Upon arrival, I was greeted by you, Alexa Joines, Director and I stated the reason for my visit. McD CRCDC LLC is listed as the owner of the child care facility and is current and active on the NC Secretary of State website. This facility was issued a temporary license on April 30, 2026, with a restriction of daytime care only. The following were posted: Temporary License, Safe Arrival and Departure Procedures, NC Summary of Child Care Law poster from January 2025 and the Emergency Medical Care Plan. Supervision, staff/child ratio, grouping of children, the use of approved space, and permit restrictions were monitored during today’s visit. The children were observed during floor time in the room used by infant aged children, bottle feeding, hand-washing routines, lunch, nap/test time, waking up from nap and departure procedures. Positive interactions between the staff and children took place throughout the visit today. You stated that there are 77 children enrolled and that there are 65 children present today between infant aged and 10 years of age. You also stated the school aged children had just left on a field trip to the roller skating rink and will return at 3:15. Ms. Rose, Administrator/Program Coordinator, accompanied the children on the field trip. Due to the field trip, I was unable to monitor the vehicle and will do so on a future visit. A portion of the children files were monitored. I monitored a portion of the staffs’ files. You had the Staff and Training Worksheets available for review, and you will need to add the new staff member that began yesterday, June 8th. Please continue to keep the Staff and Training Worksheet current using the revised Worksheet and have it available for review by DCDEE staff. We discussed that the sanitation, fire and building inspections need to be completed and submitted to me prior to the end of the temporary license expiration of October 30, 2026. If approved inspections are not submitted prior to the temporary license a rated license cannot be issued and may lead to an administrative action. You stated that the sanitation inspector, Scott Carpenter, came out June 4, 2026, and will conduct an inspection in July. The building inspector, Damon White, conducted a visit on May 8, 2026, and met with Nathan McDaniel, Owner, but you have not received the building inspection as of today’s date. You stated that you will contact the fire inspector and request that an inspection be completed. The last outdoor inspection was completed on May 28, 2026, and the outdoor learning environment (OLE) was monitored today. The last fire drill was completed on May 29, 2026. A lock down drill was completed on March 25, 2026. Rated License Discussion: During today’s visit, we discussed the three QRIS Pathways to the Stars effective July 1, 2025. You stated that this facility is choosing Pathway 2: Classroom and Instructional Quality and that you and staff have started working on the various components. We reviewed the steps that will need to be completed and submitted to me no later than August 15, 2026: •Complete the “Application for Assessment for a Rated License for Centers” electronically. •Confirm all staff have active WORKS accounts. •I have received the QRIS Staff Information and Education Worksheet electronically. Please submit to me any updates that may occur. •Identify Continuous Quality Improvement (CQI) goals and begin planning for implementation for the facility. •All staff will need to Identify Continuous Quality Improvement (CQI) goals and begin planning for implementation. •Develop strategies for Family and Community Engagement. Please submit to me the completed form along with documentation on how the strategies will be met. •You stated that your program is using the Creative Curriculum for Infants, Toddlers and Twos, Creative Curriculum 3-5 and the 4-H School-Age Curriculum. You, Ms. Rose, and seven staff members received the training for the curricula on May 27, 2026 and I was able to monitor the certificates during today’s visit. You stated that Ms. Rose will be completing the DRDP formative assessment training. •You are working with Karen Ceccato with the Catawba Partnership for Children arranging coaching or training options for administrators and lead teachers. A compliance history score of seventy-five percent or above must be maintained to receive a rated license. The following violations were observed during today's visit: Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence surrounding most of the outdoor play area was measuring between 3 feet 7 inches and 3 feet 10 inches. There were also areas in which there were protrusions where the chain link fence at the top was not covered to prevent protrusions hazards. GS 110-91(6); .0605((i) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Room 231 had Nystatin and the medication administration form had validation date that expired on 5/23/26. .0803(12) TECHNICAL ASSISTANCE: -We discussed that the fence surrounding the outdoor play areas need to be enclosed by a fence with a minimum height of 4 feet. If the top of the fence is less than six feet, then it will need to be free from protrusions. The pointed ends of the chain link fence that are sticking out are considered protrusions and will need to be made inaccessible and not visible. We discussed pulling the pole holding the fence up so that it covers the protrusions or covering with plastic corrugated solid pipe or pool noodles. We discussed that an extension of chain link fence can be added to increase the height of the fence. Over time fences do settle and mulch and dirt settles by the fence which decreases the height. -We discussed that it may be helpful for staff to have a list of their diaper creams and when they expire and post where it is visible so that it is a reminder for staff. Have families complete a new medication administration form when their children transition to a new classroom. CONSULTATION: -We discussed that the Summary of Law was updated in November 2025. You will want to get the most current one posted and make sure that new families enrolled are receiving the current one as well. -We discussed the ABCMS Portal Roster and we were able to review all staff under the facility’s old ID number. I stated that I would reach out and see what you will need to do to retrieve the new provider number so that you can update the new roster. -We discussed that doors leading to the outdoors cannot stay open if there are no screens preventing flying pests from entering. 15A NCAC 18A.2831(c) states that openings to the outer air will need to be protected against the entrance of flying pests. It may be helpful to have painters come after hours or on weekends so that they can keep doors open. I would also encourage to have classroom hallway doors closed while they are in the process of getting painted. Suggest that the painters bring fans to help with ventilation. If they need the outdoor doors open for the fumes than it is a job that should not be taken place while children are in care. -We discussed that prior to staff moving to temporary spaces that have been approved previously so that their spaces can be painted that they have an opportunity to prepare the temporary space for the children. This includes but not limited to, posting the fire evacuation route, posting the staff/child ratio sign, posting the activity plan and daily schedule, making sure that the medication and diaper creams are stored in the temporary spaces and activity centers with materials are set up. This would be best practice to provide a smooth transition for the children, families and staff as updates are made to their rooms/spaces. -We discussed the importance of conducing the annual staff evaluations and especially at a time when they are experiencing changes. Research shows that evaluations boost engagement, and reduces turnover when staff are provided clear pathways for growth and areas they can work on. This would be a great opportunity to introduce them to the individual CQI forms that will be part of the Pathway two and will need to be completed annually. -Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 23, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. While the Division allows you time to explain how you have corrected violations when violations are cited, it is expected you correct all violations immediately. When you are asked to send a compliance letter to me, it is expected and required that your compliance letter be sent to me no later than the date indicated in this documentation. Your letter must include the following: -Name of your program -ID number of your program -Date of Letter -Violation number -Explain how you corrected the violation (send supporting documentation to show compliance) -Describe your plan to make sure you will not have that same violation in the future -Name or signature of the person who wrote the letter.
