T. Kearney, Owner/Operator assisted with the visit. Four (4) children were observed in care today ranging from ages two to four years old. Children were engaged in free choice play with developmentally age-appropriate materials, outdoor playground, interacting with the caregivers, completing toileting, and hand washing routines, preparing for nap, and eating lunch. Lunch consisted of fish fillets, tater tots, sliced apples, bun, and milk. Currently this program operates with a Five-Star License earning seven points in education, five points in program standards and one quality point for serving no more than two children under age one. The last annual compliance visit was completed on November 24, 2025. The following violations were documented: Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Operator and household member were not linked in the ABCMS CBC Portal. G.S. 110-90.2 & .2703(r) The violations documented must be corrected immediately. Please send a compliance verification letter to me describing how the violations were corrected. You can submit the compliance verification letter by postal mail to Emily Hargrove, PO Box 1974 Henderson NC 27536, or email emily.hargrove@dhhs.nc.gov. The compliance verification letter must be received on or before April 28, 2026. The two-week time frame is established to allow you time for submitting your compliance verification letter. This time frame is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Please be aware that any written information submitted by you regarding correction of violations documented during the visit is legal documentation. Therefore, it is important that all information submitted be accurate and truthful. Please be aware that if it is determined information submitted was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility's Star Rated License could be issued. I searched in the ABCMS Portal under roster and no individuals were found. OUTDOOR PLAYGROUND: I reminded you that the playground needs to be free of broken equipment or equipment that needs repairing needs to be removed or fixed to ensure the safety of children while playing on the playground. Challenging Behaviors Helpline The Healthy Social Behaviors Project offers coaching for child care providers who would like support managing challenging classroom behaviors. The goal is to empower teachers to create learning environments that promote pro-social skills, like self-control and problem solving. Support is available by phone (888-600-1685, Option 1), email, or through an online form. TECHNICAL ASSISTANCE Technical assistance was provided today during the visit. I assisted you with ABCMS Portal and I reviewed the information and instructions from ABCMS provider technical assistance. DHHS Criminal Background Checks: Basic Information North Carolina Child Care Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. The Automated Background Check Management System (ABCMS) launched September 29, 2022 and can be found at this link. DCDEE has developed a user guide that you can find here. Due to legislation passed in July 2022, all background checks are now valid for five years, regardless of what your letter may currently say. Support for NCID is provided by NC Information Technology Services or by telephone at 919.754.6000. Lunch consisted of cheese pizza, green beans, peaches and milk. Your husband assisted with providing care for children today. Currently this family child care home operates with a Five-Star License earning six points in education standards, seven points in program standards and one quality point for caring for no more than two infants under age one. You reported children arrived late due to no electricity and parents provided breakfast. Upon arrival into the designated space for child care, you removed an electric heater. The heater was not operating; however, you notified me it was put in the designated space to help warm the area faster. Although I did not observe the heater in use, I reminded you to ensure electrical heaters are used in the child care space when children are present. CHILDREN FILES: A medical statement along with discipline and tobacco policies were missing from children’s files. I advised you to review your program’s tobacco and discipline policies with parents, obtain signatures and place a copy in children files. A medical assessment must be completed and provided for the preschool child enrolled. Once received, the medical statement should be placed in the child’s file. It was suggested that you periodically review children’s records to ensure the required documentation is on file. SCREEN TIME: The television was turned on for children when they entered the designated child care space. I informed you screen time is prohibited for children under the age of three. You had your husband turn the television off immediately. If you utilize the television for school age children in care, alternative activities should be offered to children under age three. You should also monitor to ensure children under age three are not watching the television when in use. HAND WASHING: After diaper changing, a child was placed in his crib without hand washing. When asked, you acknowledge the child’s hands had not been washed and immediately done so. I reminded you children’s hands should be washed after diapering. I recommended you, volunteers and additional caregivers review the hand washing procedures to ensure children’s hands are washed as required. Hand washing is important to the health and safety of children. It also cuts down on the spread of germs. T. Kearney, Owner/Operator assisted with the visit. Three children were observed in care today ranging from ages one to two years old. Children were observed completing morning activities, playing with developmentally appropriate materials, eating lunch, completing diaper changing and hand washing routines and interacting with the caregivers. Lunch consisted of cheese pizza, green beans, peaches and milk. Your husband assisted with providing care for children today. Currently this family child care home operates with a Five-Star License earning six points in education standards, seven points in program standards and one quality point for caring for no more than two infants under age one. The last annual compliance visit was conducted on January 26, 2023. The following violations of the child care requirements were documented. Violation Number Comment Rule 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. Screen time was offered to three children under age three (3). .1718(c) 716 Electrical outlets not in use were not covered. Electrical outlets not in use in the surge protector accessible to children under the operators’ desk, on a wall in the designated child care space and in the room where care was provided to children in the main area of the home upon arrival was not covered with safety plugs. 10A NCAC .1719(a)(27) 815 Children's hands were not washed after diapering. One child’s hands were not washed immediately after diapering. The child was placed inside a crib. .1725(a)(6)(C) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A medical assessment was not on file for one preschool child enrolled. GS 110-91(1); 10A NCAC 09.1721(a)(1) 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. Documentation was not on file to attest the program’s discipline policy was reviewed and given to the parent of one child enrolled. G.S.110-91(10); .1727(a)&(b) 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. Documentation was not on file verifying the operator reviewed and provided written documentation to the parents of three children enrolled regarding the program’s smoking/tobacco policy. .1719(a)(11) The violations documented must be corrected immediately. Please send a compliance letter describing how the violations were corrected. The compliance letter must be received on or before December 27, 2023. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your family child care home are correct. LICENSEE: You are listed as the sole proprietor on the child care license. DESIGNATED CHILD CARE SPACE: Upon arrival, I observed care being provided to children a space inside the main dwelling of the home instead of the added component designated for child care. You transferred the children to the designated space before the visit started. You stated the electricity in the home was out earlier and the heat in the designated child care space was not working properly. You added the space designated for child care was cold; therefore, children were brought inside the home space. You reported children arrived late due to no electricity and parents provided breakfast. Upon arrival into the designated space for child care, you removed an electric heater. The heater was not operating; however, you notified me it was put in the designated space to help warm the area faster. Although I did not observe the heater in use, I reminded you to ensure electrical heaters are used in the child care space when children are present. CHILDREN FILES: A medical statement along with discipline and tobacco policies were missing from children’s files. I advised you to review your program’s tobacco and discipline policies with parents, obtain signatures and place a copy in children files. A medical assessment must be completed and provided for the preschool child enrolled. Once received, the medical statement should be placed in the child’s file. It was suggested that you periodically review children’s records to ensure the required documentation is on file.
