An Annual Compliance Monitoring Checklist for Centers was completed during this visit. This facility currently operates with a Five-Star License effective September 26, 2024. The last sanitation inspection was conducted on 03/02/2026 and the facility earned a “superior” classification with 5 demerits. The last fire inspection occurred on 02/16/2026 and the facility was approved for daytime care. I was greeted by Ms. Patti Triplett, director. Ms. Triplett accompanied me as I gathered attendance and observed classrooms. I observed ratios/children, interactions, and activities. There were seventy-six children present today. I observed staff members attending to the individual needs of the children. I observed infants crawling and playing in the classroom, children during free play in centers in the classroom, children participating in open centers in the classroom and engaging in large group reading. I reviewed nine children files, and I reviewed seven staff files. I observed one violation of Child Care requirements on this visit. The violation was corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the 4's classroom one plug outlet was not covered. This violation was corrected during the visit. 10A NCAC 09 .0604(c) Corrective action is not required on your part at this time. You must continue to maintain compliance with applicable childcare requirements. Please remember it is your responsibility to know and follow all child care rules and requirements. We Discussed: • Making sure all employers’ information is undated on WORKS • QRIS information and the pathway you are interested in “Pathway 2 Classroom and Instructional Quality Pathway you discussed you are planning on going to a training soon that will be going over more QRIS information. Technical Assistance Criminal Background Checks (CBC) Please remember that some people CBC are coming up this year. •Start Early: Encourage staff to begin the process at least six months in advance to account for any delays. •Use ABCMS Portals: Regularly check the ABCMS Portal to track when staff qualification letters are due. •Set Reminders: Implement a system of automated reminders for upcoming CBC renewals. Maintain Compliance: Ensure all staff meet the NC Child care Law (General Statute 110-90.2), which mandates CBCs for all child care workers. Please remember to check Health and Safety and Maltreatment trainings some staff will be coming close to their one year mark/ 5 year dates for Health and Safety and some staff will be coming close to their 5 year mark for Maltreatment training. This center currently operates with a Five-Star License effective September 26, 2024. The last fire inspection was conducted on May 5, 2024. I was greeted by Ms. Patti Triplett, director. Ms. Triplett accompanied me as I gathered attendance and observed classrooms. I observed ratios/children, interactions, and activities. There were sixty-seven children present today. Children were taking a nap. Routines were being carried out in the infant classroom. All centers were open in all classrooms. Babies were on the floor playing, sleeping, and being held for a bottle. All the staff/child interactions I observed were warm and caring. I looked through three new staff member files. A partial assessment of applicable child care requirements was conducted during today’s visit. I observed one items of non-compliance during the visit today. This item of non-compliance was corrected during the visit. Today I met with Marissa Byrd, Assistant Director of the program, since the Director Patti Triplett was not present during today’s visit. We explained to you that we were here to investigate a complaint. Sharon Williams, Child Care Consultant, accompanied me during today’s visit. We conducted a walk-through of the facility. I observed the children engaged in eating, playing and napping. Limited monitoring of child care requirements was conducted during the visit: The license was posted. Supervision, Discipline, Adequate/approved space and permit restrictions and license posted were observed in compliance at the time of my observation. Information obtained determined the administrator, Patti Triplett laid across a school-aged child’s body while the child kicked and told the her they could not breathe and to get off of them. Based on our observations during the visit the staff/child ratio allegations were unable to be confirmed. Based on our interviews with multiple staff members and information gathered during the visit today, it was determined that the allegations regarding discipline of children is substantiated. The following violations were cited. Technical assistance regarding how to achieve and maintain compliance was offered. Violation Number Comment Rule 1877 A child was restrained as a form of discipline and the child's safety or the safety of others was not at risk. The Director, Patti Triplett used her body weight to restrain a child on the floor. .1803(a)(10) 1911 An incident report was not completed and mailed to a Division representative within seven days after the incident when medical treatment was required. On 6/5/24 A child had a seizure, due to a fever, and was taken to the hospital by the ambulance, and no incident report was submitted to DCDEE within 7 days. DCDEE was informed on 7/11/24 of the incident and a report was received. .0802(f) Technical Assistance reviewed during today's visit: -We reviewed the 10A NCAC 09 .1803 Prohibited Discipline in child care centers Rule with using restraint as a form of discipline. -We reviewed the procedure for submitting incident reports -We discussed that if a child is upset to let them go into a safe space to calm down and leave the child alone until they are ready to participate in activities. In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. A letter stating how the violations were corrected must be submitted to me by 7/25/24. Please submit the letter electronically to my email address at mary.mcmillan@dhhs.nc.gov A follow-up visit will be conducted. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Today I met with Patti Triplett, Director. Elizabeth Childress, Child Care Consultant, accompanied me during today’s visit. During today's visit I conducted a walkthrough of the facility and observed all classrooms of children. The children were observed during routine activities, playing, eating and sleeping. Limited monitoring of child care requirements was done. The following child care requirements were monitored: Supervision, Staff/Child Ratio, Hazardous items storage, Age-Appropriate Activities, Appropriate Discipline, required items posted and license posted. A sampling of records was monitored: 3 children's files and there were 6 new staff files, since the last visit in November 2023. All 6 new staff members have CBC qualified letters that were verified in the system. The last fire drill was recorded on 6/6/24 and the last emergency drill was recorded on 6/14/24. During today’s visit the City of High Point Building inspector was present. The following violations of child care requirements were cited during today's visit: Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. This was corrected during the visit. .0713(a)(10), (c) & (f)(3); .2818(e) 801 Written procedures were not established for pick-up and delivery of children. This was corrected during the visit. .1003(b) 807 A safe indoor and outdoor environment was not provided for the children. Indoors: There were at least 5 cribs that had very loose fitting cot sheets on the infant crib mattresses, instead of crib sheets. This was corrected during the visit. 10A NCAC 09 .0601(a) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence height in multiple areas was below 4 feet tall. GS 110-91(6); .0605((i) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member didn't have information on file, this was corrected during the visit. .0701(a) We discussed the following items of consultation today: -Making sure trash bags stay in the bin, even on the playground. -Toys being accessible to children. -Mulch and other approved surfacing. -I received your fire and sanitation inspections today, please submit your building inspection, once completed and before October 2024. We discussed the following technical assistance items with compliance items today: -Make sure that the TB results are on file by day 1 for staff. -Crib sheets must be used on crib mattresses so that they are not a hazard with bunching up around a child's face. -All required items must be posted at all times. -When you have mulch all the way up to the fence, the height will be affected. I recommend keeping the mulch in barriers or raising the fence height to at least 6 feet so that you do not have to worry about the mulch and/or protrusions on the fence. We also discussed the rated license process today. Once all of your approved inspections are completed and your violations are corrected, you can go back to your star rated license. You do not have to wait the 6 months before applying for your stars. Due to the hold harmless status, you will transition back to the stars that you had prior to the change of ownership. -You will remain in the same cohort as with your previous license. -We discussed that we do not have your pre-service form for administrators. Violations must be corrected immediately; you are required to submit a compliance letter stating how the violations are corrected to me at mary.mcmillan@dhhs.nc.gov by 7/10/24.
