You, Iesha Fitts, Operator was present and assisted with today's visit. This family child care home was issued a four star rated license on October 18, 2025. This program earned four (7) points in the Education Component, two (2) points in the Program Component and one (1) Quality point. A complete walk-through of the home was conducted. Upon arrival, you were providing care to three (3) preschool-aged children ranging in ages from one (1) year of age to two (2) in space approved to care for children. The children were playing with toys made available in the room set up and used for child care. Children ate a AM Snack and lunch. You completed hand washing before meals and after. Children were observed toileting and preparing for nap and napping. AM snack consisted of cheese puffs, water, and milk. Lunch consisted of: meatballs with tomato sauce, sweet peas, watermelon, sliced wheat bread, and milk. Caregiver interacted with children in positive manner. The following violations were documented today: Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Playground had pieces of shredded paper and cracked plastic Easter egg accessible to children. .1719(a)(1)&(17) 716 Electrical outlets not in use were not covered. Electrical outlets were not covered in the kitchen and living room. 10A NCAC .1719(a)(27) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The EPR Plan was last updated on March 31, 2025. .1714(d)(8) The violations documented must be corrected immediately. The program’s compliance history was 88% percent as of September 29, 2025. OWNER & FACILITY PROFILE This program is owned by you, Iesha Fitts. Should the status of ownership change please contact me, or your assigned Consultant, prior to doing so. I reviewed the contact and operator information for the program with you. You verified the current information listed with DCDEE remains correct. OPERATOR RECORDS: I monitored your records, as the operator, and it was found to be in compliance with Child Care Rules. PROGRAM RECORDS: Fire Drills/Emergency Drills, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and License were reviewed and not current. Emergency medical care plan was last updated on 3-31-2025. I reminded you that this form must be completed annually. This was corrected during the visit. CHILDREN’S RECORDS: I monitored children records, and they was found to be in compliance with Child Care Rules. MEDICATION: You stated you do not administer medication. TRANSPORTATION: You stated you don't provide transportation. SAFE INDOOR AND OUTDOOR ENVIRONMENT: Electrical outlets were not in use with covered with safety plugs in kitchen and living room area. I encourage you to check all outlets throughout the home to ensure they are covered with safety plugs when not in use. I encourage you to do a walk through of the playground at the beginning of the day and at the end of the day to make sure the playground is free of trash, debris, or broken toys to ensure it's safe for children to play. TECHNICAL ASSISTANCE: We discussed the importance of keeping the indoor and outdoor free of items that can cause a child to fall or get hurt, keeping environment clean, and making sure playground is free of trash or broken items that can cause a child to get hurt. We discussed you linking yourself and if you hire a additional caregiver the process of linking to ABCMS. You stated that you are aware of the process and will immediately link yourself. You informed me that you are in the process of hiring a new staff. I reviewed the forms that you had available for me to check to see if the person had what they need to be considered as a additional caregiver. We reviewed the additional and substitute checklist on the DCDEE website and discus what the person will need. I encourage you to do a complete walk through of your home during all shifts to ensure all electrical outlets are covered when not in use. DHHS Criminal Background Checks: Basic Information North Carolina Child Care Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. Once you have been notified that access has been granted, log into the provider portal to locate the unique provider code for the facility. You, Iesha Fitts, operator, were present and assisted me today. A temporary license was issued for this program on April 17, 2025, and is valid through October 17, 2025. The temporary license allows for all three shifts and a capacity of 8 children, ages 0 to 12 years old, per shift. Restrictions on the license include daytime and overnight care, maximum of 5 preschool children at any time, and the fireplace/woodstove not used during operating hours. The facility is owned by you as the sole proprietor. Contact me prior to making any ownership changes, such as developing a corporation. Upon arrival, you were providing care to five (5) preschool-aged children ranging in ages from birth to one (1) year of age. The children were playing with toys made available in the room set up and used for child care. You fed a child a bottle during the visit and also prepared and served lunch to the children. A variety of age-appropriate learning materials were observed in the child care area. When asked, no one else was present in the home. I checked all rooms at the home today as well. The children were served lunch and prepared to rest before the conclusion of the visit today. Prior to the children eating lunch, they did not wash their hands. Two (2) infants were served milk in a bottle and neither of their hands were washed. Both infants had head and neck control. We discussed ensuring you assist the children with hand washing before and after meals. You did assist the children with hand washing after they finished eating before they rested on their covered mats. Program records were also monitored today along with children’s records. Continue completing monthly fire drills and outdoor playground inspections. The following violations were documented today: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Two (2) cans of paint were stored in a room adjacent to the room used to care for children. The laundry room was not locked that stored laundry detergent, insect and pest control liquid, Clorox wipes, and other chemicals. .1719 (a)(7) 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Documentation was not on file verifying an outdoor playground inspection was completed in July 2025. 