R. Harris, owner/operator, was present and assisted with the visit. This family child care home was issued a Child Care License on March 30, 2022 and received a three star rated license on December 29, 2022. This program earned four (4) points in the Education Component, two (2) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on 5/15/2025. Currently children are only enrolled on first shift. I observed children play in activity centers, handwashing and toileting, eating am snack, and preparing for lunch. AM Snack consisted of mango slices, graham crackers, and milk. Interactions between the children and caregiver were nurturing in nature. The following violations were documented today: Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Operator was not linked in the CBC ABCMS Portal. G.S. 110-90.2 & .2703(r) The violations documented must be corrected immediately. Please send a compliance verification letter to me describing how the violations were corrected. You can submit the compliance verification letter by postal mail to Emily Hargrove, PO Box 1974 Henderson NC 27536, or email emily.hargrove@dhhs.nc.gov. The compliance verification letter must be received on or before April 22, 2026. The two-week time frame is established to allow you time for submitting your compliance verification letter. I assisted you with ABCMS Portal and I reviewed the information and instructions from ABCMS provider technical assistance. DHHS Criminal Background Checks: Basic Information North Carolina Child Care Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. Complete information and instructions are available in this ABCMS Provider Technical Assistance. Once you have been notified that access has been granted, log into the provider portal to locate the unique provider code for the facility. Applicants and employees will use this code to create a connecting application to be associated with your facility or Family Child Care Home. The links include a help document to assist you. Tracy Stanley, Lead Investigations Consultant, was also present during the visit. Rachel Harris, Operator, accompanied us during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Harris. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 601 Nutritious meals and snacks served did not meet Meal Patterns for Children in Child Care Programs. Today the provider served the children in care pizza, juice and goldfish. This does not meet Meal Patterns for Children in Care. 10A NCAC 09. 1706(a) 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. The indoor area where children are cared for has goldfish and pizza crust strewn on the floor that at least one child was picking up and eating during the visit. .1719(a)(1)&(17) 1603 The home was not in compliance with permit restrictions. Today the home was not incompliance with permit restrictions because the operator was providing care to six preschool aged children. This is one child over the required capacity. GS 110-91 1892 Application was not on file for each enrolled child, including his/her own preschool child(ren) who are not school-age. Three children in care today did not have an application on file. .1721(a)(3) 1942 Children were not cared for in a smoke free and tobacco free environment. The child care area smelled strongly of some type of smoke and evidence of that smoking had taken place in areas of the home was present. .1719(a)(10) Violations must be corrected immediately. R. Harris, provider, was present and informed of the reason for the visit. Limited monitoring of the child care requirements was completed today. E. Hargrove, Child Care Consultant, was present and assisted with the visit. The following child care requirements were monitored today: Adequate approved space, permit restrictions, staff/child ratio, supervision. The following child requirements were monitored: supervision, permit restrictions, staff/child ratios, adequate/approved space, and nurture and care. Permit restrictions for this facility include daytime care only, a max of five (5) preschool aged children at any one time, serves no more than two (2) infants under the age of one (1) year, and the fireplace/wood stove is not used during operating hours. The following violation of child care requirements was observed today. Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. A push pin was observed on the floor of the space used to care for children and a used glove on the floor of the bathroom. .1719(a)(1)&(17) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Arrival times were not documented for the children in attendance. .1721(e)(6) 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. Four (4) of the children in care slept on cots that did not have linens on them. 10A NCAC 09 .1718(a)(2) The violations documented must be corrected immediately. The signed and dated letter of compliance should be mailed or emailed to me by September 3, 2024, stating how the violations were corrected and how compliance will be maintained at all times. Specifically, address steps that will be taken to ensure that children are attended to in a nurturing and caring way that is always developmentally appropriate. It should also address how to address parent grievances with other enrolled children and/or families. My mailing address is located at the bottom. Please be aware that any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all included information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. We observed that the fire inspection has been documented for this month and the emergency medical care plan was updated. The grill and charcoal have been moved from the front yard, which is used for outdoor play, to the side of the house. The litter has also been removed from around the entrance used by families and front yard. I recommend keeping the grill and charcoal in the locked storage shed located in the backyard. Creating a daily, weekly and monthly cleaning schedule may be helpful in continuing to ensure you are meeting safe indoor environment and sanitation requirements. You informed us that you are waiting for results from the well water bacteriological tests completed after you had to shock the well a second time. You expect to receive the email results tomorrow or Monday from Vance County Environmental Health Services (EHS). Please contact me as soon as you receive the results as documentation must be submitted for compliance with that violation documented on June 13, 2024. Technical assistance discussed regarding reaching out to EHS regarding the issues with your septic tank to ensure that this was not the cause of the previous positive bacteriological results. SAFETY AND SUPERVISION While monitoring the indoor environment we observed that four (4) of the enrolled children napped on cots that did not have linens/sheets to cover them. Engaging in safe and healthy sleep practices is important in child care and included in the Child Care Rules. Sleep routines play an important role in physical, emotional, and behavioral health of young children. Adequate rest helps young children to recover from and be prepared for the daily activities that aid in their physical and educational development. Ensuring each child has an adequate and separate sleep surface, as well as clean linen to cover it, also helps to reduce the spread of illness. Child Care Rules require that every child in care be supplied an individual sleeping space such as a bed, crib, play pen, cot, mat, or sleeping bag with individual tight-fitting linens when in care for more than four (4) hours a day. A push pin was observed on the floor near the patio doors. Items like this must be kept out of reach when in use to ensure that a child does not injury themselves with it or put it in their mouth. You removed the push pin during the visit therefore it was documented as corrected. We also observed the potty seat that fits over the toilet on the bathroom floor. This must be cleaned and sanitized between use and should be stored in a way that it will not become soiled prior to use. PROGRAM RECORDS The arrival times were documented today for the children in care. Child Care Rules require that arrival and departure times be documented as they occur for each child. If you do not required parents to sign children in and out, then you as the provider must do so. Ensure that you initial or sign your name for each child you document times for, and parents should do the same. TECHNICAL ASSISTANCE Technical assistance discussed related to purchasing cot sheets, if you do not already own some, to cover all cots when children nap. I would recommend purchase enough sheets to cover half of the cots used for care to ensure you can change them quickly if soiled or in disrepair. This family child care home was issued a temporary Child Care License on June 28, 2022 and received a three star rated license on December 29, 2022. This program earned four (4) points in the Education Component, two (2) points in the Program Component and one (1) Quality point. The last well water bacteriological analysis was completed March 8, 2022 and is delinquent. Currently children are only enrolled on second shift and arrive for care around 2:30pm. I observed children arrive and play in activity centers. Interactions between the children and caregiver were nurturing in nature. The following violations of child care requirements were documented.
