Upon arrival, I was greeted by Denise Brice who stated that she is now the Program Coordinator for this site effective yesterday. She transferred to this site from Southwest Primary YMCA Childcare where she was a Group Leader. I introduced myself and the reason for my visit. Danyalle Dooley, Group Leader, was also present at the time of my visit. Students were observed during departure procedures, some playing basketball and others were observed coloring with markers in the gym area. I requested to see Ms. Brice’s file and she stated that she does not have access to it. I contacted you, Kimberly Curtis, Regional Programing Director, and you stated that you will be at the site shortly. Once you arrived you pulled up her file electronically and I monitored the documents. You also gave me an updated Staff and Training Worksheet during today's visit. Currently, this facility operates with a temporary license issued from August 9, 2025, through February 9, 2026. The following restrictions are listed on the license: daytime care only and school age children only. This facility’s temporary license, NC Summary of Child Care Law poster, Classroom Staff to Child Ratio chart, safe arrival and departure procedures, emergency medical care plan that will need to be revised to include the current staff members, evacuation plan, daily schedule, activity plan for the week of December 15, 2025, a menu for the month of December and emergency phone numbers were posted in a prominent area in the hallway outside the gym. The last monthly fire drill documented on the Fire Drill Log dated November 13, 2025. The last emergency drill documented on the Emergency Drill Log was a lock down drill dated December 8, 2025. Supervision, adequate approved space use, capacity and group size, staff/child ratios and permit restrictions were observed and in compliance. I monitored hazardous product storage, general safety and discipline practices. Staff stated that there are no children on medication This program does not provide transportation at this time. RATED LICENSE ASSESSMENT INFORMATION: During the visit conducted on September 16, 2025, we discussed the three QRIS Pathways to the Stars effective July 1, 2025. We covered staff education (including ensuring all staff have WORKS accounts, understanding the 50% requirement for the administrator and for other educators, and how work experience may count), enhanced ratios and space expectations, the Family and Community Engagement Foundational Practices, and Continuous Quality Improvement (CQI) Plans. We discussed if you want the programs to be eligible for a 4 or 5 star rated license at any or all of the YMCA Childcare locations in Catawba County, you may move forward with the ‘Classroom and Instructional Quality (Pathway 2)' option with the plan to use the curriculum you have submitted for approval. However, you will need to choose a curriculum that has already been approved as a back-up plan just in case your curriculum (that is under review) is not approved before your rated license assessment is processed. Two reminders: • Formative Assessments aren’t required for SA only programs. • Rule .3205(d)(5), ‘Classroom and Instructional Quality (Pathway 2)’, three-star level doesn’t require an approved curriculum to be used. You can use any curriculum when earning a three-star rating. “ Here is a review of the steps that need to be completed for the “Classroom and Instructional Quality Assessment (Pathway 2)” option which was also discussed on December 2, 2025 and during the visit conducted on December 16, 2025: If you choose Pathway 2: • Complete the “Application For Assessment for a Rated License for Centers” electronically and submit to your child care consultant no later than, January 9, 2026. • Confirm all staff have active and up to date WORKS accounts. • Complete the QRIS Staff Information and Education Worksheet electronically and submit to your me no later than January 9, 2026. • Identify Continuous Quality Improvement (CQI) goals for the facility and individuals and begin implementation and documenting ongoing progress toward achieving the goal(s). Complete the Facility CQI Plan form electronically and submit to me no later than January 9, 2026. • Identify Family and Community Engagement practices. Complete the “Family and Community Engagement” form electronically and submit to me no later than January 9, 2026. • Select and implement a curriculum. Per child care rule .3205(f)(10) pertaining to coaching, mentoring, and training options for the administrator, the administrator should plan to participate in one of the following activities regarding classroom and instructional quality practices: 1. annual completion of five hours of coaching or mentoring by an administrator of another licensed child care center with a star-rating of five stars, or a community resource partner; 2. annual completion of five on-going training hours, in additional to applicable requirements in child care rule .1103 and child care rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218; or 3. annual completion of 0.5 continuing education unit, in addition to applicable requirements in child care rule .1103 