Name of Operation: HIGH POINT FAMILY DAYCARE LLC Facility ID: 41002458 Consultant: ERICKA BAILEY Operation Type: Center Case Number: Visit Date: 12/4/2025 Number Present: 29 Completed Date: 12/4/2025 Age: From 0 To 5 Total Minutes: 120 Time In: 10:15 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to check compliance during an annual compliance visit. I was greeted by Mr. Bilal Aslam, the owner/director. This center currently operates with a Five-Star license, effective August 24, 2023. An approved fire inspection was conducted on May 6, 2025, and I received a copy. The latest sanitation inspection was conducted on July 29, 2025, with a superior score. The compliance history is 98% as of December 03, 2025. A full assessment of applicable child care requirements was conducted, I used the Annual Monitoring Checklist during today’s visit. I conducted a walk-through of the center and gathered attendance. I observed the children, staff, ratios, and supervision. The infants were playing on the floor, and the teacher was providing care. The three-and four-year-old's were playing outside on playground, transitioning to lunch time, then rest time. The two- and 3-year-olds were participating in a large group activity, then transitioned to lunch and rest time. The one-year-olds were eating and the teachers were sitting on the floor assisting the children with their lunch. I observed teacher interaction with children and all observations I saw were warm and nurturing. The center is well organized and neat. I observed the playground. The playground has shade and equipment for outdoor play. I reviewed a sampling of staff and children files. Program files were current and in order. Your signature on these forms will serve as verification that the information provided is accurate and complete. Mr. Aslam has completed the Staff and Training Worksheet. All items are up to date and training is current, and special training is also current. I observed one violation of childcare rules during the visit today, and that violation was corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the pre-k classroom one outlet did not have a cover. That violation was corrected during the visit. 10A NCAC 09 .0604(c) Corrective action is not required on your part at this time. You must continue to maintain compliance with applicable childcare requirements. Please remember it is your responsibility to know and follow all child care rules and requirements. Consultation: EQulPD: We discussed training opportunities with EQuIPD and I left an informational document that you can use also coaching and consultation toolbox was on the document as well. QRIS: We went over information about the QRIS, and we discuss what Pathways you fell might be a good fit for your staff, program, and families. You mention that you needed a couple weeks to view information to decide which way your program might take. I left the Pathways To The Stars document with you to fill out and email me when you fill it out.
