    Name of Operation: TAYLOR TUBBIES ACADEMY Facility ID: 91000298 Consultant: EMILY HARGROVE Operation Type: Center Case Number: Visit Date: 5/27/2026 Number Present: 20 Completed Date: 5/27/2026 Age: From 0 To 5 Total Minutes: 290 Time In: 09:10 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of my unannounced visit today was to monitor compliance with child care requirements applicable to an annual compliance visit. K. Perry, Administrator was present and informed of the reason or the visit and assisted in its completion. Currently, this center operates with a Five-Star License issued March 14, 2020, earning five (5) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last Annual Compliance visit at this center was completed June 4, 2025. The posted sanitation inspection was completed October 24, 2025 earning a “Superior” rating. The last fire inspection was completed March 17, 2026 and the facility was approved for daytime and nighttime. Twenty children were present ages ranging from Zero (0) months to Five (5) year old. Children were observed playing in activity centers, participating in group times, art activities, outdoor play, handwashing procedures, toileting and diapering procedures, eating lunch, preparing for nap, and napping . Lunch consisted of Hot Dogs, Baked Beans, fruit cocktail, bread, and milk. Observed interactions were nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Three staff members were not linked in the ABCMS portal. G.S. 110-90.2 & .2703(r) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by June 10, 2026. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how you corrected the violation. Please be aware any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules. The best way to do so is to periodically review the child care laws and rules. The current rule book dated July 1, 2023, is available at the DCDEE website https://ncchildcare.ncdhhs.gov/. I recommend you frequently click the “What’s New” tab to stay informed of all updates on the DCDEE website. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 100% over an eighteen (18) month period. FACILITY PROFILE This facility is owned by Taylor-Tubbies Academy Incorporated. The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database. CHILDREN RECORDS I reviewed ten (10) percent or more of children’s files and they were in compliance. STAFF RECORDS I reviewed ten (10) percent or more of staff's files and they were in compliance. ABCMS: K. Perry, M. Martin, and T. Blackwell were not linked in the ABCMS Portal. You immediately during the visit started the process on linking staff that were not on the roster. I verified that you were linking staff during the visit. REQUIRED WATER TESTING Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. You completed water testing on 12-19-2024 and have received results. You will be due to complete a water testing by December 2027. You will need to complete the lead and asbestos according to the data on the website you have started the enrollment and not completed the testings. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs should have completed the application summary for their program regarding lead paint and asbestos by November 1, 2024. TECHNICAL ASSISTANCE Technical assistance discussed regarding QRIS. I reviewed and discussed with you the Overview of Pathway 2. We reviewed all the forms required for Pathway 2. I discussed the education star level for each staff listed on the staff and training worksheet. Staff information was verified in DCDEE WORKS on 5-26-2026. I explained that I would review T. Perry and any other staff education information within 4 to 6 months to see where they are with the education. K. Perry, administrator, was present and informed of the reason or the visit and assisted in its completion. Currently, this center operates with a Five-Star License issued March 14, 2020, earning five (5) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last Annual Compliance visit at this center was completed July 12, 2022. The posted sanitation inspection was completed September 29, 2022 earning a “Superior” rating. The last fire inspection was completed March 3, 2023 and the facility was approved for daytime and nighttime. An attempted visit was conducted on June 28th to complete the annual compliance visit. The facility was closed and upon emailing you I was informed that the program would be closed from June 28th through July 12th for vacation. Children were observed participating in group times which included the teacher reading a story to children and a teacher asking the group to identify letters posted at eye level on the wall. Observed interactions were nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was one (1) uncovered outlet in space #1b. 10A NCAC 09 .0604(c) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Several diaper creams/ointments were stored in space #1 that are no longer being used and were returned to parents or disposed of. .0803(12) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Staff person hired 9/13/22 completed their TB test or screening after their hire date. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Staff person hired 9/13/22 completed the first aid training after the required time frame. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Staff person hired 9/13/22 completed the CPR training after the required time frame. .1102(d) The violations documented above must be corrected immediately. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 95% over an eighteen (18) month period. FACILITY PROFILE This facility is owned by you, K. Perry, as a sole proprietor. The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database and is required to remain so. I reviewed contact information for the owner and the facility with you, K. Perry, and you verified the current information listed with DCDEE remains correct. CHILDREN RECORDS I reviewed ten (10) percent or more of children’s files and they were in compliance. STAFF RECORDS I reviewed all new staff, and ten (10) percent of remaining sand three (3) violations were documented. The staff person hired 9/13/22 completed the CPR and first aid training after the required ninety (90) day time frame. There was no certificate or card available to review that they had a current training prior to the 2023 certificate. The same staff person also completed their TB screening/test after their first day of work. TB screenings/tests are required to be completed prior to the first day to ensure they are negative before working with children. GENERAL SAFETY ITEMS While monitoring classrooms I observed an uncovered outlet in space #1b near the entrance door. You covered outlet at the time I made you aware of it. Therefore, this violation was corrected during the visit. MEDICATION While monitoring the class for infants I observed several containers of diaper cream and ointment that were not in use. Diaper creams and ointments must be returned to parents or discarded when treatment ends or permission expires. When informed teachers discarded the creams so this was corrected during the visit. TECHNICAL ASSISTANCE Technical assistance given related to ensuring that the page listing the current date of revision for EPR plan is printed and added to the notebook for easy verification. We also discussed having parents add the date of their signature for any forms that require them. I also recommend having parents enter “not applicable” under the application health care needs section. Therefore, you know there are no health concerns and not that parents just forgot to complete it. Technical assistance discussed regarding teachers in the room for infants ensuring that all plastic bags that can be torn into small pieces be made inaccessible when infants are present. This can be done by storing plastic bags five feet or higher above the floor or a cabinet with some type of locking mechanism but does not have to be two-step lock. REMINDERS Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules.
