This facility currently operates with a Five-Star license issued May 28, 2019. The last annual compliance visit was completed March 13, 2025. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on January 14, 2026, earning a superior classification and “4” demerits. The last satisfactory fire inspection was completed on September 29, 2025, and was approved for day time care only. F. Branch, Principal and J. Chin, Lead Teacher, was present and available to assist. Twenty-seven (27) children were present during the visit. Children were observed in teacher directed activities, group/circle time, screen time, specials (Art), outdoor play, preparing for lunch, and toileting and handwashing procedures. Lunch consisted of hot dogs on bun, baked beans, orange medley juice, and milk. All staff interacted with children in a positive manner. The off-site forms for transportation and staff were available for review. Fire, lockdown drills and playground inspections were current. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. NC PreK requirements were monitored during today’s visit. The following violations were documented today: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In classroom #118, power strip located underneath teacher desk had five outlets not covered with safety plugs while not in use. 10A NCAC 09 .0604(c) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Staff members were not linked to the ABCMS Portal. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by March 19, 2026. DCDEE WORKS: All staff should ensure they have an education qualification letter on file. PROVIDER DOCUMENTS/FORMS: You should visit the website often to ensure you’re using all updated forms. All requirements must be included if you choose to use your own forms electronically or hard copy. Child care rules updated effective July 1, 2023, and January 1, 2024. Criminal Background Check: All staff were verified in ABCMS Portal has a valid qualification letter on file and in the ABCMS person summary but needs to be linked to ABCMS employee roster in ABCMS Portal. I reviewed with you J. Chin the ABCMS Technical Assistance Instructions to assist with linking new hires. SAFE INDOOR ENVIRONMENT: In classroom #118 power strip located underneath teacher desk had several outlets not plug with safety plugs while not in use. I encourage you to keep all electrical outlets covered with safety plugs at all times during hours of care. TECHNICAL ASSISTANCE: Technical assistance was provided today during the visit. I assisted you with ABCMS Portal and I reviewed the information and instructions from ABCMS provider technical assistance. Storage cabinets and sink in space in classroom #118 are cluttered and is not free of items and materials. I encourage you to de clutter and arrange items so that the areas are can be cleaned and organized. F. Branch, Principal observed the space during the visit and is aware of the clutter and cleaning that's needed. I reviewed the sanitation report with principal and lead teacher J. Chin during the visit and it is recorded that this space was observed with clutter in the cabinets and items stored on the floor, at the care givers desk, and cabinets needs locks and not tied with a rope to keep cabinet closed. QRIS RATED LICENSE TRANSITION I discussed and reviewed with you the Pathways to Stars QRIS modernization information from the DCDEE website. We discussed pathway 1 and you stated that you are choosing Pathway 1. I strongly recommend that you visit the North Carolina Rated License Assessment Project’s website at ncrlap.org. There are a variety of videos and infographics with information about ITERS-3, ECERS-3 and SACERS in completing pathway #1. Find information about self-study, updated resources, credit hour trainings, and outreach assessment opportunities from the North Carolina Rated License Project (NCRLAP). I encourage you to continue working with FGV Smart Start for technical assistance related to preparing for pathway 1. I reviewed the Application for Rated License Assessment and each form and where to access them on the DCDEE website under the "provider" then go to "provider documents and forms". I recommend for you to review and print Pathways to the Stars rules, Section .3200, Standards for Two through Five Star Rated Licenses which became effective July 1, 2025. Fire inspection: The posted Fire inspections was last conducted on 9-4-2024. You stated that you have informed program coordinator about the expired fire inspection. I talk with S. Alston, PreK Coordinator via phone during the visit and she stated that she has called and was told that Vance County has a new fire chief and that it would be toward the end of the month before he comes out to conduct inspection. I encouraged you to request an extension for the above violation to ensure you are following applicable child care requirements. Administrator: This facility has a new principal. I verified Criminal background check for administrator. I reviewed the preservice administrative requirements administrative form which was completed by the principal and awaiting signature from superintendent. I encouraged you to follow up to check the status of the form and submit to me once completed. QRIS: The Division of Child Development and Early Education is moving October 1, 2025! Pathways to the Stars: The NC Quality Rating and Improvement System Where Quality and Choice Meet Better school and life outcomes for children depend on quality early childhood teachers who have the skills to ensure that children, supported by their families, have the early experiences they need for healthy development and a strong foundation for learning. North Carolina led the nation in 1999 when it launched its Quality Rating and Improvement System (QRIS), a star rating system for early childhood education facilities. QRIS is a systemic approach to assess, improve, and communicate the level of quality in child care. It encourages programs to engage in continuous quality improvement and families may use the star rating to make informed decisions when choosing a child care facility. Begun in February 2023, the Child Care Commission’s work to modernize North Carolina’s QRIS is now complete. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts. You will receive more information according to what cohort you’re in. This facility currently operates with a Five-Star license issued May 28, 2019. The last annual compliance visit was completed May 4, 2023. The last sanitation inspection was completed on April 20, 2023, earning a superior classification and “4” demerits. The last satisfactory fire inspection was completed on September 20, 2023, and was approved for daytime care only. J. Chin, Lead Teacher, was present and available to assist. Thirty-one (31) children ages four (4) and five (5) years of age were present. Children were observed toileting, handwashing, preparing for nap and napping. All staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. The following violations of child care requirements were documented. G.S. 110-90.2(b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment.
