The last annual compliance visit was conducted 4/16/24 Currently this program operates with a five (5) Star License, issued 8/29/19. The facility earned seven (7) points in staff education, six (6) points in program standards, and one (1) quality point for education options met. Kenisha Edwards, teacher, assisted me today. I conducted an unassisted walk-through of the center. The last sanitation inspection was completed 11/19/24 with a ‘Superior’ classification and 7 demerits. The last fire inspection was conducted on 9/13/24. Children were observed playing outside, playing in activity areas, group activities, eating lunch, and routine care activities. Transportation is provided by children’s parents. This facility has four NCPK classrooms with thirty-four NCPK and six EC children enrolled. NC Pre-K requirements were not monitored during today’s visit. The files for three new staff members were reviewed today. Today a partial assessment of applicable childcare requirements was conducted. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The qualification letter for G. Mendoza was due 9/26/24 and received 10/10/24. The qualification letter for K. Edwards was due 9/19/24 and received 10/4/24. Technical assistance was provided. G.S. 110-90.2(b) & .2703(n)&(o) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Edwards. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or 12/18/24. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. CONSULTATION: All licensed childcare facilities are currently under the ‘Hold-Harmless’ state for their license from DCDEE. Once QRIS is implemented, you will receive information from DCDEE regarding the new pathways for licensure and your timeframe for completing your license renewal. The NCRLAP website, https://ncrlap.org, has trainings geared towards the new ECERS3 assessment scales. These will introduce you to the updated assessment scale and provide tips for completing the new scale. It is best practice to begin the renewal process for the criminal background letters at least three months in advance. This allows time for scheduling of an appointment or any technical issues that may arise. Fuller does not state that she completed the course for childcare including pediatric (infant/child) CPR. A copy of the course description must be sent to me no later than 12/11/24. A checklist was used to note the requirements I monitored today, and a copy was left with you. The last annual compliance visit was conducted 4/25/23. Currently this program operates with a five (5) Star License, issued 8/29/19. Transportation is provided by children’s parents. This facility has six NCPK classrooms with eighty-two children enrolled. NC Pre-K requirements were monitored during today’s visit. The program operates for 6.5 hours a day. A monthly breakfast and lunch menu was posted in each classroom. Ten NC Pre-K children's files were monitored and all contained a health assessment within 30 days, a DIAL screening within 90 days, and evidence of each child's on-going progress. The teachers use Tools of the Mind and activity plans were current and posted. Staff/child ratio of 1:9 or less is being maintained. The parents receive a monthly newsletter, daily notifications via class DOJO, and are able to volunteer throughout the school year. A volunteer log is maintained. The teachers also have parent meetings and conferences three times a year. The current ECERS-R scores were received 2/25/16. The facility received scores of 5.78 and 6.14. Today a full assessment of applicable childcare requirements was conducted. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. One bumper on the upper playground was split open around one of the bolts. On the lower playground the following items were observed: wooden frame on planter box was falling apart with exposed screws, screws on chairs on the 'bus' were raised up, bolts holding the bumpers together were raised up, pallet board had peeling paint and exposed nails, chalkboard frame had peeling paint and exposed nails, one section of bumper was split open. 10A NCAC 09 .0604(p) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Staff member S. Russell was short seven hours of ongoing training. These were due by 2/12/23. .1103(a) 1898 Staff did not complete the health and safety training within one year of employment. The trainings for S. Russell were due by 2/13/24 and were not completed. .1102(a) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. We discussed creating a substitute teacher list of all staff who have a valid qualification letter for teachers to use when a teacher must be out. We reviewed the requirements from Environmental Health regarding lunches brought from home and how to properly store them.
