    Name of Operation: APPLEVILLE ACADEMY Facility ID: 41002249 Consultant: VACANT TEAM 7 Operation Type: Center Case Number: Visit Date: 8/13/2025 Number Present: 18 Completed Date: 8/14/2025 Age: From 1 To 5 Total Minutes: 82 Time In: 09:03 AM Time Out: 10:25 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a routine unannounced visit. The last AC visit was conducted in February 18, 2025. Jen Jones, Child Care Consultant, accompanied me during today's visit. We discussed that Jen will be your new consultant once she is done with her training. We observed ratios/children. One-year old's were playing inside and all centers were open. The older two-year-old's and three-year-old's were playing in centers in the classroom. Four-year-old's were also playing in classrooms. All classrooms had age-appropriate activities, and they were observed in each classroom for the children. The following items were monitored during today's visit: Supervision, Staff/Child Ratio, license posted, approved space, required trainings, items posted, permit restrictions, program records. There was one new staff member since the last visit and we checked that staff member files. The last fire drill was recorded on 07/16/25 with 1 minute evacuation time. The last shelter-in place was recorded on 06/27,2025. A Fire Inspection was conducted on 03/12/2025 and I have a copy attached to file. The last sanitation inspection was completed on 6/5/25 with a Superior classification and 8 demerits. A partial assessment of applicable child care requirements was conducted during today’s visit. There was one violation of childcare requirements cited today. The violation was corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet cover was not covered in the four and five year old classroom. This violation was corrected during the visit. 10A NCAC 09 .0604(c) Please continue to visit DCDEE’s website to get the latest information for child care at https://ncchildcare.ncdhhs.gov/. MOODLE has training courses that are free and will provide additional in-service hours. Technical Assistance We discuss: QRIS: For more important information regarding The North Carolina Child Care Commission on the Quality Rating and Improvement System (QRIS): How to Access the QRIS Modernization: 1. Visit the NC Division of Child Development and Early Education (DCDEE) website. 2. Navigate to the “What’s New” section. 3. Click on “QRIS Modernization” to explore resources, webinars, and full modernization plan. The QRIS Modernization introduces three pathways to earn a 2-5 Star Rated License: • Program Assessment • Classroom & Instructional Quality • Accreditation and Head Start These updates reflect feedback from over 1,800 early childhood professionals and aim to better support educators, families, and communities across North Carolina. Technical Assistance Criminal Background Checks (CBC) • Start Early: Encourage staff to begin the process at least six months in advance to account for any delays. We also discussed: ABCMS Portal Reminder: Add Your Staff Before the Next Visit: To remain in compliance with North Carolina’s criminal background check requirements, all child care providers must ensure that new staff are entered into the ABCMS Portal within five days or hire. 1. Complete Require Training: • Directors must complete the ABCMS Moodle training available through the DCDEE Criminal Background Check Unit. • DCDEE Criminal Background Check units. • After training ,submit the signed perform Powerform to request access to the provider Portal. 2. Log into ABCM Provider Portal • Visit the ABCMS Provider portal and log in using your Business NCID credentials. • If you don’t have access yet, contact the CBC Unit for support. 3. Add New Staff: • Go to the Application Menu and click “Add New”. • Name or DOB to begin the application. • Share your facility’s unique provider code with the staff so they can link their application to your center. 4. Fingerprinting & Documentation: • Ensure staff complete fingerprinting at an approved location. • Upload and required out-of-state background documents if applicable. Important Notes: • Background checks are valid for five years. • Staff must not begin work until they receive a valid CBC Qualification Letter. • Keep a copy of the letter on file and notify DCDEE of any new hires promptly. Reminder: Some staff CPR/First Aid training is expiring soon. You were given the opportunity to ask questions regarding interpretation and implementation of child care requirements.
