S. Richards, Operator, assisted me with the visit. Your program currently operates with a four-star license, issued 4/8/2023, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under one year of age. The last annual compliance visit was conducted on 4/23/25. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent as of 3/31/26. The NC Secretary of State website was reviewed on 3/31/26 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. All indoor and outdoor areas were monitored. Children were observed to be engaged in free play in activity centers. The children transitioned to the outdoor space and were observed swinging, climbing on the stationary structures, and running relay races. Upon transitioning inside, the children completed personal care routines and washed their hands. The outdoor space was observed and included stationary climbers, swings, riding toys, and balls to engage in gross motor activities. The home has an enclosed porch with a variety of toys and activities for the children to engage in. Lunch was observed and consisted of chicken nuggets, French fries, fresh mixed fruit, garlic bread, and milk. The following violations were documented. Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. The operator and additional caregiver had certification of First Aid that expired 3/26. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. The operator and additional caregiver had certification of CPR that expired 3/26. .1703(a)(3) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not complete the ABCMS Provider Portal Training. G.S. 110-90.2 & .2703(r) 2034 Operator did not complete an incident report, following an injury to a child. An incident report was not completed for a child that received a minor injury while in care. .1721(e )(3) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Being trained in CPR and first aid can be invaluable when someone is in serious medical distress. Therefore, it is so important that all staff working directly with children have current CPR and First Aid Certification. Staff must renew certification every 2 years. The operator and additional caregiver had certification of CPR/First Aid that expired 3/26. You stated that you will call and set up a training as soon as possible and forgot that it was due. We discussed putting a reminder in your digital calendar to send you a reminder several months in advance of the training expiring and placing the verification of required information form with staff files as a quick reference. Review staff files regularly to ensure that all trainings are taken in advance of expiration. Please send a copy of staff cards upon completion as proof of correction. Incident Reports: An incident report should be completed and logged each time a child is injured, regardless of whether medical care is needed. It is helpful to remember the “three B’s”: bumps, bruises, and blood, as a reminder of when to complete incident reports. The incident report should specify what happened to cause the injury and then by reviewed with the parent before the parent signs the report. An incident report was not completed for a child that received a minor injury while in care. During the visit, a child received a scratch on the forehead. You notified the parent that the child received a scratch but did not complete an incident report. You stated that you complete incident reports for injuries. We discussed placing copies of an incident report on the clipboard so that they are available and ready if a child is injured. Complete an incident report any time a child receives an injury while in care and have the parent sign. Add the incident report to the incident report log. ABCMS: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. You completed the training during the visit to correct the violation. You must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. S. Richards, Operator, assisted me with the visit. Your program currently operates with a four-star license, issued 4/8/2023, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under one year of age. The last annual compliance visit was conducted on 6/5/23. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety three percent as of 4/7/24. The NC Secretary of State website was reviewed on 4/7/24 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. All indoor and outdoor areas were monitored. Children were observed to be engaged in free play in activity centers. The children sat at the table and drew on dry erase boards and completed puzzles. Before lunch, the children completed personal care routines and washed their hands. The outdoor space was observed and included stationary climbers, swings, riding toys, and balls to engage in gross motor activities. Lunch was observed and consisted of pizza, cucumbers, fresh mixed fruit, and milk. The following violation(s) were documented. Violation Number Comment Rule 1718 The written plan of care was not given and explained to parents of children in care on or before the first day the child attended the home. Parents did not sign a statement acknowledging the receipt and explanation of the plan. Parents did not give written permission for their child to be transported by the operator for specific routine tasks that are included on the written schedule. Four (4) out of the ten (10) children’s files reviewed, had written plans on file that were not complete. .1712(e )(6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. This ensures that routine tasks do not interfere with the care of children during hours of operation. Four (4) out of the ten (10) children’s files reviewed, had written plans on file that were not complete. We discussed that the written plan that you currently use is hard to understand and that the version located on the Division’s website could be an easier option. You stated that you are not providing transportation and will print out written plans for each child enrolled and ensure that they are current and complete before they are signed by the parents. Ensure that if there are any changes to the plan of care, that the written plan of care is updated and given to parents 30 days before the changes goes into effect. Send a copy of the written plan with your compliance letter. Additional Comments: Natural Learning Initiative: Time outdoors is a critical tool for supporting mental and physical health in young children. Adoption of best practices for outdoor play and learning spaces is critical to increase daily time spent outside. The four components of this project include: 1. Relaunch a supportive network for outdoor play and learning environments in North Carolina; 2. Research nature-based programs and spaces and develop a plan to embed these in licensed childcare in North Carolina; 3. Motivate and train early educators and specialists to promote best practices and increase time in outdoor play and learning in NC childcare facilities; 4. Increase time outdoors playing and learning in NC family child care homes through professional development and technical assistance. Today, we discussed ways to create a natural learning environment in the outdoor spaces. Some of the ideas included a music wall made from kitchen accessories, a garden that the children can help plant, a science center, and stepping stone made from logs. Rule Changes Effective January 2024 Training Modules Available in DCDEE Moodle The Child Care Commission adopted child care rule changes in January 2024. Changes relate to definitions; lead and asbestos for centers and family child care homes; building requirements for family child care homes; multi-unit child care centers; and criminal background checks. Consultants will assist as you begin to review and implement the changes, but please note, some of the rule changes may or may not impact your facility. An example is the rules in section .2600 for multi-unit child care center. These rules are specific to child care centers with multiple licensed centers within one building. Please ensure you are using the updated January 2024 rule book, and view information in the DCDEE Moodle (enroll if necessary). Personal care routines were completed after lunch before the children laid down on child sized cots covered with sheets. The operator read the children a book about animals and turned-on lullabies. The outdoor environment was observed to have stationary climbers, swings, riding toys, and balls for the children to engage in gross motor activities. The following violation(s) were documented. You must conduct either a shelter-in-place or a lockdown drill every three months. An emergency drill was not documented for the fourth quarter of 2023. The last documented fire drill was October 3, 2023. You stated that you complete drills each month but have a hard time remembering to document the drills. You have the fire drill/emergency drill log on a clipboard that is hanging by the back yard. We discussed bringing the clipboard with you during the drill will ensure that you document each drill before hanging it back on the hook. We also discussed what would be helpful for you to make a monthly routine to ensure that monthly tasks are completed. You stated that at the beginning of each month, you will set a day to complete monthly tasks such as fire drills and playground inspections and have one of your additional caregivers check behind you in the middle of the month to ensure that the tasks were completed. Put a reminder in your phone’s calendar that will alert you to complete monthly tasks. Outdoor Inspections: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. Monthly playground inspections had not been documented since June 4, 2023. You stated that after your home was remodeled and reopened in August you had forgotten to document playground inspections. You have the playground inspection forms on a clipboard hanging by the door that are easily accessible to take with you when you complete the inspection. As notated above, we discussed that putting a system into place with setting a date at the beginning of each month, along with a reminder on your phone, will ensure that monthly tasks are completed. Your second line of defense will be that your additional caregiver will check halfway through the month that documentation of drills and inspections have been completed. Additional Comments: CPR/First Aid: The operator’s and two additional caregivers’ certification of CPR/First Aid expires 3/2024. It would be helpful to enroll in a class as soon as possible so that there is not a lapse in certification. Criminal Background Check: The operator’s qualifying letter expires 2/28/24. As this is a legislatively mandated effort, facilities are required to meet rule requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816). Clean Classrooms for Carolina KidsTM is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources.
