I was greeted by Ms. Marie Hough, the center director. This center currently operates with a Five-Star license, effective March 20, 2019. This center has two NCPK classrooms and serves children ages 2 – 5 years old. An approved fire inspection was conducted on February 10, 2025. The latest sanitation inspection was conducted on August 18, 2025, with a Superior score. The compliance history is 91% as of January 20, 2026. A full assessment of applicable child care requirements was conducted, I used the Annual Monitoring Checklist during today’s visit. I walked through the center and the playground. I observed the children and gathered attendance. Children were participating in group activities, individual play, ate lunch and transition to nap time. The classrooms are supplied with many activities and toys for the children.The playground has plenty of shade, also there is room for vigorous play. I reviewed a sampling of staff and children files. Program files were current and in order. Your signature on these forms will serve as verification that the information provided is accurate and complete. Ms. Hough and I reviewed the staff and training worksheet. All staff have completed their Health and Safety training. One staff member has not completed the Child Maltreatment training. This center does not provide any type of transportation. Not do the have any children requiring any medications. I observed one item of non-compliance of childcare rules during the visit today. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Two staff members did not complete their and submit required forms to complete a criminal background check within their five year date of last submission. G.S. 110-90.2(b) & .2703(n)&(o) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by February 3, 2026. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. QRIS: We went over information about the QRIS again since your last RUV in October 2025, and we discuss what Pathways you fell might be a good fit for your staff, program, and families. You mention that you were leading towards Program Assessment Pathway this coming Fall 2026. You also mentioned that you and your staff will began Mocks training this coming February 2026 with Brooke Hough to help prepare for the coming assessment. Reminders: • Please make sure all ongoing training is completed with certificates in files by your annual compliance visit next year for viewing to verify. • Make sure criminal background letters are up to date before your expiration date, you can apply for those up to six months before your due date. • Please make sure your annual updates are completed in your file for review per annual compliance visit such as health questionnaire, emergency information, EPR. • Please ensure that your staff Recognizing /Responding to Child Maltreatment training is completed for new staff within 90 days and every five years afterwards and Health & Safety are completed for new staff before being at your facility for a year and every 5 years afterwards. Ericka Bailey, a childcare consultant accompanied me during today’s visit. Upon our arrival, we were greeted by one of the lead teachers. The lead teacher led us to the director Ms. M. Hough on the playground. We introduced ourselves informed Ms. Hough the purpose of today’s visit. Ms. Hough gave me the necessary paperwork we needed to complete today’s visit. We observed and documented the attendance. Thirty-three (33) children were present during today’s visit. They were observed playing outside on the playground, washing their hands in preparation for lunch, eating lunch and napping. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions. There were( 4) four violations cited during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Two children's emergency medication forms did not have the parent's signatures on the form. 10A NCAC 09 .0803(4)(6-9) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not complete First Aid within 90 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not successfully complete certification in CPR training within 90 days of employment. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff did not complete the did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) The above violations must be corrected immediately, and a letter must be email to me no later than October 30,2024 stating how the one item was corrected. Consultation: I discussed with Ms. Hough new staff member need in-service training hours, health and safety training by December 1,2024. EPR Plan: Today we discussed the EPR plan. Ms. Hough stated she didn’t know how to get back in the facility’s EPR Plan. Ms. Hough asked us how do you access the system. I contacted Ms. V. Vample Education & Training/RCCR&R Director. Ms. Vample informed that Ms. Hough has been automatically archive in the NC Emergency Management System. I informed Ms. Hough of my findings.
