    Name of Operation: ANGELS & CO Facility ID: 91000305 Consultant: KESHIA HAYWARD Operation Type: Family CC Home Case Number: Visit Date: 7/24/2023 Number Present: 5 Completed Date: 7/24/2023 Age: From 1 To 4 Total Minutes: 200 Time In: 09:40 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your facility for compliance with applicable child care requirements for an annual compliance visit. J. Carver, Operator assisted with the visit. D. Hawley, volunteer was also present and assisting. I visited the indoor and outdoor spaces designated for child care in your facility. Five (5) children were observed in care today ranging from ages one (1) to four (4) years old. Children were engaged in free-choice play using age-appropriate materials, outdoor play, interacting with the caregiver, completing toileting and hand washing routines and eating lunch. Lunch consisted of hot dogs, French fries, mixed fruit, and milk. Care was provided in a positive and nurturing manner. Currently this family child care home operates with a Four-Star License, issued February 2, 2023, earning seven (7) points in education, two (7) points in program standards and one (1) quality point for serving no more than two (2) infants under age one (1). This is your first annual compliance visit since licensing. The program’s compliance history score was reviewed with the operator. The compliance history score for your family child care home was 100% as of July 23, 2023. The following violation(s) were documented: Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. Bottles of Tylenol, prescription cream and cough medicine was stored in an unlocked container on top of the refrigerator. .1719(a)(5) 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The written record of up-to-date vaccination on file for one pet expired on July 6, 2023. .1719(b)(1) 1401 Prescribed medications were not stored in their original containers in which they are dispensed with the pharmacy label. A tube of Triamcinnolone on site for one child was not store in its original container. 10A NCAC 09 .1720(c)(1)&(2) 1741 Prior to enrollment of children in a Family Child Care Home, and before animals come into the Family Child Care Home, the operator did not obtain each parent's signature on a form acknowledging the type of animal located on the premises and where the animal will be kept during child care operating hours and /or the signed form was not maintained in the child's file. The pet acknowledgment form signed by the parents was not on file for two children enrolled. 10 NCAC 09 .1719(b)(3) 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. Documentation was not on file and available to verify the operator provided written statement to parents of children enrolled regarding the smoking and tobacco restriction. .1719(a)(11) The violation(s) documented must be corrected immediately. On or before August 7, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your family child care home are correct. If changes in your facility information occurs, please contact me at telephone number 252-214-2709 or email keshia.hayward@dhhs.nc.gov to discuss the changes and ensure the information is updated in the system. LICENSEE: You are listed as the sole proprietor on the child care license. Prior to changing the facility name and/or ownership (i.e., forming a corporation), please contact me to discuss change of ownership procedures. Failure to comply may affect your child care license. MEDICATION: Tylenol, cough syrup and prescription and non-prescription creams were observed in a basket on top of the refrigerator. I reminded you all medications, except rescue medicine must be stored in a locked space. You stated the medication would be placed in a locked closet or cabinet moving forward. RECORDKEEPING: The pet acknowledgment and tobacco restriction forms were not on file and available for review during the visit. I advised you to review both documents with parents and have them sign documents acknowledging the information was reviewed and provided. I recommended creating a sample file or having and enrollment packet prepared for parents to ensure all documentation is available to be completed and provided for children records. I also recommended conducting periodic reviews of files to ensure all documentation is on file. TRANSPORTATION REQUIRMENTS: You stated transportation services are not currently provided for children enrolled. You added you plan to start transporting children in the future to off-premises activities. I recommended you review the transportation requirements of the child care rules. You should ensure all requirements are met prior to transporting children. Also contact your child care consultant to discuss meeting requirements for transportation. EARLY CHILDHOOD EQUIVALENCY EXAM: The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam. STAR RATED LICENSE: I informed you information regarding the resumption of the star rated license assessment process is located on the Division of Child Development and Early Education and North Carolina Rated License Project website. I advise you to visit ncrlap.org to obtain detailed information about how the process will be implemented. You can also complete training modules located in MOODLE platform, access provider's documents, and the child care rules and laws.
