N. Heggie, administrator, was present and assisted. Currently, this center operates with a Four-Star License issued October 22, 2018, earning five (5) points in the Education Component, six (6) points in the Program Component and one (1) Quality point. The last Annual Compliance visit at this center was completed June 16, 2021. The last sanitation inspection was completed May 25, 2023 earning a “Superior” rating. The last fire inspection was completed July 30, 2024 and the facility was approved for daytime care only. CORPORATION & FACILITY PROFILE This facility is owned by New Beginning Pre-School Inc The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database. The corporation that owns this facility is required to always maintain a current/active status. Failure to do so could result in an Administrative Action including Suspension or Revocation of the facility’s child care license. I reviewed the facility contact and owner information with you, N. Heggie. You confirmed the information currently listed in the system remains correct. Thirty-seven (37) children were present ages (0) zero months to (5) five years of age. Children were observed playing in free choice centers, outdoor playground, handwashing, diapering and toiletings, and eating lunch. Lunch consisted of chicken rice, turnip greens, mandarin oranges, wheat bread, and milk. Caregivers interacted with children in a caring, nurturing, and positive manner. The following violations of child care requirements were documented. TECHINAL ASSISTANCE AND CONSUTATION: Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 96%. PROGRAM RECORDS: Fire Drills/Emergency Drills, Fire inspection, Sanitation report, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and License were reviewed and current. You submitted the most current fire inspection. I discussed with you to make sure that you submit within one week of inspection to DCDEE. SAFE INDOOR/ELECTRICAL OUTLETS: In classroom #4, a surge protector had three electrical outlets not covered with safety plugs while not in use. One child’s bottles were labeled but not dated and one bottle was stored with no name or date. 15A NCAC 18A .2804(d) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Aerosol products and Lysol, and Clorox containers were observed in unlocked storage in spaces #1, and the right hallway bathroom. .2820(b) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. There was no current health questionnaire for staff hired 2/28/22 .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. There was no current emergency information for staff hired 2/28/22. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. There is no documentation that staff hired 4/9/24 and 4/11/24 completed the sixteen (16) hours of orientation within the required time frame .1101(a) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. There was no medical report on file for a child enrolled 8/18/22 and no immunization report for a child enrolled 2/15/23. GS 110-91(1);.0302(d)(2); .0304(g) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. No lockdown or shelter-in-place drill has been documented since April 2023. .0604(u);.0302(d)(8) 1867 The depth of the loose surfacing was not based on critical height of the equipment. There are several areas around play equipment that wood mulch measures four (4) or five (5) inches. .0605(k)(1-4) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by May 16, 2024. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how you the violation was corrected. Please be aware any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 100% over an eighteen (18) month period. STAFF RECORDS I monitored all new staff, and ten (10) percent of remaining staff records and two violations were documented. The staff members hired 4/9/24 and 4/11/24 do not have documentation that the required sixteen (16) hours of orientation were completed within the first two (2) weeks of hire. Staff hired 2/28/22 have not updated their health questionnaire or emergency information since July 2023. CHILDREN’S RECORDS I monitored ten (10) percent of children’s records, and two (2) violations were documented. A child enrolled 8/18/22 does not have medical report on file. A child enrolled 2/15/23 does not have an immunization record on file. Both forms are required to be completed by a health care professional and on file within the first thirty (30) days of enrollment to ensure the children are medically cleared to participate in regular early learning activities. PROGRAM RECORDS No shelter-in-place or lockdown drill has been documented since April 2023. These drills must be practiced every three (3) months and documented on the Division form provided. Quarterly practice ensures the children and staff are familiar with the safety procedures in case of an emergency. NUTRITION While monitoring space #1 I observed that a set of bottles for one of the infants was labeled with a name but not dated. There was also one bottle in the fridge that did not have a name or a date. Child Care Rules require that all infant bottles be labeled with a name and the current date. This ensures that the right child receives the right bottle and that only fresh milk, formula, etc. is given. OUTDOOR PLAY AREA The wood mulch around both slides measured four (4) to five (5) inches in several areas. I recommend tiling the mulch and remeasuring prior to purchasing additional mulch. Resilient surfacing must be at six (6) inches deep at all times to decrease the risk of injury should a child fall form the equipment. STORAGE OF HAZARDOUS ITEMS I observed three containers of Clorox and an aerosol Lysol stored in a cabinet with no lock in the right hallway bathroom. I also observed aerosol spray and Clorox in an unlocked cabinet in space #1. Please ensure that cabinets where cleaning products and aerosol containers are stored have an approved lock. This means it should be a magnetic, key or combination lock. Cleaners and aerosol products are considered hazardous and can cause injury to children if used incorrectly or ingested. ENVIRONMENTAL RATING SCALES You were reminded that this facility is in cohort one related to resuming the environmental rating scales (ERS). This means that the preparation year for your facility began July 1, 2023, through June 30, 2024. The assessment year begins July 1, 2024, through June 30, 2025, and the program must complete the ERS process during that year. Please visit the North Carolina Rated License Assessment Project (NCRLAP) website for informational webinars related to the current process and infographics with information on activities to complete during the preparation year (ncrlap.org). Ensure that all new and current staff have submitted all education to DCDEE WORKS for evaluation prior to completing the ERS. You may also complete an ERS assessment prior to your reassessment year and only use the scores if they are acceptable to you. TECHNICAL ASSISTANCE & REMINDERS Technical assistance discussed related to continuing to make sure that plastic grocery bags used to store children’s clothing or other items are kept five (5) above the ground in spaces that hold children under three (3) years of age. Ensure that you or parents add the enrollment of their children on forms that require that information. Technical assistance given regarding monitoring classroom walls for signs of peeling or disrepair so it may be repaired before damage is done. You requested the website for reviewing and updating your center’s EPR plan. Use https://rmp.nc.gov/portal/portal.aspx to log in and manage your plan. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by May 1, 2024. The application is on the following website: https://www.cleanwaterforuskids.org/en/carolina/. The rule also requires all licensed child care centers to test all drinking water faucets and food preparation sinks for lead contamination once every three years. I recommend you review the date of your initial lead water testing to see if you are due this year. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules.
