    Name of Operation: OAKWOOD ELEMENTARY YMCA CHILDCARE Facility ID: 18000637 Consultant: CAROLYN CONLEY Operation Type: Center Case Number: Visit Date: 10/7/2025 Number Present: 22 Completed Date: 10/7/2025 Age: From 5 To 11 Total Minutes: 170 Time In: 02:10 PM Time Out: 05:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor your compliance with all applicable child care requirements during the first Temporary Time Period (TTP#1) visit. Upon arrival, I was greeted by school office staff, and they contacted you, Heather Price, Program Coordinator, to meet me in the office. We proceeded to your office which is located in the gym. You stated that this program uses the gym, cafeteria and the front playground. Currently, this facility operates with a temporary license issued from August 9, 2025, through February 9, 2026. The following restrictions are listed on the license: daytime care only and school age children only. You stated that the before school hours are 6AM-8AM and the afterschool hours are 3:28PM-6:00PM. I was able to monitor your file, the program’s file and a portion of the student’s files prior to them arriving in the gym area. Ms. Kimberly Curtis, administrator, arrived as I was completing my walkthrough of the cafeteria. She stated that she had emailed me the signed Staff and Training Worksheet and also gave me signed copies of the Staff and Training Worksheets for two other sites before she left. The facility’s temporary license, NC Summary of Child Care Law poster, safe arrival and departure procedures, emergency medical care plan, evacuation plan, daily schedule, activity plan, and emergency phone numbers were posted on a bulletin board outside the cafeteria and gym. The Classroom Staff to Child Ratio charts was also located on the bulletin board. We discussed that this would need to be placed in each space used by the children (gym and cafeteria). The last monthly fire drill was completed on September 30, 2025. The gym, cafeteria, outdoor play area and bathrooms used by this afterschool program were monitored. The students were observed during arrival procedures, playing dodge ball or participating in dramatic play in the gym. They were also observed transitioning to the cafeteria for snack, completing homework and free choice activities and departure procedures. The families were observed signing out the students on the facility's cell phone using Brightwheel and you signing them out on the sign in/out paper document. Supervision, adequate approved space use, capacity and group size, staff/child ratios and permit restrictions were monitored. I monitored hazardous product storage, general safety, and discipline practices. Ms. Price stated there is no medication on site at this time. Kimberly Crane, Child Care Licensing Consultant, reviewed the QRIS/Pathways documents with Ms. Simmons and Ms. Curtis during her visit she conducted at Viewmont Elementary YMCA Program on September 16, 2025. If this program to apply for the two through five-star license, please access the fillable document, “Application for Assessment for a Rated License for Centers” through the DCDEE website at this link: https://ncchildcare.ncdhhs.gov/Provider/Provider-Documents-and-Forms. A compliance history score of seventy-five percent or above must be maintained to receive a rated license. The following violation was cited and corrected during today's visit: Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. The staff/child ratio was not posted in the cafeteria. .0713(a)(10), (c) & (f)(3); .2818(e) TECHNICAL ASSISTANCE: -It can be challenge when the out of school program shares the space with the school. It is important that out of school staff monitor the spaces prior to the students arriving to ensure that required information is posted included but not limited to staff child ratio signage. CONSULTATION: -We discussed the importance of monitoring all spaces prior to the students arriving to ensure that all hazardous products including but not limited to cleaning supplies, hand sanitizer, etc. -We discussed that the Emergency Medical Care Plan will need to be updated because there is a staff member that is no longer at this site listed. -It was observed during the transitioning time between leaving the gym, working with a student that was having a difficult time during the transition, students using the restroom and entering the cafeteria that it is a challenge for one staff member as students have to wait in the hallway before entering the cafeteria. It was also observed that it is a challenge for one staff member to to be in the cafeteria and having to let families into the building through the hallway door right outside the cafeteria. This may lead to a supervision concern. It may be helpful to have an additional staff member to assist during this transition time and as families are coming to pick up their children. It may also be helpful to have families use the outside door that leads directly into the cafeteria. -All staff must have their current education evaluated by the WORKS Unit in Raleigh. Instructions for the DCDEE WORKS system, including establishing a NCID, is located on our website at https://ncchildcare.ncdhhs.gov/. Remember to track the date information is submitted by staff. If you have questions regarding submitting information in the WORKS system, contact WORKS at 1-800-859-0829. -Have staff visit the North Carolina Rated License Assessment Project’s website: ncrlap.org. There are training and resources available for the Environmental Rating Scale 3 (ERS3) assessments. -Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. -For continued eligibility in the subsidized childcare program, you must achieve a three star or higher rated license at the end of the temporary license period. -As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days.