10A NCAC .1721(e)(5)(A-F) 804 Soiled diapers were not placed in a covered, leak-proof container which was not emptied and cleaned daily. Soiled diapers were stored in an uncovered container in the bathroom, accessible to children. .1725(a)(6)(D) 816 Each child's hands were not washed before and after eating. None of the five (5) children present washed their hands prior to drinking a bottle and eating lunch. .1725(a)(7)(B) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A health assessment was not on file for two (2) children that were enrolled July 14, 2025 and August 25, 2025. A health assessment was completed September 19, 2025 for a child that was enrolled August 11, 2025. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Immunization records were not on file for two (2) children, who were enrolled August 11, 2025 and August 25, 2025. GS 110-91(1); .1721(a)(2) The violations documented need to be corrected immediately. Mail or email me a compliance letter detailing the correction of each violation on or before October 13, 2025. TECHNICAL ASSISTANCE & CONSULTATION: CHILDREN’S RECORDS: Children's records consist of various documentation such as a child's medical and immunization history, emergency medical care information, written plan of care, and parental acknowledgements. A health assessment/medical can be accepted if completed within the last 12 months prior to a child’s enrollment provided the information listed on DCDEE’s medical form is answered. Immunization records are required to be on file within a preschool aged child’s first 30 days of enrollment. Immunization records were not on file for a couple of children. A medical exam was not on file for two children, and one medical exam was on file more than 30 days after a child was enrolled. STORAGE OF POTENTIALLY HAZARDOUS ITEMS, INCLUDING LIGHTERS: Two (2) cans of paint were observed stored in a bedroom that was not locked today. The room is located across for the bathroom, used by the children. The laundry door, that can be locked, was not locked today and the key was stored in the lock of the doorknob. There were items in the laundry room such as Clorox wipes, insect and pest spray, and laundry detergent. The door was locked, and the key was placed on the nail on the wall, that can not be accessed by the children. You also removed the paint from the bedroom and put the paint in the laundry room to make it inaccessible as well, correcting the violation. A lighter was observed on the couch near the laundry room today. You also removed the lighter during the visit to correct the violation. Keep any items such as lighters and matches in locked storage. The laundry room and the living area, where the lighter was observed, are separated with a child gate that is positioned in the kitchen area. You, Iesha Fitts, operator, were present and assisted me today. A temporary license was issued for this program on April 17, 2025, and is valid through October 17, 2025. The temporary license allows for all three shifts and a capacity of 8 children, ages 0 to 12 years old, per shift. Restrictions on the license include daytime and overnight care, maximum of 5 preschool children at any time, and the fireplace/woodstove not used during operating hours. The facility is owned by you as the sole proprietor. Contact me prior to making any ownership changes, such as developing a corporation. Upon arrival, you were providing care to five (5) preschool-aged children ranging in ages from birth to one (1) year of age. The children were playing with toys made available in the room set up and used for child care. You fed a child a bottle during the visit and also prepared and served lunch to the children. A variety of age-appropriate learning materials were observed in the child care area. When asked, no one else was present in the home. I checked all rooms at the home today as well. The children were served lunch and prepared to rest before the conclusion of the visit today. Prior to the children eating lunch, they did not wash their hands. Two (2) infants were served milk in a bottle and neither of their hands were washed. Both infants had head and neck control. We discussed ensuring you assist the children with hand washing before and after meals. You did assist the children with hand washing after they finished eating before they rested on their covered mats. Program records were also monitored today along with children’s records. Continue completing monthly fire drills and outdoor playground inspections. The following violations were documented today: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Two (2) cans of paint were stored in a room adjacent to the room used to care for children. The laundry room was not locked that stored laundry detergent, insect and pest control liquid, Clorox wipes, and other chemicals. .1719 (a)(7) 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Documentation was not on file verifying an outdoor playground inspection was completed in July 2025. 10A NCAC .1721(e)(5)(A-F) 804 Soiled diapers were not placed in a covered, leak-proof container which was not emptied and cleaned daily. TECHNICAL ASSISTANCE & CONSULTATION: CHILDREN’S RECORDS: Children's records consist of various documentation such as a child's medical and immunization history, emergency medical care information, written plan of care, and parental acknowledgements. A health assessment/medical can be accepted if completed within the last 12 months prior to a child’s enrollment provided the information listed on DCDEE’s medical form is answered. Immunization records are required to be on file within a preschool aged child’s first 30 days of enrollment. Immunization records were not on file for a couple of children. A medical exam was not on file for two children, and one medical exam was on file more than 30 days after a child was enrolled. STORAGE OF POTENTIALLY HAZARDOUS ITEMS, INCLUDING LIGHTERS: Two (2) cans of paint were observed stored in a bedroom that was not locked today. The room is located across for the bathroom, used by the children. The laundry door, that can be locked, was not locked today and the key was stored in the lock of the doorknob. There were items in the laundry room such as Clorox wipes, insect and pest spray, and laundry detergent. The door was locked, and the key was placed on the nail on the wall, that can not be accessed by the children. You also removed the paint from the bedroom and put the paint in the laundry room to make it inaccessible as well, correcting the violation. A lighter was observed on the couch near the laundry room today. You also removed the lighter during the visit to correct the violation. Keep any items such as lighters and matches in locked storage. The laundry room and the living area, where the lighter was observed, are separated with a child gate that is positioned in the kitchen area.