and child care rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218.2. • (OPTIONAL) Per child care rule 10A NCAC 09 .3224 (a & b) Recognition Of Quality Initiatives, a child care operator may choose to request one or more areas of recognition to accompany the issuance of a two through five-star rated license earned through compliance with any licensure pathway requirement of this Section. Quality initiatives will not be used to earn a star rating. Verification of requirements for each recognition shall be reassessed at least every three years, at the time of reassessment for a two through five-star rated license. The following violations were cited and two were corrected during today's visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted was from the week of December 15, 2025. GS 110-91(12); .0508(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu posted was for the month of December. 10A NCAC 09 .0901(b) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last documented fire drill was conducted on 11/13/25. .0604(t); .0302(d)(5) TECHNICAL ASSISTANCE: -Place a reminder on the calendar to conduct a monthly fire drill and document the information on the required for as soon it is completed. Conduct the drills at different times of the day. You could conduct one during before school hours, after school hours and on days you the students all day. -Ms. Brice stated that she attempted to print of the activity plan today but was unsuccessful due to printer errors and that this was her 2nd day. Staff should ensure that when they first arrive for coming week that the activity plan was posted. You stated that yesterday was the first day back for the children and staff. This was also discussed during the visit conducted on December 16, 2025. -Make sure that the monthly menu is posted by the first day of the month that the staff and children return. CONSULTATION -Ms. Brice and I reviewed the daily schedule posted and the activities that were occurring. She stated that she is in the process of revising it now that students are back from Christmas break. -We discussed that the Emergency Medical Plan will need to be revised because of the change of staffing. -Star Rated License Child Care programs are expected to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent at all times [NC GS 110-90(4)(d)]. This facility’s compliance history prior to today’s visit was 96% and was reviewed with you today. The violation(s) documented must be corrected immediately. On or before January 21, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. While the Division allows you time to explain how you have corrected violations when violations are cited, it is expected you correct all violations immediately. When you are asked to send a compliance letter to me, it is expected and required that your compliance letter be sent to me no later than the date indicated in this documentation. You could conduct one during before school hours, after school hours and on days you the students all day. -Ms. Brice stated that she attempted to print of the activity plan today but was unsuccessful due to printer errors and that this was her 2nd day. Staff should ensure that when they first arrive for coming week that the activity plan was posted. You stated that yesterday was the first day back for the children and staff. Upon arrival, I was greeted by Denise Brice who stated that she is now the Program Coordinator for this site effective yesterday. She transferred to this site from Southwest Primary YMCA Childcare where she was a Group Leader. I introduced myself and the reason for my visit. Danyalle Dooley, Group Leader, was also present at the time of my visit. Students were observed during departure procedures, some playing basketball and others were observed coloring with markers in the gym area. I requested to see Ms. Brice’s file and she stated that she does not have access to it. I contacted you, Kimberly Curtis, Regional Programing Director, and you stated that you will be at the site shortly. Once you arrived you pulled up her file electronically and I monitored the documents. You also gave me an updated Staff and Training Worksheet during today's visit. Currently, this facility operates with a temporary license issued from August 9, 2025, through February 9, 2026. The following restrictions are listed on the license: daytime care only and school age children only. This facility’s temporary license, NC Summary of Child Care Law poster, Classroom Staff to Child Ratio chart, safe arrival and departure procedures, emergency medical care plan that will need to be revised to include the current staff members, evacuation plan, daily schedule, activity plan for the week of December 15, 2025, a menu for the month of December and emergency phone numbers were posted in a prominent area in the hallway outside the gym. The last monthly fire drill documented on the Fire Drill Log dated November 13, 2025. The last emergency drill documented on the Emergency Drill Log was a lock down drill dated December 8, 2025. Supervision, adequate approved space use, capacity and group size, staff/child ratios and permit restrictions were observed and