The last Annual Compliance visit was conducted on 12/12/2023. An Annual Compliance Monitoring Checklist for FCCH checklist was used to note the requirements I monitored today. Meg Riddle, Supervisor Childcare License Consultant, accompanied me during this visit. Upon arrival we were greeted by Antonette Embree, Owner/Operator. This Family Child Care Home operates with a Three Star License effective 06/09/2017. Compliance score of 99% as of 10/29/2025. We walked through the indoor and outdoor licensed spaces of the home that the children use during the visit. We observed the children laying down during rest time. One child was up, and Ms. Embree interacted with the children in a pleasant manner. A full assessment of applicable child care requirements was conducted during today’s visit as per the attached checklist. Two violations were observed during today’s visit. Violation Number Comment Rule 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The FCCH provider did not complete their Health and Safety training every 5 years. .1703(d)(2) 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. The FCCH provider initiated environmental testing on 09/03/2025 but did not complete required procedures for lead in drinking water, lead-based paint, or asbestos. 10A NCAC 09 .1725(a)(2) The above violations must be corrected immediately, and a letter must be email to me no later than November 13, 2025 stating how the one item was corrected. If I do not receive your letter by November 13, 2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: EQulPD: We discussed training opportunities with EQuIPD and I left an informational document that you can use also coaching and consultation toolbox was on the document as well. We also discussed: Clean Water for US Kids: • Going to the clean water for US Kids website and completing lead water testing, I saw that your FCCH begin the process on 09/03/2024 for beginning lead in water, but did not continue, the website also showed that you enrolled in the testing for lead in paint, but did not complete the process, and you also enrolled in Asbestos testing but did not complete the process. You mention that you were leading towards Program Assessment Pathway and Classroom and instructional Quality Pathway. Recognizing and Responding to Suspicions of Child Maltreatment training is located on this website, https://www.preventchildabusenc.org. Reminder: Recognizing and Responding to Suspicions of Child Maltreatment training is due next year April 2026. Ms. Ericka Bailey accompanied me on the visit. We were greeted by the owner, Ms. Antonette Embree. This Family Childcare Home currently operates with a Three-Star License effective June 9, 2017. The last annual compliance visit was conducted on November 06, 2024. Five children are enrolled, and four children were attending today. The children were resting in the childcare room. The childcare room has activities and toys for the children. The ate lunch and transitioned to nap time. Ms. Embree has items for the children for outdoor play. The children play on side yard and covered porch. The children play on the covered and gated porch for the time being. All interactions we observed today were warm and positive. I reviewed Ms. Embree's Verification of Required Information for Operator form from the Annual Compliance Visit. Ms. Embree has an expired CBC qualification letter. Special training is current. Ms. Embree does not have any guns in the house, she does not have any pets, nor does she provide any transportation. One item of noncompliance was observed during today’s visit. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). Ms. Antoinette had difficulty updating her criminal background check by the five year expiration date. G.S. 110-90.2 & .2703(n) & (o) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by April 1, 2025. Include the following information in your correction letter: 1.Facility Name 2.Facility ID 3.Date of the letter 4.Each violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. If I do not receive your letter by April 1, 2025, you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Reminder: •CPR/First Aid training is due at the end of October 2025. •Please remember that you need to complete another application for criminal background check and get your fingerprints done. Hold harmless has been extended until the new QRIS is implemented. This means that you do not need to schedule an assessment of your rated license during your cohort year. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements.
