    Name of Operation: Above and Beyond Christian Daycare Facility ID: 41002560 Consultant: MARY MCMILLAN Operation Type: Center Case Number: Visit Date: 4/23/2026 Number Present: 0 Completed Date: 4/23/2026 Age: From 0 To 0 Total Minutes: 225 Time In: 11:00 AM Time Out: 02:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a Temporary Time Period Visit. The license was issued on 1/1/26, due to a change of location. Today I met with Janice Johns Ingram. Upon arrival 4 classrooms, with children, were observed during lunch and transitioning from lunch to nap time. Limited items were monitored: staff/child ratio, supervision, approved space, hazardous storage, age-appropriate activities, required items, one new staff file, program records and permit posted. The last fire drill was recorded on 3/2/26 at 9:54 with 3.5 minutes evacuation time. The last playground inspection was recorded on 3/19/26. The following violations were cited during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In two classrooms with preschool children the activity plan was for the week of April 13th. GS 110-91(12); .0508(a) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. There were foam blocks in the block center in the classroom with 2 and 3 year olds present. .0604(q) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One new staff member did not have orientation completed and was hired 2/16/26. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member did not have approved training within 90 days of hire and the staff was allowed to return to work without the training. This is a repeated violation from 2/12/26. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member did not have approved training within 90 days of hire and was allowed to return to work without the training. This is a repeated violation from 2/12/26. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One new staff member did not have the training within 90 days of hire. .1102(g) During today's visit we discussed the following items of technical assistance: -CPR and FA must be completed, as required, for the safety of the children all staff members must have current CPR and FA trainings. This staff members, J. Brown, CPR and FA was also not completed during my last visit and the violations were cited and in the compliance letter, that the provider sent, stated the training was going to be taken on March 20, 2026 and that J. Brown would not be allowed to return without the training. The staff member, J. Brown, was present and caring for children during today's visit and did not take the trainings. -Foam blocks or any other type of foam/Styrofoam is not allowed within reach of children under 3 years old, due to it being a choking hazard. Unless the teachers have a supervised activity. -Recognizing and Responding to child maltreatment training must be taken within the required 90 days of hire for the safety of the children and recognizing signs/symptoms. -Lesson/Activity plans must be current to know what is being taught and the parents to be aware of activities being done in the classroom. -Staff orientation must be completed within 6 weeks of hire. The first 2 weeks have specific topics that must be covered and then the other remaining topics in the 4 weeks after that. This is for the administration to know that the staff member understands the topics. The following items of consultation were reviewed today: -Staff education in our WORKS system shows that only a few staff members have education and/or education needs to be updated. During my last visit on 2/12/26, we discussed that education must be submitted to WORKS unit if staff needed it to be entered/updated. -If education is not in the system to meet the requirements for the star level that you wish to meet, then we will have to use whatever education that has been verified and you may drop below 3 stars, if only the current education is in the system. -I reviewed pathway 1 and pathway 2 along with all requirements for each pathway with Mrs. Janice, since she stated that she didn't go to the trainings and didn't understand them. After my review of each pathway, Mrs. Janice stated that she would like to meet the pathway 2- Classroom and Instructional Quality. She stated that she understands the requirements for 3 stars for this pathway and she received copies of all requirements that must be met, as listed on our DDCEE website under the QRIS modernization tab. Mrs. Janice stated that she understands that she must decide on a curriculum for all ages and the staff must be trained on it, and she can use these things for her CQI for the center and each staff member for this year. -We discussed setting up a filing system that works best for you, and using the staff files checklist and the staff and training worksheets along with the children's files checklist to organize paperwork by. All violations must be corrected immediately and a compliance letter, stating how the violations are corrected, must be received no later than 5/7/26. You can email me the letter to mary.mcmillan@dhhs.nc.gov All items must be in compliance before you can apply for a star rated license. We discussed that once all violations are corrected you can send in your rated license application, with all required forms but no later than June 1, 2026. The application can be found on our DCDEE website under provider documents tab then "Application for rated license" link and the other forms are embedded in the application for you to click on and save and email those as well. Reminders: When you receive your Child Care License from the State of NC, you are agreeing to comply at all times with the Law, Rules and Requirements pertaining to and governing Child Care in NC. It is your responsibility to read, learn, and maintain all Child Care Requirements that apply to your licensed facility. It is also your responsibility to ask Division staff questions for clarification of requirements you do not understand, and to monitor your staff to ensure that all requirements are maintained at all times. -You must have at least a 75% or higher compliance history in order to apply for a star license and at the end of your Temporary Time period you must met the requirements to be at least a Three Star child care license in order to be eligible for DSS vouchers. After today's visit your compliance history is at 80%. Upon arrival the children were observed during rest time. I met with Co-Director Tiffanie Malzon. I conducted a walk through of the classrooms and Staff/child ratios were in compliance during today's visit. There was 1 infant present and sleeping in a bouncy seat during my walk-through. Once I asked the teacher, Ms. Jashika, about the infant being asleep in the device, she removed the infant and was holding the baby. The following violation was cited and corrected during today's visit. Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. There was an infant asleep in a bouncy seat, instead of a crib or cot. 10A NCAC 09 .0606(a) I discussed the following technical assistance information with Ms. Jashika and Ms. Tiffanie: -SIDS policy states that all infants must be placed in a crib, cot or mat to sleep. Infants under 12 months cannot sleep in a bouncy seat or other device, for the increase risk of SIDS. Reminder: When you receive your Child Care License from the State of NC, you are agreeing to comply at all times with the Law, Rules and Requirements pertaining to and governing Child Care in NC. It is your responsibility to read, learn, and maintain all Child Care Requirements that apply to your licensed facility. It is also your responsibility to ask Division staff questions for clarification of requirements you do not understand and to monitor your staff to ensure that all requirements are maintained at all times. Today I met with Angelica Johns and Janice Ingram was present upon arrival. Upon arrival the children were observed during rest period. I conducted a walk-through of all 4 classrooms being used. Limited items were monitored during today’s visit: Supervision, Staff/Child Ratio, required items posted, permit restrictions, approved space, 4 children’s files, CBC qualification letters, required trainings and program records. No fire drills were recorded since the change of location occurred on 1/1/26. The last playground inspection was recorded on 1/15/26. The following violations were cited during today’s visit: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In the classroom with infants and 1-year olds, 7 children were present with only 1 teacher. GS 110-91(7);.0713(a-d) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There were no fire drills since the license was issued on 1/1/26. .0604(t); .0302(d)(5) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. G.S. 110-91(9) We discussed the following items of technical assistance during today’s visit: -Upon monitoring the classroom with infants and 1-year olds, there was only 1 teacher present with 7 children and the teacher and Ms. Ingram stated that the other teacher was out of the room taking her lunch break. Staff can ONLY reduce to one staff member while children are resting in classrooms with children ages 2 and up. If you have any children under 2 years of age in a classroom, those teachers must have a staff member come into the classroom and cover their breaks, since ratio must ALWAYS be maintained, even at naptime, for this age group. -Fire drills are required to be completed every month and I recommend completing them the same day that you complete your monthly playground inspection forms. -At all times, all children’s files must be available for review by a DCDEE representative and the staff that need access to them for emergency and other information, if required. -CPR and FA must be completed prior to the expiration date AND for new staff must be completed within 90 days of being hired (if it no longer valid,) This is for the safety of the children. Reminders: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff and children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. -Mrs. Ingram and Ms. John’s CBC qualification letters expire 2/19/26. -Ms. Brown’s CBC qualification letter expires in August 2026, the process for CBC can be submitted as early as 5-6 months before the expiration date. We discussed the following items of consultation during today’s visit: -Once the outside playground items are corrected and all other items are in compliance, you can re-apply for your star rated license. -You can find all child care pathway requirements on our website ncchildcare.ncdhhs.gov under the child care rules in section .2800 and the handouts explaining the requirements under the what’s new tab and the QRIS modernization tab. We reviewed the overview for Pathway #2 -Classroom and Instructional Quality and what it requires -The application, which includes the links for the additional forms, is found on our website under the provider documents tab, then Application for star rated license. We discussed the Pathways to the stars and Ms. Johns stated that they are interested in the curriculum and instructional quality pathway. We discussed that you must have an approved curriculum and approved assessment prior to the end of your Temporary License, and all application paperwork must be completed and received by June 1, 2026. All administrators and Lead Teachers must complete training on the curriculum and assessments, as in child care rules -Staff education must be updated, if needed, or submitted to WORKS education unit by May 15, 2026 at the latest. Your staff’s education must be verified by our WORKS unit prior to the stars being re-issued. Each staff member must register in the WORKS system as soon as they are hired at this location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment. -We discussed utilizing the requirements that you will need to do to meet a 4 or 5 star license for your family and community engagement and/or CQI requirements for the facility and each individual staff member. -You must meet the education level for 50% of lead teachers and 50% of all other staff for whichever star level that you are applying for.
