    Name of Operation: PINKSTON STREET ELEMENTARY PRE-K Facility ID: 91000203 Consultant: EMILY HARGROVE Operation Type: Center Case Number: Visit Date: 2/25/2026 Number Present: 31 Completed Date: 2/25/2026 Age: From 4 To 5 Total Minutes: 290 Time In: 09:25 AM Time Out: 02:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor this program with applicable child care requirements during a Annual Compliance visit. This facility currently operates with a Five-Star license issued April 19, 2018. The last annual compliance visit was completed March 19, 2025. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on February 17, 2026, earning a superior classification and “8” demerits. The last satisfactory fire inspection was completed on September 25, 2025, and approved for day care only. C. Mayes, Principal, and all teachers were present and available to assist. Thirty-one (31) children four (4) to five (5) years of age were present. Children were observed in free choice play, preparing for lunch, and teacher directed activities. Lunch consisted of: hot dogs on bun, baked beans, french fries pear cups, grape juice, and milk. All staff interacted with children in a positive manner. Playground inspections, fire and lockdown drills were all current. Off-site forms were monitored. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. NC Pre-K requirements were monitored during the visit. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Staff member was not linked to the ABCMS Portal. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. CONSULTATION: Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 96%. PROGRAM RECORDS: Fire Drills/Emergency Drills, Fire inspection, Sanitation report, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and License were reviewed and current. CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children records were monitored. Children records reviewed were in compliance. You should review other children’s records not monitored to ensure compliance for the same information. Ten percent (10%) or more of staff records were monitored. Staff records reviewed were in compliance. Staff should review records that were not monitored to ensure compliance. DCDEE WORKS: All staff should ensure they have an education qualification letter on file. Each staff member should create an account, apply for DPI teacher or DPI teacher assistant, and submit transcripts and licensure if not already done so. PROVIDER DOCUMENTS/FORMS: You should visit the website often to ensure you’re using all updated forms. All requirements must be included if you choose to use your own forms electronically or hard copy. Child care rules updated effective July 1, 2023, and January 1, 2024. Criminal Background Check: All staff were verified in ABCMS Portal. Principal C. Mayes has a valid qualification letter through 6/11/2026 in the ABCMS person summary but needs to be linked to ABCMS employee roster in ABCMS Portal. I reviewed with you C. Mayes the ABCMS Technical Assistance to assist with linking new hires. You started the process during the visit. You made a call to CBC Unit during the visit and they provided assistance so that you can get linked. You stated that you would send proof that you are linked along with correction letter soon as it's processed. TECHNICAL ASSISTANCE: Technical assistance was provided today during the visit. I assisted you with ABCMS Portal and I reviewed the information and instructions from ABCMS provider technical assistance. QRIS: I reviewed QRIS modernization page and Application for Rated License for Centers with J. Peace and C.Mayes. We reviewed and discussed all the forms that are required to be completed by staff and administrator. I strongly encourage you to start the Self-Study. During the visit on November 17, 2025 we had a initiative date for March of 2026. However, J. Peace informed me during this visit that this program is not ready for the Environment Rating Scale. I encourage you to contact FGV Smart Start for resource to work with staff and program to prepare for assessment. J. Stallings, Lead Teacher was present and assisted with today's visit. This facility currently operates with a Five-Star license issued April 19, 2018. The last annual compliance visit was completed May 11, 2023. The last sanitation inspection was completed on September 25, 2024, earning a superior classification and “8” demerits. The last satisfactory fire inspection was completed on September 9, 2024, and was approved for day time care only. J. Stallings, Lead Teacher and other NCPREK staff were present and available to assist. Twenty-eight (28) children; four (4) to five (5) years of age were present. Children were observed in free choice play, preparing for lunch, handwashing and toileting procedures, teacher directed activities, and eating lunch. Lunch consisted of chicken nuggets, mash potatoes, baked beans, mixed fruit, and milk. All staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. The following violations of child care requirements were documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Fire inspection was not submitted to the consultant within one week of the inspection visit. A copy of the fire inspection was received. 10A NCAC 09 .0304(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #5 disinfectant spray was laying on top of the cots and microband aerosol can was laying on the floor near teachers station. .2820(b) The above violations must be corrected immediately. The fire inspection was current dated September 9, 2024. This was corrected during the visit. A copy of the inspection was received during the visit. SAFE INDOOR ENVIORNMENT: In space #5, disinfectant spray and microband was not stored properly and locked out of reach of children. TA stated that she had used it and forgot to put it away. TA immediately removed microband and placed out of reach of children and placed disinfectant out of reach of children. We discussed that all hazardous items must be locked and or stored out of reach of children. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by May 1, 2024. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Most of you have completed your initial testing and now it is time for your three-year retest. Please log into https://www.cleanwaterforuskids.org/en/carolina/ to ensure you are meeting the requirements for Lead in Water / Paint/Asbestos testing. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. More information will be provided as your cohort approaches. DCDEE WORKS: All staff, including the principal, should ensure an education qualification letter is on file to avoid a delay in processing the rated license when your cohort approaches. If staff does not have an account, an account should be created in DCDEE WORKS. Staff should apply for position, upload licensure and, or submit transcripts.
