    Name of Operation: KIDS WORLD Facility ID: 9155040 Consultant: EMILY HARGROVE Operation Type: Center Case Number: Visit Date: 5/6/2026 Number Present: 71 Completed Date: 5/4/2026 Age: From 0 To 6 Total Minutes: 260 Time In: 08:40 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of my unannounced visit today was to monitor compliance with child care requirements applicable to an annual compliance visit. K. Thompson/Director, were present and assisted with the visit. Currently, this center operates with a Four-Star License issued August 17, 2019, earning four (4) points in the Education Component, five (5) points in the Program Component and one (1) Quality point. The last Annual Compliance visit at this center was completed May 28, 2025. The posted sanitation inspection was completed January 29, 2026 earning a “Superior” rating. The last fire inspection was completed March 21, 2025 and the facility was approved for daytime care only. You confirmed that the program does not provide transportation. Seventy-one (71) Children were observed participating in eating breakfast, free choice activity center play, infant tummy time and lunch time routines,diapering procedures, outdoor play, group/circle time, handwashing and toileting, eating lunch, and napping. Breakfast consisted of pineapples, pancakes, and milk. Lunch consisted of Sausage patties, mixed veggies, diced peaches, bread, and milk. Observed interactions were nurturing in nature. The following violations were documented today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was last completed 3/21/2025. 10A NCAC 09 .0304(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Staff member Emergency Information form expired on 5-2-2025. .0701(a) COMPLIANCE LETTER REQUIRED: The violations documented must be corrected immediately. Please send a compliance verification letter to me describing when and how the violation was corrected and your plans for maintaining compliance in the future. You may submit the compliance verification letter by postal mail to Emily Hargrove, PO Box 1974 Henderson, NC 27537 or email emily.hargrove@dhhs.nc.gov. The compliance verification letter must be received on or before May 20, 2026. The two-week time frame is established to allow you time for submitting your compliance verification letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Please be aware that any written information submitted by you regarding correction of violations documented during the visit is legal documentation. Therefore, it is important that all information submitted be accurate and truthful. Please be aware that if it is determined information submitted was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility's license could be issued. GENERAL RULE REMINDERS: Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your center. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. CONSULTATION: COMPLIANCE HISTORY Prior to today’s visit your compliance history was 100% over an eighteen (18) month period. FACILITY PROFILE: This facility is owned by Kids World, Inc. The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database and is required to remain so. I reviewed contact information for the owner and the facility with you, K. Thompson, and you verified the current information listed with DCDEE remains correct. CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children’s records were monitored. Children records reviewed, were in compliance. Children’s files not reviewed should be monitored to ensure compliance for the same information. Ten percent (10%) or more of staff files were monitored. Staff records reviewed were in compliance. Staff files not reviewed should be monitored to ensure compliance for the same information. Staff member D. Greenway last Emergency Information and Health Questionnaire was last completed on 5-2-2025. PROGRAM RECORDS: Fire Drills/Emergency Drills, Fire inspection, Sanitation report, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and License were reviewed and not current. You submitted the most current fire inspection which expired on 3/21/2025 during the visit. During the inspection you completed the application for the fire inspection and you also called the fire inspection office and you were told that they would get you scheduled. SANITATION INSPECTION REPORT: You submitted sanitation report. ABCMS: All staff were verified in the Roster in the CBC Portal. MEDICATION: All medicine and medical forms was reviewed and in compliance. TRANSPORTATION: Do not provide transportation. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids status for lead in water is “section complete”. For the lead base paint and asbestos, the status is “section complete”. This information was verified in the clean water for US kids facility data and records from RTI showing all testings had been complete. TECHNICAL ASSISTANCE: We discussed QRIS and I reviewed the Application For Assessment for a Rated License Center. We discussed Pathway 2 and I explained to you what each form requires. We discussed and reviewed the CQI Plan for the facility and individuals professional development plan. You checked the Family and Engagement form during the visit and I verified the information during the visit. I encouraged you to check staff files and review their Education Qualification letter that lists their education. D. Greenway, owner/administrator, and K. Thompson, director, were present and assisted with the visit. Currently, this center operates with a Four-Star License issued August 17, 2019, earning four (4) points in the Education Component, five (5) points in the Program Component and one (1) Quality point. The last Annual Compliance visit at this center was completed June 21, 2023. The posted sanitation inspection was completed February 9, 2024 earning a “Superior” rating. The last fire inspection was completed February 6, 2024 and the facility was approved for daytime care only. You confirmed that the program does not provide transportation. Children were observed participating in free choice activity center play, infant tummy time and lunch time routines. Observed interactions were nurturing in nature. The following violations of child care requirements were documented. