This FCCH earned a Four-Star License on January 24, 2020. Meg Riddle, supervisor license consultant, accompanied me during today’s visit. Upon my arrival, we were greeted by Ms. Deloris Davis, the owner of this FCCH. Ms. Davis and the children were in the livingroom area. Ms. Davis and the children were playing with toys. The outdoor play area is fenced in and there are many items for active play. Currently you do not provide any type of transportation. As of February 10, 2025, the compliance history for this FCCH is 95%. Fire drills and Playground inspections were current and accessible. I used the Annual Compliance Monitoring Checklist for FCCH and the FCCH Item Number Listing (DCD 0375) as basic tools to assess compliance with applicable child care requirements pertinent to this FCCH. Five children are in attendance. All centers were open for the children. Ms. Davis participated in activities with the children. All interactions we observed were warm and caring. All the children’s files were reviewed today, along with Ms. Davis. Program files were also reviewed. To verify compliance with specific requirements, provider information was recorded onto the Verification Form for FCCH, and Ms. Davis completed the FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form. I received the signed forms during today’s visit. Your signature on these forms will serve as verification the information provided is accurate and complete. I observed one item of noncompliance during the visit today. Violation Number Comment Rule 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. Four children files did not have the Summary of the NC Child Care Law document in it. GS 110-102 In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by February 24, 2025. Include the following information in your correction letter: 1.Facility Name 2.Facility ID 3.Date of the letter 4.The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. Please to remember to check and update the children files. All special training is up to date, as is the health and safety training. Ms. Davis needs two more training hours before 2-16- 25. Name of Operation: DAVIS INFANT, TODDLER, AND SCHOOLAGE CARE Facility ID: 41000757 Consultant: SAMANTHA MCLEOD Operation Type: Family CC Home Case Number: Visit Date: 8/29/2024 Number Present: 4 Completed Date: 8/29/2024 Age: From 0 To 5 Total Minutes: 115 Time In: 10:20 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s routine unannounced visit was to assess compliance with applicable child care requirements for a Family Child Care Home (FCCH). Ericka Bailey, Child Care Consultant was present and assisted during today’s visit. This (FCCH) currently operates with a Four Star License effective August 12, 2019. Upon my arrival, I was greeted by the licensed provider Delores Davis, the FCCH/caregiver. Mrs. Davis assisted me during my visit today. I completed a walk-through of the indoor and outdoor areas of the residence used for child care. Four (4) children were present during today’s visit. During today’s visit the children were observed being cared for in the front room of the residence. During today’s visit I observed positive interaction between the child care provider and the children. A partial assessment of applicable child care requirements was conducted during today’s visit. Limited monitoring of operator files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Special Training including Health and Safety Training and Criminal Background Checks. Mrs. Davis reported that her husband is the only other person that lives in the home. Mr. Davis has a current Criminal Qualifying letter on file. The following violations were observed and documented. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. In the backyard a container of gasoline for a lawnmower was out accessible to the children. In the living room (primary space for children) a hanging cord from the TV was observed hanging near the playpen where the children were sleeping. .1719 (a) (1) 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. On the table in the kitchen a bottle of medication was left out and accessible to the children. .1719(a)(5) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. One child enrolled did not have an immunization record on file. GS 110-91(1); .1721(a)(2) 1892 Application was not on file for each enrolled child, including his/her own preschool child(ren) who are not school-age. One child enrolled did not have a file available. .1721(a)(3) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. In the bathroom a bottle of Lysol wipes was observed stored under the cabinet. .1719(a)(6) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by September 12, 2024. Mrs. Davis is currently working on completing the final steps for the Carolina Kids Program for clean water testing. Once she has completed the survey she will receive the kit for testing, please keep me updated on your steps and email me the final copy of your results. RATED LICENSE UPDATE: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. UPDATED RULES: Effective January 1, 2024, we have new/updated rules posted on the DCDEE website. The changes relate to definitions; lead and asbestos for centers and family child care homes; building requirements for family child care homes; multi-unit child care centers; and criminal background checks. Please refer to the website for more information.
