. There were three violations cited during today’s visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. The permission for two medications (Cortizone and Aveeno) had expired. .0803(12) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member had an expired CBC. G.S. 110-90.2(b) & .2703(n)&(o) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The surfacing (mulch) was not deep enough around the large climber. .0605(k)(1-4) To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by 5/26/26. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected and information regarding how the violation was corrected. I provided technical assistance regarding: Training: We discussed pro-rating training based on part-time employment and if the staff member has worked in early childhood education for 10+ years. Most staff had recently participated in CPR/First Aid and It’s SIDS. We discussed the Southwestern Child Development site and that CEUs can be obtained through this organization to meet desired star levels. EQUiPD The EQUiPD website has many webinar trainings that can be completed online for no cost and have been approved by NCDCDEE. Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way to for staff at the CBC Unit to keep you updated of the status of out-of-state checks if a new employee has a Provisional Qualification. Instructions on how to link to the facility was provided today. In the future, new staff need to link within five days of hire or transfer to your center. Deborah Brooks, Program Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Brooks and one additional staff member. Limited monitoring of child care requirements occurred during today’s visit. The following violation was confirmed during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member failed to attend to a four-year old child in an nurturing manner during naptime, by grabbing the child by the ankle, and leaving scratch marks. G.S. 110-91(10) Violations must be corrected immediately. Within one week, no later than August 26, 2025 you must submit documentation of the corrections you made and your plan to maintain compliance with the identified childcare requirements to me, via email at rhonda.m.grady@dhhs.nc.gov. An Annual Compliance Monitoring Checklist for Centers was completed during this visit. This facility currently operates with a Five-Star License issued 10/5/2018. The last Annual Compliance Full Visit was conducted on 06/11/2024. Jen Jones, child care consultant accompanied me on this visit. The last sanitation inspection was conducted on 01/06/2025 and the facility earned a “superior” classification with 8 demerits. The last fire inspection occurred on 03/26/2025 and the facility was approved for day and night time care. W. Wolfe, the facility’s administrator, and D. Brooks, assistant administrator, and 48 children were present during the visit. We observed staff members attending to the individual needs of the children. We observed children during free play in centers in classroom, children participating in open centers in the classroom, engaging large group counting in Spanish and English , art activities, and singing songs. We reviewed five children files. There were five violations of Child Care requirements observed on this visit. Violation Number Comment Rule 619 Diaper changing surfaces were not cleaned with a detergent solution and disinfected after each use. Toddler teacher did not clean changing surface after two diaper changes, toddler teacher did not wash their hands after two diaper changes, diaper cream was expired, and lotion did not have parent permission slip with it. 15A NCAC 18A .2819(c) 807 A safe indoor and outdoor environment was not provided for the children. plastic bags, foam blocks, outlets were uncovered in toddler classroom, all violations were corrected during the visits. 10A NCAC 09 .0601(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members did not complete first aid training. .1102(c) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not have maltreatment training within 90 days. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. One staff member did not complete health and safety training within one year of employment. .1102(a) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately.
