Currently, this facility operates with a Five-Star license issued on May 3, 2018. The center's last annual compliance visit was completed on February 25, 2025. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on January 13, 2026, earning a “Superior” classification and “3” demerits. The last satisfactory fire inspection was completed on September 6, 2025, and was approved for daytime care only. L. Fields, Principal, and F. Mitchell, Lead Teacher, were present and available to assist. Thirty- five (35) children ages four (4) to five (5) years old were present. Children were observed in free choice activities, teacher directed activities, group/circle time, outdoor play, preparing for lunch, handwashing and toileting procedures, eating lunch, preparing for nap and napping. Lunch consisted of cheeseburgers, corn, green beans, strawberry cup, and milk. All staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. NC PreK requirements were monitored during today’s visit. The monitoring tool was onsite and available for review. The following violations of child care requirements were documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The most current fire inspection was completed on September 26, 2025 and was not recorded on fire inspection form provided by the Division. 10A NCAC 09 .0304(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Substitute D. Best did not have First Aid training on file for review. .1102(c) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Two staff members were hired and not linked to ABCMS. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by March 20, 2024. Fire inspection: Fire inspections was conducted on 9-4-2024 and was not submitted to DCDEE and fire inspection must be filled out on fire inspection form provided by DCDEE located under the "provider" tab under "forms and documents". You stated that you have informed program coordinator about the expired fire inspection. I talk with S. Alston, PreK Coordinator via phone during the visit and she stated that she has called and was told that Vance County has a new fire chief and that it would be toward the end of the month before he comes out to conduct inspection. I encouraged you to request an extension for the above violation if it is pass 14 days to ensure you are in compliance with applicable child care requirements. STAFF RECORD: Substitute record was not on file or available for review. Staff explained she was hired through Vance County School System. I explained to you, that all staff must have records on file and available for review. All information required for a substitute should be completed prior to employment. I encourage you to utilize the substitute checklist located under provider tab on the Division website. Criminal Background Check: Staff was working in classroom without a CBC qualifying letter on file. I discussed with staff how to go to DCDEE and follow guidelines for CBC. Mitchell, Lead Teachers, were present and available to assist. Thirty- one (31) children ages four (4) to five (5) years old were present. All staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. Currently, this facility operates with a Five-Star license issued on May 3, 2018. The center's last annual compliance visit was completed on April 4, 2023. The last sanitation inspection was completed on April 20, 2023, earning a “Superior” classification and “4” demerits. The last fire inspection was completed on March 6, 2023, on the NC Public School Fire inspection form. Dr. M. Putney, Principal, was present during the visit. J. Adams and F. Mitchell, Lead Teachers, were present and available to assist. Thirty-four (34) children ages four (4) to five (5) years old were present. Children were observed in free choice activities, toileting, handwashing and preparing for lunch. All staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. The following violations of child care requirements were documented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Two electrical outlets in space 156 were not covered when not in use. 10A NCAC 09 .0604(c) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member in space 156 and one staff member in space 152 did not renew certifications on or before expiration. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member in space 156 and one staff member in space 152 did not renew certification on or before expiration. .1102(d) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. There was no documentation of health and safety trainings being completed for two staff members. One in space 152 and one in space 156. .1103(b) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by October 10, 2023. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 96%. TECHNICAL ASSISTANCE/CONSULTATION CPR/FIRST AID: CPR and first aid should be renewed on or before expiration of certification. One staff member in space 156 and space 152 did not renew certifications on or before expiration. The staff member in space 152 renewed certification on July 24, 2023. However, it was not renewed before expiration date of June 2023. ELECTRICAL OUTLETS: Electrical outlets not in use should be covered with a safety plug. Electrical outlets in space 156 were not covered when not in use. This was corrected during the visit when staff covered the outlets. HEALTH AND SAFETY TRAININGS: Health and safety trainings should be completed within one year of the hire date and five years thereafter. A staff member in space 156 did not have documentation of health and safety trainings within one year of hire date. The staff member stated that she had issues with logging into MOODLE with her NCID and she had been emailing the noted email for assistance. I informed the staff to call. Both numbers for MOODLE assistance and NCID were provided to the teacher. A staff member in space 152 also did not have documentation of health and safety trainings within five years of completing the previous trainings. SANITATION UPDATED RULES: Children’s Environmental Health has newly readopted child care sanitation rules effective July 1, 2023. You should review new sanitation rules on our website to ensure compliance. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. Your facility is in cohort one, and this is your preparation year. Your reassessment year will start July 1, 2024. I encourage you to start preparing. EARLY CHILDHOOD EQUIVALENCY EXAM: The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam.
