Name of Operation: Anne's Child Care Facility ID: 41002528 Consultant: ELIZABETH CHILDRESS Operation Type: Family CC Home Case Number: Visit Date: 6/19/2025 Number Present: 0 Completed Date: 6/19/2025 Age: From 0 To 0 Total Minutes: 180 Time In: 09:00 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Announced The purpose of today's visit was to monitor your program for compliance with applicable child care requirements during the second temporary time period visit. You, A. Davis, Owner/Operator, assisted me with the visit. This Family Child Care Home was issued a Temporary License on March 12, 2025. During this visit, we walked through the entire facility and observed the indoor and outdoor environment, but a full assessment was not completed for each space. The facility was assessed for supervision, staff/child ratio, and health and safety requirements. No children were present during the visit. Four children are enrolled. The following items were monitored during today’s visit: Supervision, Staff/Child Ratios, Approved Space, Criminal Background Checks, CPR/FA requirements, ITS-SIDS training, Permit posted and Permit Restrictions. Three violations were cited during today’s visit. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance).One staff member did not submit required forms for qualification letter. G.S. 110-90.2 & .2703(n) & (o) 1739 All records required shall be available during operating hours, for review by a representative of the Division. All posted items were not posted and/or available per Pre-Licensing Checklist for Family Child Care Homes. .1721(f)(7) 1831 A valid qualification letter was not on file and available for review at the facility. The Owner/Operator did not have a qualification letter on file. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) Child Care programs are expected to achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before July 03, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. We discussed the following items of Technical Assistance during today's visit: -Because you were previously qualified, but the qualifying letter has expired, DCDEE will allow you fifteen (15) days from our visit on 06/19/2025, to get the new criminal background check completed and a copy of the qualification letter on file. -You will not be able to enroll any children until this is corrected. Please be mindful, you can only enroll a total of six children. One school age and five preschools. (per local ordinances) I also provided guidance on the following: -I provided a sampling of additional items that you will need prior to your next visit. I also provided the Pre-Licensing Checklist again for your review. Please be sure all items are completed on that checklist. You can find all the other forms and documents needed at https://ncchildcare.ncdhhs.gov/ -You will need water for bottles and for children during the day. Parents can bring the water, but each container must be labeled with that child’s name. - Steps inside your home shall never be used during operation hours. Families must enter through the side entrance. -You can have a changing pad to change the children but please be mindful to always clean and sanitize after each use. -A child size table needs to be provided for the children to eat. The same table can be used for activities but must be cleaned and sanitized prior and after eating. -You can use the same sink for handwashing and food prep, but the sink must be sanitized between each use. -Each child needs their own individual space to store their belongings. -Children need to be able to get to the sink to wash their hands safely. At the completion of this visit, the visit summary was reviewed with you, and a copy was emailed to you. We appreciate all you are doing to serve the children and families of NC.
