This visit was conducted by Karen Young, Childcare Consultant, with Natasha Brown, Teacher. Lead teacher, Melinda Woodruff was out during today’s visit. Your program currently operates with a Five (5) Star license issued 4/25/18. Restrictions include daytime care only; meets enhanced ratios; meets enhanced space; school age equipment cannot be used by preschoolers. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. I observed one violation on the outdoor playground. I observed some Kleenex tissues, an empty water bottle and a candy wrapper on the play area. I observed the fence does not go down to the ground level on the right side of the playground. The opening is four (4) inches from the ground to bottom of fence. The opening is to be no more than three (3) inches. I also observed a tripping hazard around the sidewalk where the ground has washed away. I observed two violations in the indoor classroom. There was a substitute teacher in place of M. Woodruff today. I observed the substitute teacher’s file and her CPR and First Aid certification expired on 10/21/23. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center play and cleaning up for circle time during the visit today. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum and Teaching Strategies Gold with developmental domains listed on the lesson plans. The last annual compliance visit was conducted on 1/17/23. A sanitation inspection was completed 11/1/23 with a Superior classification and fifteen (15) demerits. The last fire inspection was conducted 9/11/23 by Keith Walsh. A copy of this fire inspection was received today. Program records and required postings were monitored. A fire drill was conducted on 11/29/23 at 10:27 am with one (1) minute, thirty (30) seconds to evacuate forty-nine (49) children and four (4) adults by principal, R. Kilpatric. A shelter-in-place drill was documented on 11/9/23 at 10:15 am with fifty-seven (57) seconds to shelter-in-place by R. Kilpatric. An outdoor inspection was documented on 12/4/23 by N. Brown with two (2) items noted on the playground inspection checklist. Ms. Brown had documented the fence opening at the bottom as well as the potential tripping hazard at the sidewalk. Staff files and children’s records were monitored per DCDEE procedures. Six (6) children’s files were monitored today, two (2) from each classroom. Two (2) staff files were monitored during the visit, one (1) teacher and one (1) substitute. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. I observed no medications and no allergies in Space 173. In Space 174, I observed a peanut allergy with an Epi-Pen and Benadryl with medication paperwork, Benadryl for the bee sting allergy with medication paperwork and an inhaler with medication paperwork. In Space 172, I observed an inhaler with medication paperwork. You stated that Wilkes County Schools provides transportation in the am & pm for NC Pre-K children. During today’s visit, I monitored the Transportation Policies for Parents signed 8/22/23 by parents, the Public-School Off-Site Transportation Records Verification signed 8/23/23 by Westley Wood, Assistant Superintendent for Wilkes Couty Schools. I observed Off-Site Transportation Requirements signed by Eric Barker, Director of Transportation, on 8/23/23. The School Bus Driver and Procedures Orientation was also observed and signed on 8/22/23 by six (6) bus drivers. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff is the lead teachers, M. Woodruff, J. Johnson, and N. Brown. Teacher assistants are K. Absher, C. Whitley, and S. Lily. A selection of six (6) children’s files was monitored for completed health assessments and developmental screenings. Ms. Woodruff returned to the school during the visit and stated that she has finished the children’s Fall assessments. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses the Creative Curriculum with Teaching Strategies Gold and developmental domains. The DIAL-4 screenings are conducted prior to the children entering NC Pre-K. This NC Pre-K program uses the Teaching Strategies Gold instrument for ongoing formative assessments. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first checkpoint was due November 9, 2023. The first day of school was 8/28/23. The classroom operates from 7:25 a.m. to 3:05 p.m. Parent conferences are held two (2) times per year. You stated that you use email to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by sending at-home activities to families through the class Dojo app, as well as information regarding home activities connected to the current classroom themes, hold parent-teacher conferences twice yearly, email parents regularly, conduct a Parent Orientation Meeting at the beginning of the school year with individual families, host an Open House event for all families, send home folders with children’s work samples, invite parents to donate materials to and attend classroom special activities and celebrations. Parents also engage in Preschool Pyramid Model activities with their children to support their social/emotional health. The following violations were cited at today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the outdoor environment, I observed trash, including kleenex tissues, an enpty water bottole and a candy wrapper. I observed to the right of the playground, an opening at the bottom of the fence, between the ground and the bottom of the fence measuring four (4) inches. I also observed a tripping hazard at the sidewalk where mulch needs to be added to build up to the sidewalk. 10A NCAC 09 .0601(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. I observed a substitute who expired in her First Aid certification on 10/21/23. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. I observed a substutute who expired in her CPR certification on 10/21/23. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Wednesday, December 20, 2023. Contact your local Wilkes Partnership for Children to schedule an upcoming training. 10A NCAC 09 .1102 (d) All staff who provide direct care or accompany children when they are off premises shall successfully complete certification in a cardiopulmonary resuscitation (CPR) course appropriate to the ages of children in care. At all times when children are in care one staff member present must have successfully completed CPR training. The training shall be completed by June 30, 2018 or for new staff hired on or after September 1, 2017 training must be completed within 90 days of employment. Distance learning shall not be permitted for CPR training. CPR training shall be renewed on or before the expiration of the certification. Verification of each staff member's completion of this course from an approved training organization shall be maintained in the staff member's file in the center. The Division shall post a list of approved training organizations on its website at http://ncchildcare.nc.gov/providers/pv_sn2_ov_pd.asp. All staff who provide direct care or accompany children when they are off premises shall successfully complete certification in First Aid appropriate to the ages of children in care. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff is the lead teachers, M. Woodruff, J. Johnson, and N. Brown. Teacher assistants are K. Absher, C. Whitley, and S. Lily. A selection of six (6) children’s files was monitored for completed health assessments and developmental screenings. Ms. Woodruff returned to the school during the visit and stated that she has finished the children’s Fall assessments. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses the Creative Curriculum with Teaching Strategies Gold and developmental domains. The DIAL-4 screenings are conducted prior to the children entering NC Pre-K. This NC Pre-K program uses the Teaching Strategies Gold instrument for ongoing formative assessments. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first checkpoint was due November 9, 2023. The first day of school was 8/28/23. The classroom operates from 7:25 a.m. to 3:05 p.m. Parent conferences are held two (2) times per year. You stated that you use email to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by sending at-home activities to families through the class Dojo app, as well as information regarding home activities connected to the current classroom themes, hold parent-teacher conferences twice yearly, email parents regularly, conduct a Parent Orientation Meeting at the beginning of the school year with individual families, host an Open House event for all families, send home folders with children’s work samples, invite parents to donate materials to and attend classroom special activities and celebrations. Parents also engage in Preschool Pyramid Model activities with their children to support their social/emotional health. The following violations were cited at today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the outdoor environment, I observed trash, including kleenex tissues, an enpty water bottole and a candy wrapper. I observed to the right of the playground, an opening at the bottom of the fence, between the ground and the bottom of the fence measuring four (4) inches.
