Currently, this facility operates with a Four-Star license issued March 22, 2018. The last annual compliance visit was completed on April 1, 2025. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on March 11, 2026, earning a superior classification and “0” demerits. The last satisfactory fire Inspection was completed on September 24, 2025, and was approved for daytime care only. K. Boyd, Principal and T. McElihnney, Lead Teacher was present and available to assist. Fifth-teen (15) children ages four (4) and (5) years old were present. Children were observed during circle time, library, free choice centers, teacher directed art activity, toileting, handwashing, and preparing for lunch. Lunch consisted of Pizza cheese sticks with Manaria sauce or chicken salad with crackers, sweet peas, mandarin oranges, orange medley vegetable juice, and milk. All staff interacted with children in a positive manner. Off-site forms were available for review for staff and transportation. Playground inspections, fire and lockdown drills were not current. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. NC PreK requirements were monitored during today’s visit. The following violations were documented today: Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last completed playground inspection was last conducted September 25, 2025. .0605(q) 1061 One staff member had not completed training in playground safety. Staff member T. McElhinney don't have playground and safety training. .1102(e) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Staff members were not linked into the ABCMS Portal. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by April 8, 2026. Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 97%. CONSULTATION: PROGRAM RECORDS: Fire Drills/Emergency Drills, Fire inspection, Sanitation report, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and License were reviewed and not current. Playground Outdoor Inspections: You T. McElhinney informed me that playground outdoor inspections had not been being completed by yourself due to you not having the playground safety training. Playground outdoor inspections were not available for review for the months of: October 2025, November 2025, December 2025, January 2026, and February 2026. I explained that you must have this training completed within six months of employment. CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children records were monitored. Children records reviewed were in compliance. You should review other children’s records not monitored to ensure compliance for the same information. Ten percent (10%) or more of staff records were monitored. Staff records reviewed were not in compliance. Staff should review records that were not monitored to ensure compliance. You informed me T. McElhinney that you would be leaving as of June 3, 2026. You, informed that you nor any other staff member has Playground and Safety training. You stated that you have been searching for the training and hadn't found anyone offering it. You will have two-weeks to meet compliance with this violation. You will need to request a extension from the Division if you have not taken the class on or before April 8, 2026. Please submit request to me if you will need a extension. I recommend checking with the local Smart Start agency to see if they will offer the training. I verified new staff member substitute M. Simmons record and it was found in compliance. ABCMS PORTAL and Criminal Background Check: All staff were verified in ABCMS Portal has a valid qualification letter on file in the ABCMS person summary and not linked to ABCMS employee roster in ABCMS Portal. I explained to you K. Boyd and T. McElhinney that all staff need to be linked the ABCMS Technical Assistance Instructions to assist with linking new hires. K. Boyd started working on the process during the visit. QRIS: I discussed with you (T. McElhinney) the status of Pathway 1 and you stated that you have started the self study and that the program will have a Easter Egg Hunt on Friday, March 27 for the children and you said that you sent notices to parents inviting them to come and participate. I encouraged you to record and keep all documentation to ensure you are meeting Family and Community Engagement options. DCDEE WORKS: All staff should ensure they have an education qualification letter on file. Each staff member should create an account, apply for DPI teacher or DPI teacher assistant, and submit transcripts and licensure if not already done so. PROVIDER DOCUMENTS/FORMS: You should visit the website often to ensure you’re using all updated forms. All requirements must be included if you choose to use your own forms electronically or hard copy. Child care rules updated effective July 1, 2023, and January 1, 2024. TECHNICAL ASSISTANCE: Playground Safety This training addresses playground safety hazards, playground supervision, maintenance and general upkeep of the outdoor learning environment, and age and developmentally appropriate playground materials and equipment. To meet requirements for Playground Safety training, at least one staff member shall complete the training. If you are employed with a North Carolina licensed child care center, this training may be required as part of your professional development. Completion of playground safety training shall be included in the number of hours needed to meet annual on-going training requirements. If required to comply with Rule .1102(e), you shall have six months from the date of employment, or from the date a vacancy occurs, to complete the required safety training. A certificate of completion of this course shall be maintained and available for review upon request. Playground Safety training