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. I observed expired diaper rash creams in spaces #1, #2 and #3. .0803(12) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. There was no playground inspection available to review for the months of September 2023 or February2024. .0605(q) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The medical report on file for a staff person hired 10/12/23 was more than a year old. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The TB screening/test and medical report on file for a staff person hired 10/12/23 was more than a year old. .0701(a) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by June 20, 2024. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how the violations were corrected. Please be aware any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. I reviewed contact information for the owner and the facility with you, K. Thompson, and you verified the current information listed with DCDEE remains correct. STAFF RECORDS I monitored ten (10) percent or more staff records and two violations were documented. While the staff person hired 10/12/23 does have a medical report and TB screening/test on file it was more than one (1) year old. The medical report was completed in 2018 and the TB screening/test in 2019 which is more than a year prior to 10/12/23. This is required to ensure that the person remains in good health to perform duties required in early education prior to employment at a new facility. CHILDREN RECORDS I monitored ten (10) percent of children’s records, and no violation of child care requirements were documented. PROGRAM RECORDS There was no playground inspection form available for review for the months of September 2023 or February 2024. These completed forms must be available to review for verification the inspections were completed. It may be helpful to have a wall hanger for placing completed inspection forms to help avoid misplacing them. Setting calendar reminders on a phone or computer calendar may also be helpful. I observed diaper rash creams in spaces #1, #2, and #3. You removed the creams to be returned to parents at the end of the day. Please ensure that parents indicate the amount of products to be used such as dime sized, quarter sized, 30ml, etc. ENVIRONMENTAL RATING SCALES You were reminded that this facility is in cohort two related to resuming the environmental rating scales (ERS). This means that the preparation year for your facility begins July 1, 2024 through June 30, 2025. The assessment year begins July 1, 2025, through June 30, 2026, and the program must complete the ERS process during that year. Please visit the North Carolina Rated License Assessment Project (NCRLAP) website for informational webinars related to the current process and infographics with information on activities to complete during the preparation year (ncrlap.org). Ensure that all staff members have submitted all education to DCDEE WORKS for evaluation prior to completing the ERS. If staff is not in WORKS it will affect the program’s education points. You may also complete an ERS assessment prior to your reassessment year and only use the scores if they are acceptable to you. TECHNICAL ASSISTANCE & REMINDERS Continue to ensure that staff store personal belongings like purses in locked storage. Purses often contain items considered hazardous like over the counter medication, lighters, etc. If it is accessible it's a part of the environment and require monitoring as well. Monitor the shade structure bases for stakes that may begin to rise and protrude. Eventually they may require someone hammer them back down to remain flush with the base. You stated that you have a company coming out to replace the playground fencing. New fencing will measure five (5) feet tall. This will ensure that the playground fence remains at the required four (4) foot height or higher. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs should have completed the application summary for their program regarding lead paint and asbestos by May 1, 2024. The application is on the following website: https://www.cleanwaterforuskids.org/en/carolina/. The rule also requires all licensed child care centers to test all drinking water faucets and food preparation sinks for lead contamination once every three years. You informed me that your program has recently had their lead paint and asbestos samples taken and awaiting the results. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules. The best way to do so is to periodically review the child care laws and rules. The current rule book dated January 1, 2024, is available at the DCDEE website https://ncchildcare.ncdhhs.gov/. There are also modules reviewing the new Child Care Rules available on Moodle, which can be found on the Division website.
