T. Yancey, Teacher was present and assisted with today's visit. C. Wilkins, Principal was present during today's visit. Currently, this facility operates with a Five-Star license issued May 28, 2019. The last annual compliance visit was completed February 10, 2025. The last sanitation was completed on May 21, 2025, earning a "superior" classification and "0" demerits. The last satisfactory fire inspection was completed on September 28, 2025. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. NCPREK requirements were monitored, and the monitoring tool was observed. Twenty-four children were present and observed ranging ages four (4) and five (5) years old. Children were observed in group/circle time, free choice centers, handwashing and toileting procedures, transitioning for lunch, eating lunch, and preparing for nap. Lunch consisted of popcorn chicken, sweet potatoes, mandarin oranges, biscuit, eating PM snack (goldfish and orange tangerine juice), and milk. All staff interacted with children in a nurturing and caring manner. The following violation was documented today: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Operator did not submit fire inspection visit within one week of the inspection. 10A NCAC 09 .0304(a) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by January 28, 2026. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how you corrected the violation. PROGRAM RECORDS: Fire Drills/Emergency Drills, Fire inspection, Sanitation report, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and were reviewed and current. Fire Inspection: I reminded you that you have to submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. You submitted the fire inspection report during today's visit. CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children records were monitored. Children records reviewed were in compliance. You should review other children’s files not monitored to ensure compliance for the same information. Ten percent (10%) or more of staff files were monitored. Staff records reviewed were not in compliance. Staff should review files that were not monitored to ensure compliance. Offsite forms for transportation, staff and children were observed. QRIS: J. Lundy teacher was present at the QRIS meeting held by me and another consultant on 9-17-2025. I discussed with you C. Wilkins and T. Yancey the Pathways to Stars QRIS modernization. I reviewed the QRIS information from the DCDEE website. We discussed pathway 1 and you stated that you are interested in Pathway 1. I showed you how to access the Application For a Rated License under the provider tab under documents and forms. I reviewed the Education Standards with you. I discussed with you the Workforce Unit to ensure all staff education is uploaded. I discussed with you and showed you how to navigate on the website for NCRLAP to learn about the self-study and resources to assist. I recommend for you to review and print QRIS Rules section .3200 from the Child Care Rules. I have included information below from DCDEE website that discusses Section. 3200. Currently, this facility operates with a Five-Star license issued May 28, 2019. The last annual compliance visit was completed April 20, 2023. A checklist was used to note the requirements monitored during this visit. The last sanitation inspection was completed on December 13, 2023, earning a superior classification and “4” demerits. The last satisfactory fire inspection was completed on March 13, 2024. J. Lundy, Lead Teacher, was present and available to assist. Twenty-nine (29) children ages four (4) and five (5) years old were present. Children were observed in free choice play, teacher directed activities, preparing for lunch, toileting, and handwashing. All staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. NCPREK requirements were monitored and the monitoring tool was observed. The following violations of child care requirements were documented.
