Kimberly Curtis, Off-Site Administrator, and Kayla Cushing, Program Coordinator, assisted me with today’s visit. Ms. Cushing signed the visit summary today. This program operates with a four-star license, issued on February 10, 2026 by meeting the requirements for Pathway to the Stars: Classroom and Instructional Quality (Pathway 2). This facility’s compliance history score was 98% as of May 4, 2026. The license, NC Summary of Child Care Law poster, Classroom Staff to Child Ratio chart, safe arrival and departure procedures, emergency medical care plan, evacuation plan, daily schedule, activity plan, menu and emergency phone numbers were posted on a “Parent Information Board” in the cafeteria. The last monthly fire drill documented on the Fire Drill Log dated April 27, 2026. The last emergency drill documented on the Emergency Drill Log was a shelter-in-place drill dated April 20, 2026. The lead water test results dated July 8, 2025 indicated that water at W.M. Jenkins Elementary School is within acceptable limits. The lead-based paint test results for W.M. Jenkins Elementary School are pending. Resources indicated that W.M. Jenkins Elementary School is exempt from asbestos testing with no additional data available. A current and posted menu documented appropriate nutritious snacks and meals, showed a variety of foods, and complied with Meal Patterns for Children in Child Care Programs. Today’s snack met the meal pattern requirements. I completed a walkthrough of the indoor environment. Students arrived in the gymnasium where they played a group game of dodge ball and then transitioned to bathrooms to wash hands in preparation for snack. Students ate snack in the cafeteria and transitioned to the playground. Students played outdoors. You stated the students would return to the cafeteria for a planned activity documented on the activity plan and free choice of activity materials. You stated students would return to the gym for a planned and organized game documented on the activity plan prior to departure. I monitored supervision, adequate approved space use, capacity and group size, staff/child ratios and permit restrictions and observed these being maintained during today’s visit. I monitored hazardous product storage, medication storage, medication administration, general safety, and discipline practices. I monitored program records, children’s records, and staff records. A Staff and Training (S&T) Worksheet was available for review and included in this visit summary packet. Please keep the Staff and Training Worksheet current and available for review by DCDEE staff. I understand that this facility does not provide transportation. As discussed today, this facility completed the star rated license assessment process in February 2026 by completing the requirements for Pathways to the Stars: Curriculum and Instructional Quality. Today, I completed an informal evaluation of this facility’s four-star rated license to verify that the required components for the Pathways to the Stars: Curriculum and Instructional Quality license listed below were being maintained as expected: 1. Family and Community Engagement Practices: • Foundational: The family and community engagement foundational practices were documented in the family handbook and were in process. • Communication Practice 1: “Bright Wheel” was used daily as means of two-way communication between the facility and the families. • Engagement and Leadership Practice 3: A “Family Festival” is being planned and will be held on May 16, 2026 at the local YMCA campus as an end-of-the-school year celebration and a summer orientation family event. • Engagement and Leadership Practice 6: Family surveys were sent out earlier in 2026. • Educational Opportunity 1: Plans were pending to implement the family and community engagement educational opportunity option 1. 2. Facility Continuous Quality Improvement Plan: The facility continuous quality improvement plan was in place and in process. 3. Individual Continuous Quality Improvement Plans: Individual continuous quality improvement plans were on file and in process. 4. Approved Curriculum: I observed, SCALED, the approved curriculum of choice for this facility, being implemented by the activities documented on the activity plan and being implemented with students today. 5. Four Star Education Standards: The administrator, program coordinator, and group leaders assigned to this facility during the rated license assessment process in February 2026 had not changed. During the on-going monitoring of the rated license components listed above, I determined this facility continues to maintain a four-star rated license through ongoing compliance with the Pathways to the Stars: Curriculum and Instructional Quality (Pathway 2) requirements. The following violation was observed, cited, and corrected during today's visit: Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. An emergency drill was conducted December 17, 2025 making the next emergency drill due during the month of March 2026. The next emergency drill was conducted on April 20, 2026. .0604(u);.0302(d)(8) TECHNICAL ASSISTANCE: 1. Per child care rules 10A NCAC 09.0604(u) and .0302(d)(8), emergency drills (shelter-in-place or lockdown drills) should be practiced every three months and recorded on the drill log. During today’s visit, I observed that an emergency drill was conducted December 17, 2025 making the next emergency drill due in three months during the month of March 2026. However, the next emergency drill was conducted on April 20, 2026 resulting in a violation. Because the emergency drill was conducted prior to today’s visit, the violation is considered corrected during today’s visit. To maintain compliance with this child care requirement, I suggest you set a reminder on your phone to conduct a shelter-in-place or lockdown drill every three months and continue to record the drill on the emergency drill log. CONSULTATION: 1. Per NC General Statute 110-102, the current version of Summary of North Carolina Child Care Law should be posted in a prominent place in the facility and included in the enrollment packet to be given to parents prior to the child’s first day of attendance. The most current Summary of North Carolina Child Care Law was published in November 2025. Today, I observed the date of the posted Summary of North Carolina Child Care Law was September 2023 and February 2025. Please print the updated version, post it in your lobby, and add it to your new enrollment packets. I also suggest you regularly check the DCDEE website for updated versions and especially when you receive notification of child care or sanitation rule updates. 2. While the Classroom Staff to Child Ratio Sheet for the cafeteria space was posted as required, the worksheet did not reflect the new four star license requirement for meeting voluntary enhanced staff/child ratio and group size. Please update this worksheet to reflect the licensed expectations. 3. Please print the facility continuous quality improvement plan and place a copy with your program records. Progress towards completing the documented goal and the establishing of new goals will be monitored at each visit. 