This facility is owned and operated by GenerationED. This center currently operates with a Five Star license reissued on May 28, 2024. Upon arrival to the facility, I was greeted by the Director, Brandy Marshall. I made you aware of the purpose of today’s visit. Ms. Marshall assisted me during today’s visit. During today’s visit I observed the classrooms for preschool aged children (NC-Pre-K Classrooms). I observed these classrooms during group time, center play, and teacher directed small group activities. The children were also observed during gross motor play, lunch time, rest time and several transition times. The teachers were actively engaged with the children while they moved freely throughout their learning environment. Age-appropriate sufficient materials were also observed in the learning environment. It was also observed in several classrooms written documentation of the students verbal communication posted throughout the classroom. In the spaces for infant/toddler aged children (Early Head Start classrooms) I observed the children actively engaging in free play and gross motor play. The teachers were observed interacting and engaging with the children as they explored their indoor and outdoor learning environment. Age-appropriate sufficient materials were also observed in the learning environment. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. During my observation the staff was observed using the “Face to Name” method to document the attendance of the children. The outdoor playground spaces was not monitored during today’s visit due to the rainy weather outside. Seventy-two (72) children were present during today’s visit. Staff files were monitored for compliance for current Criminal Background qualifying letters and current CPR/First Aid and other specialized training as required. A sanitation inspection was conducted on April 25, 2025. This facility received a superior classification and four (4) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. This facility’s corporation status was verified current on the NC Department of the Secretary of State on May 27, 2025. The following violations of childcare requirements were cited today: Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Monthly playground inspection have been completed as required but were not completed by a staff member who has received Playground Safety training as required. .0605(q) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. In classroom space LE#6, a bottle of Benadryl did not have the child's name on the box to assist with identifying whose medication it belonged to. 10A NCAC 09 .0803(2)(b)(i-v) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by June 11, 2025. As a reminder as the administrator, you have Playground safety training within six months of hire. Please send me a copy of your playground certificate once you have received the training. CONSULTAITON & RESOURCES: MEDICATION: Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. Prevention is the key to prevent poisoning by making sure medications are inaccessible to children. Prescribed medications as well as over the counter (OTC) medicines must be labeled properly when brought to the childcare facility by the parent/guardian. The medication must be in the original container (with a label that includes the child’s name, date filled, prescribing clinician’s name, pharmacy name and phone number, dosage/instructions, and relevant warnings). Tracking System: We discussed in detail options to assist you with monitoring on a more consistent basis for compliance with medication requirements in your classrooms. I recommend that you keep a tracking log available for review that lists the medications (prescribed/over the counter) in each classroom. The tracking log could include the medication, the date it expires and when to notify the parents. It is also recommended that you complete at least monthly checks of the classrooms to ensure all required forms, authorization and action plans are current and accessible. Medical Action Plans: Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. STAFF & CHILDREN’S FILES: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. It is essential to review staff & children’s files often to ensure the program maintains accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff to ensure accuracy. As an administrator, it is your responsibility to review all employment documentation and verify that the required documents are received within the specified time frame. Using a checklist to check and date receipt of documents will help you track employment documents. A staff & children’s file checklist is available on the DCDEE website under the Provider tab and provider documents links. During today’s visit I suggested that you may want to develop a tracking system to assist you with keeping up with staff’s upcoming due dates for CPR/First Aid training, child maltreatment training, and health and safety training requirements. RATED LICENSE UPDATE: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. As discussed with you today it is important to have all your staff use this time to submit updated educational information to our Workforce system. All staff will need to have a current Workforce letter on file. It is also suggested that you visit the NCRLAP (www.NCRLAP.org) website for upcoming training and view their resources for the ITERS-3, ECERS-3 and SACERS-U. NC Pre-K requirements were also monitored for compliance during today’s visit. A checklist was used to note the requirements I monitored today. This facility is owned and operated by GenerationED. This center currently operates with a Five Star license reissued on May 28, 2024. This facility is also NAEYC accredited. The accreditation will expire in October, 2028. Upon arrival to the facility, I was greeted by the Center Administrator, Ms. Brandy Marshall. Ms. Marshall assisted me during my visit today. A walk-thru of the indoor and outdoor spaces used by the children was conducted. Ninety-three (93) children were present during today’s visit. During today’s visit I observed the children during center play, small group activities and outdoor play. During today’s visit I observed diaper changes and infant feedings. Positive interaction was observed between the staff and the children. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. A sanitation inspection was conducted on October 15, 2024. This facility received a superior classification and two (2) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. A satisfactory fire inspection was completed on April 12, 2024. This facility was approved for daytime care only. This facility’s corporation status was verified current on the NC Department of the Secretary of State on February 7, 2025. The following violations of childcare requirements were cited today. Violation Number Comment Rule 468 When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. In classroom space #4 (three year old's) the art area did not have a sufficient quantity of materials available for the children to use in this activity area. .0510(d)(1) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There were outlets and power strips observed in several classrooms that were uncovered. 10A NCAC 09 .0604(c) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In the space for toddler children plastic bags were observed accessible to children. .0604(q) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by February 26, 2025. CONSULTATION AND RESOURCES: Technical assistance regarding how to achieve and maintain compliance was offered. Health & Safety Requirements were also reviewed. --During today’s visit I monitored your updated infant/toddler space (Playground #3). This space has been redesigned for use by the infant and toddler age children. Once the broken storage house has been removed from this space, the space can be used. Please send me an email when this has been removed. During today’s visit I reviewed with you the importance of having a system to monitor the expiration dates of the action plans, mediation authorization and the expiration dates of medicines. We discussed several options for monitoring the classrooms for compliance. STAFF TRAINING: All staff members that have responsibility for planning and supervision of a childcare program; as well staff who work directly with children must complete in-service training annually. --The required number of in-service training hours to be completed yearly is based on the education and/or experience of each individual staff member. --Please use the Record of In-service Training Chart to keep track of your training completed each year. (It is recommended that you use one sheet for each year). Also have at least two years available for review (current year and previous year). --If you receive more than the required number of hours required, you may only carry forward half of what you are required for the next year. *For a complete review of the number of annual training hours required please review child care rule.1103 (On-going training and professional development requirements). Upon my arrival, I was greeted by Ms. S. Jordan, the director. I informed Ms. Jordan the purpose of today’s visit. Ms. Jordan gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Eighty-one (81) children were present during today’s visit. They were observed playing in centers, washing their hands in preparation of lunch, eating lunch and napping. During today’s visit, Ms. Jordan informed me of a situation that happened at the facility. A staff member left a child on the playground. Ms. Jordan informed me these two employees are no longer are employed at the facility. I informed Ms. Jordan this will be a supervision violation. Today I did observed the children being supervised. The supervision violation was corrected during this visit. Ms. Jordan also informed the facility has a mandatory supervision training for all staff on Monday, September 18,2023. I informed Ms. Jordan to send me a copy of the attendance rooster for this meeting and I need a plan to ensure this will not happen in the future. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions CPR and First Aid Certification. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. A child was left unsupervised on the playground. .1801(a)(1-5) 1043 Six new staff records, except financial records, were not made available for review. G.S. 110-91( 9) The above violations must be corrected immediately, and a letter must be sent to me no later than September 28,2023 stating how each item was corrected.