may be received in-person, or through real-time virtual training. Distance learning shall not be permitted for playground safety training. Lunch consisted of choice of Pizza or Peanut Butter and Jelly sandwich, broccoli, carrots, sliced apples, and milk. Staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. STAFF RECORDS: New Staff member T. McElhinney hired 8-19-2025 record was reviewed and found in compliance. STAFF RECORD: Substitute record was not on file or available for review. Staff explained she was hired through Vance County School System. I explained to you, that all staff must have records on file and available for review. All information required for a substitute should be completed prior to employment. I encourage you to utilize the substitute checklist located under provider tab on the Division website. Criminal Background Check: I discussed with You, K. Boyd Principal, staff B. Baker (Teacher Assistant) was working in classroom without a CBC qualifying letter on file and was not completed prior to employment. I explained to K. Boyd and staff member that she could not work at this program with out qualifying CBC. The staff completed a application through Vance County Schools System and was transferred to PRE-K. I encouraged administrator to go on the Division website and review the substitute checklist when hiring staff for NC Pre-K programs. You removed teacher assistant during the visit and replaced substitute with K. Burwell. K. Burwell file was reviewed and found in compliance with applicable child care requirements. CRIMINAL BACKGROUND: New feature links criminal background applications to facility North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. QRIS: I discussed with you the Pathways to Stars QRIS modernization. I reviewed the QRIS information from the DCDEE website. We discussed each pathway and you stated that you are interested in Pathway 1. I discussed with you and showed you how to navigate on the website for NCRLAP to learn about the self-study and resources to assist. T. McElhinny shared Rated License Assessment Application during the visit. I recommend for you to review and print QRIS Rules section .3200 from the Child Care Rules. I have included information below from DCDEE website that discusses Section. 3200. Click on the tab under "What's New" and then scroll down to "QRIS MODERNIZATION" you will then find the information for the forms for each pathway, questions and answers, and resources. The following violations were documented today: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An electrical outlet on the surge power plug was uncovered located behind the cots area near the teacher work station. 10A NCAC 09 .0604(c) 1041 Prior to employment a Criminal Background Check was not completed. This was corrected during the visit. 10A NCAC 09 .0604 SAFETY REQUIREMENTS FOR CHILD CARE CENTERS STAFF RECORD: Substitute records was not on file or available for review. Staff explained she was hired through a sub contracting agency. Staff was working in classroom without a CBC qualifying letter on file. I discussed with staff how to go to DCDEE and follow guidelines for CBC. Staff member was informed that she could not work at this program with out CBC. Staff stated that she would get this completed immediately. Staff contacted Sub contracting Agency and I discussed that staff could no longer work at this program with out CBC. I suggested with agency to go on the Division website and review the substitute checklist when hiring staff for NC Pre-K programs. You, K. Boyd Principal, informed me that she was new at the school. I printed off Preservice Requirements for Administrator of a Child are Center form and left it with you to complete and email back to me. You stated that you would complete form and get it to me as soon as possible. SANITATION: Children’s Environmental Health has newly readopted child care sanitation rules. You should visit our website under the services tab, then select child care rules, law and public information, and then select section 2800 Sanitation of Child Care to review newly readopted rules. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. Currently, this facility operates with a Four-Star license issued March 22, 2018. The last annual compliance visit was completed on April 9, 2024. The last sanitation inspection was completed on September 30, 2024, earning a superior classification and “0” demerits. The last satisfactory fire inspection was completed on September 9, 2024, and was approved for daytime care only. E. Hunsinger, Lead Teacher was present and available to assist. Eighteen (18) children ages four (4) and (5) years old were present. Children were observed in eating lunch, handwashing and toiletings, free choice centers, and group/circle time. Lunch consisted of Turkey sausage, pancakes, potato rounds, veggie juice, and mixed fruit cup. Staff interacted with children in a positive manner. Playground inspections, fire and lockdown drills were current. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. The following violations were documented today: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. I suggested with agency to go on the Division website and review the substitute checklist when hiring staff for NC Pre-K programs. You, K. Boyd Principal, informed me that she was new at the school. I printed off Preservice Requirements for Administrator of a Child are Center form and left it with you to complete and email back to me. You stated that you would complete form and get it to me as soon as possible. SANITATION: Children’s Environmental Health has newly readopted child care sanitation rules. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026.
