This facility currently operates with a Five-Star License, issued August 6, 2025. The last annual compliance visit was conducted on August 6, 2025. The last sanitation inspection was completed on October 3, 2025, earning a superior classification and zero (0) demerits. The last satisfactory fire inspection was completed on November 7, 2023. T. Gallentine, Teacher was present and available to assist. Thirty-three (33) children, ages one (1) to five (5) years of age were present during the visit. The children were observed free-choice centers, group-time teacher directed activity, handwashing and toileting procedures, diapering, preparing for lunch, and eating lunch. Lunch consisted of cheese and pepperoni pizza, broccoli, pears, and milk. Caregivers interacted with children in a positive manner. This facility currently operates with a Five-Star License, issued February 25, 2019. The last annual compliance visit was conducted on October 3, 2023. The last sanitation inspection was completed on September 18, 2023, earning a superior classification and zero (0) demerits. The last satisfactory fire inspection was completed on November 7, 2023. P. Harris, Lead Teacher was present and available to assist. Twenty-one (21) children, ages one (1) to five (5) years of age were present during the visit. The children were observed during nap time. Staff monitored children while they were napping. The following violations of child care requirements were documented. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Authorization had expired for one child. .0803(12) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member did not have a medical report prior to employment 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member did not receive a tb test prior to employment. .0701(a) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. TECHNICAL ASSISTANCE/ CONSULTATION: PERMISSION TO ADMINISTER: Medication authorization to administer should be on file for children who have any type of medication, ointment, diaper cream, sunscreen etc. Authorization forms to administer medication should also be current. A child enrolled did not have a current permission to administer form. STAFF MEDICAL REPORT/TB TEST: A staff member did not receive a medical report or TB test prior to employment date. To ensure compliance, you should use the staff checklist to refer to when forms are due. SHAKEN BABY POLICY RECEIPT: There was no shaken baby policy receipt for one staff member acknowledging the policy was reviewed. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by November 1, 2024. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Most of you have completed your initial testing and now it is time for your three-year retest. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. This facility currently operates with a Five-Star License, issued February 25, 2019. The last annual compliance visit was conducted on November 4, 2022. A checklist was used to note the requirements monitored during the visit. The last sanitation inspection was completed on September 18, 2023, earning a superior classification and zero (0) demerits. The last satisfactory fire inspection was completed on November 8, 2022. K. Tompkins, Executive Director of Human Resources was present and available to assist. Thirty-three (33) children, ages one (1) to five (5) years of age were present during the visit. The children were observed in teacher directed activities, free choice play, and outdoor play. Staff interacted with children in a positive manner. The following violations of child care requirements were documented during the visit. GS110-91(1) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete the training within 90 days of employment date of January 17, 2023. The training was completed on June 6, 2023. .1102(g) The above violations must be corrected immediately. TECHNICAL ASSISTANCE/CONSULTATION CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children’s records were monitored. Children records reviewed, were not in compliance. Two children, one in space 6158 and one child in space 6157 did not have a permission to administer medication form. A child in space 6158 did not have documentation of a health assessment/medical report on file before or within thirty days of enrollment date. A copy of the permission to administer form was left at the facility. You should review and monitor other children’s files that were not reviewed to ensure compliance. Ten percent (10%) or more of existent staff files were monitored. Staff records reviewed were not in compliance. One staff member did not complete recognizing and responding to suspicion of child maltreatment training within 90 days of hire date of January 17, 2023. This was corrected during the visit because the training was completed on June 6, 2023. You should review the staff and training worksheet and monitor staff files that were not reviewed to ensure compliance. SANITATION TRAINING ANNOUNCEMENT: Children’s Environmental Health has newly readopted child care sanitation rules effective July 1, 2023. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam. This facility currently operates with a Five-Star License, issued February 25, 2019. The last annual compliance visit was conducted on November 4, 2022. A checklist was used to note the requirements monitored during the visit. The last sanitation inspection was completed on September 18, 2023, earning a superior classification and zero (0) demerits. The last satisfactory fire inspection was completed on November 8, 2022. K. Tompkins, Executive Director of Human Resources was present and available to assist. Thirty-three (33) children, ages one (1) to five (5) years of age were present during the visit. The children were observed in teacher directed activities, free choice play, and outdoor play. Staff interacted with children in a positive manner. The following violations of child care requirements were documented during the visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Two children, one in space 6158 and one 6157 did not have written permission to administer prescribed medication. 10A NCAC 09 .0803(1)(a & b) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A child in space 6158 did not have a health assessment record on file before or within thirty days of enrollment date. GS110-91(1) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete the training within 90 days of employment date of January 17, 2023. The training was completed on June 6, 2023. .1102(g) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by October 17, 2023. Please be aware that any written information submitted by you regarding correction of the violations documented during today's visit is legal documentation. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 83%. TECHNICAL ASSISTANCE/CONSULTATION CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children’s records were monitored. Children records reviewed, were not in compliance. Two children, one in space 6158 and one child in space 6157 did not have a permission to administer medication form. A child in space 6158 did not have documentation of a health assessment/medical report on file before or within thirty days of enrollment date. A copy of the permission to administer form was left at the facility. You should review and monitor other children’s files that were not reviewed to ensure compliance. Ten percent (10%) or more of existent staff files were monitored. Staff records reviewed were not in compliance. One staff member did not complete recognizing and responding to suspicion of child maltreatment training within 90 days of hire date of January 17, 2023. This was corrected during the visit because the training was completed on June 6, 2023. You should review the staff and training worksheet and monitor staff files that were not reviewed to ensure compliance. SANITATION TRAINING ANNOUNCEMENT: Children’s Environmental Health has newly readopted child care sanitation rules effective July 1, 2023. You should review new sanitation rules on our website to ensure compliance. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Information will be shared with according to what cohort you are in. EARLY CHILDHOOD EQUIVALENCY EXAM: The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam.
