    Name of Operation: 4Ever Mema Kings Kidz Facility ID: 41002542 Consultant: MARY MCMILLAN Operation Type: Center Case Number: Visit Date: 8/20/2025 Number Present: 18 Completed Date: 8/20/2025 Age: From 0 To 8 Total Minutes: 120 Time In: 10:15 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a Temporary Time Period visit. This facility had an ownership change on 7/1/25. Ericka Bailey, Child Care Consultant, accompanied me during today’s visit. This is the first visit conducted and today we met with Aseelah Battles, Owner representative and Director. We observed all classrooms and the children were engaged in playing in centers and outside play. The following items were monitored during today’s visit: Supervision, staff/child ratio, approved space, age appropriate activities, items posted, hazardous items storage, medication storage, safe environment both inside and outside, 1 new staff file and 1 child’s file. The teachers were observed caring for the children in an age appropriate manner. There were hazardous items of disinfectant wipes and bottle of cleaner observed on the teacher’s desk and in an unlocked cabinet with cleaning supplies, roach spray, disinfect aerosol spray in the classroom with school-aged children. The mats were stored all on top and sideways of each other with the sheets on and directly touching other children’s sheets. Foam blocks were observed on a shelf in both classrooms with children ages 2 years old, along with first-aid ointment and alcohol wipes within reach of the preschool children in the first aid kit. The last fire drill and playground inspections were documented in June 2025. Outside the fence along the woods and backside of the playground was not the minimum requirement of 4 feet tall, some areas measured 3 feet 4 inches. The stationary climbing structure that was over 18 inches in climbing height did not have any mulch under or around it, only dirt. There were weeds and vines that were coming through the fence and on the playground that appeared to be poison sumac. The fence has protrusions along the top and some holes and sharp rusted areas along with bottom and sides of the fence. The following violations were cited during today's visit: Violation Number Comment Rule 615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. The mats were stored with sheets on, stacked directly on top of each other and sideways. 15A NCAC 18A .2821(a) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last fire drill was recorded in June 2025. .0604(t); .0302(d)(5) 807 A safe indoor and outdoor environment was not provided for the children. Indoors: There were foam blocks in the 2 classrooms with children under 3 years old on a shelf in the centers, and a first aid kit with first aid ointment and alcohol wipes on a low shelf within reach of the children. Outdoors: There was rust, sharp edges and holes in the playground fencing. The climbing structure that is anchored and over 18 inches tall did not have any mulch underneath or around it. There were weeds/vines coming through the fence and on the playground that appeared to be poisonous plants. 10A NCAC 09 .0601(a) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. Fence had protrusions around the top and measured 3 feet 4 inches along one side. GS 110-91(6); .0605((i) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There were cleaning chemicals, including disinfectant wipes and aerosol sprays, cleaning sprays and roach spray in a cabinet with only a paperclip used to keep it closed in the room for school-aged children. .2820(b) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last playground inspection was completed in June 2025. .0605(q) We discussed the following items of technical assistance today: -options of how to correct the fencing issues -Foam toys, activities, bowls or cups are not allowed in classrooms with children under 3 years old. Plastic bags should be removed from areas that children under 4 years old have access to when they are not full of diapers/pull-ups -requirements for mulch We discussed the following items of consultation during today’s visit: -QRIS pathways and you can find more information about this on our DCDEE website at ncchildcare.ncdhhs.gov under the “what’s new” tab and then QRIS tab. Please review each of the pathways to decide which option that you would like to achieve, for a 2-5 star license. -Staff and children’s files checklist were discussed and all items must be on file before the next visit. -A sample compliance letter will be emailed to you and a compliance letter training will be conducted on 8/22/25 along with a rules review. You will have 2 more temporary time period visits before January and your pathway to the stars information will need to be completed by the first week of December. -A provider training will be conducted in September 2025, about the QRIS requirements. More details will be coming about this training/meeting. Please know that with a child care license you must know and follow all child care laws and rules, even if they have not been reviewed with you. You can find all of these on our website at ncchildcare.ncdhhs.gov You must correct your violations immediately and a letter of compliance is required by 9/3/25, stating how the violations were corrected. If the violations were corrected during the visit, you do not have to include those items.