in compliance. I monitored hazardous product storage, general safety and discipline practices. Staff stated that there are no children on medication This program does not provide transportation at this time. RATED LICENSE ASSESSMENT INFORMATION: During the visit conducted on September 16, 2025, we discussed the three QRIS Pathways to the Stars effective July 1, 2025. We covered staff education (including ensuring all staff have WORKS accounts, understanding the 50% requirement for the administrator and for other educators, and how work experience may count), enhanced ratios and space expectations, the Family and Community Engagement Foundational Practices, and Continuous Quality Improvement (CQI) Plans. We discussed if you want the programs to be eligible for a 4 or 5 star rated license at any or all of the YMCA Childcare locations in Catawba County, you may move forward with the ‘Classroom and Instructional Quality (Pathway 2)' option with the plan to use the curriculum you have submitted for approval. However, you will need to choose a curriculum that has already been approved as a back-up plan just in case your curriculum (that is under review) is not approved before your rated license assessment is processed. Two reminders: • Formative Assessments aren’t required for SA only programs. • Rule .3205(d)(5), ‘Classroom and Instructional Quality (Pathway 2)’, three-star level doesn’t require an approved curriculum to be used. Upon arrival, I was greeted by school office staff who directed me to the gym/cafeteria area where the afterschool program takes place. Danyalle Doolley, Group Leader, arrived at 3:10. She contacted the Program Coordinator, and she stated that she would not be at the site today. I requested to see Ms. Doolley’s file and the children’s files and she stated that she does not know where the files are kept. She contacted you, Kimberly Curtis, Regional Programing Director, and you stated that you will be at the site shortly. Once you arrived you pulled up the staff files and were able to locate the children’s files. Currently, this facility operates with a temporary license issued from August 9, 2025, through February 9, 2026. The activity plan posted was for the week of December 8th. Ms. Doolley showed me activities written on a paper on clip board but she was unaware what was planned for today. You found the current plan and posted it during today’s visit. The last monthly fire drill was completed on November 13, 2025. You and I discussed that a fire drill will need to be completed by the end of this for the month of December. The gym, cafeteria, outdoor play area and bathrooms used by this afterschool program were monitored. The students were observed during arrival procedures, gross motor and free choice activities in the gym and outdoors, departure procedures, and eating snack. Supervision, adequate approved space use, capacity, group size, staff/child ratios and permit restrictions were monitored. You stated that there are no medications at this program. A portion of the student files were monitored. There is one staff member hired since my last visit, and I was able to monitor her file virtually. I received the Staff and Training Worksheet prior to today’s visit, and I monitored the WORKS database. There was no record in WORKS that the two staff members, K. Joyner and D. Doolley, have accounts. I discussed this with you today. During today’s visit we looked up all the Hickory out of school staff, and you made a list of all what is needed. I stated that I would email you the WORKS status letters for the staff that had them. It is very important that all staff have an account, submit their HS diploma/GED, BSAC, official transcripts and request to be evaluated as a group leader and program coordinator. The WORKS evaluations is how we determine the stars that will be awarded for education. You and I spoke today by phone, and you stated that Pathway #2 Classroom and Instructional Quality Assessment was the option that would be used. We reviewed the email and attachments that I sent to you on December 3rd. We discussed by phone and during today’s visit that you will need to submit the following to me by the end of the business day Tuesday, December 23rd: • Complete the “Application For Assessment for a Rated License for Centers” • Complete the QRIS Staff Information and Education Worksheet • Complete the Family and Community Engagement Form • Select and implement a curriculum. (4-star and 5-star centers must use an approved curriculum and assessment for all ages.) • Identify CQI goals for each program planning for implementation form By January 5th all staff should have completed their CQI goal form and have it placed in their file for review. You will want to include on your CQI Plan the additional training or coaching hours you will receive annually. A one-star license will be issued if you do not plan to apply. Please be mindful that programs with less than a three-star license will not be eligible to receive subsidy funding. Your temporary license will end after February 9, 2026. A compliance history score of seventy-five percent or above must be maintained to receive a rated license.