4. During today’s visit and safety inspection of the playground used as the outdoor play space for this facility, I observed an area of the interior fence along the left-hand side of the basketball court that was pushed out from the bottom and with broken fence links causing barbs and sharp fence links to be exposed. While not a violation today because no children were observed playing on that side of the basketball court, I advised you to keep your groups on the right-hand side of the basketball court for safety purposes. 5. Upon arrival, I was greeted by school staff who directed me to the office in the library used for the school aged program staff. I was greeted by you, Kalli Kubik, Coordinator, and Rachel Webb, Group Leader. Currently, this facility operates with a temporary license issued from August 9, 2025, through February 9, 2026. The following restrictions are listed on the license: daytime care only and school age children only. The facility’s temporary license, NC Summary of Child Care Law poster, Classroom Staff to Child Ratio chart, safe arrival and departure procedures, emergency medical care plan, evacuation plan, daily schedule, activity plan, menu and emergency phone numbers were posted on a bulletin board in the cafeteria. The last monthly fire drill was completed on October 30, 2025. The gym, cafeteria, outdoor play area and bathrooms used by this afterschool program were monitored. The students were observed during arrival procedures, snack, homework and drawing activities in the cafeteria, outdoor free choice activities and departure procedures. Once the students came in from outside they had the following options available: playdough, kinetic sand and an art activity. Supervision, adequate approved space use, capacity, group size, staff/child ratios and permit restrictions were monitored. You stated there were no students with medication in this out-of-school program. A portion of the student files were monitored. There are two new staff hired since my last visit. I requested to see the staff files, and you had access to your file virtually, but you stated that you did not have access to Ms. Webb’s file. I contacted Kimberly Curtis, Regional Programing Director, and she emailed the staff file to you for my review. I monitored WORKS during today's visit: -You have an account but you will need to update your information, including but not limited to uploading your BSAC certificate once completed, and request to be evaluated as a Program Coordinator. -Ms. Webb's account is showing that her registration is in process. Please submit the completed Staff and Training Worksheet to me no later than the end of the business day Monday, December 8th . You stated that this facility does not provide transportation. Kimberly Crane, Child Care Licensing Consultant, reviewed the QRIS/Pathways documents with Ms. Simmons, Youth Development Vice President and Ms. Curtis on September 16, 2025. Prior to today’s visit I contacted Ms. Curtis and she stated that she was meeting with Ms. Simmons this afternoon to discuss the rated license process. During the conversation Ms. Curtis stated that Pathway #2 Classroom and Instructional Quality Assessment was the option that would probably be used. I sent an email with the steps, resources and documents needed for this pathway to Ms. Curtis and Ms. Simmons. Here is what will need to be completed for the Classroom and Instructional Quality Assessment option (Pathway 2) CC RULE .3205: • Complete the “Application For Assessment for a Rated License for Centers” electronically. • Confirm all staff have active WORKS accounts. • Complete the QRIS Staff Information and Education Worksheet electronically. • Identify CQI goals and begin planning for implementation. • Develop strategies for Family and Community Engagement. • Select and implement a curriculum. (4- and 5-star centers must use an approved curriculum for all ages.) • Arrange coaching or training options for administrators. A compliance history score of seventy-five percent or above must be maintained to receive a rated license. The following violation was cited during today's visit: Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There was no documentation that a fire drill was conducted for the month of November. .0604(t); .0302(d)(5) TECHNICAL ASSITANCE: -Place a reminder in your calendar on your phone to conduct a fire drill monthly. Make sure to document the information on the log as soon as it is completed. CONSULTATION: -Please review CC RULE .2508 concerning the number of age appropriate activities for school aged student: (a) Child care facilities that provide care to school-age children shall provide a balance of teacher directed and free choice activities appropriate to the age, needs, and interests of the children. (b) Opportunities must be provided for children to participate in the planning and the implementation of activities. (c) Facilities that operate a school-age component for three or fewer hours per day shall make three of the following activities available daily; those that operate a school-age component for more than three hours per day shall make four of the following activities available daily: (1) career development activities; (2) community awareness activities; (3) creative arts activities; (4) cultural activities; (5) games or manipulatives; (6) hands-on academic enrichment activities including language, math, science, social studies, or foreign language activities; (7) health education or wellness activities; (8) homework with assistance available as needed from center personnel; (9) reading activities; (10) sand or water play; (11) social skills, life skills or problem-solving activities; (12) structured or unstructured physical activities; or (13) technology skill-building activities. -All staff must have their current education evaluated by the WORKS Unit in Raleigh. While the Division allows you time to explain how you have corrected violations when violations are cited, it is expected you correct all violations immediately. When you are asked to send a compliance letter to me, it is expected and required that your compliance letter be sent to me no later than the date indicated in this documentation. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. On or before October 15, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. While the Division allows you time to explain how you have corrected violations when violations are cited, it is expected you correct all violations immediately. When you are asked to send a compliance letter to me, it is expected and required that your compliance letter be sent to me no later than the date indicated in this documentation. Your letter must include the following: -Name of your program -ID number of your program -Date of Letter -Violation number -Explain how you corrected the violation (send supporting documentation to show compliance) -Describe your plan to make sure you will not have that same violation in the future -Name or signature of the person who wrote the letter.
