L. Gatling, owner and U. Murphy, Administrator, assisted me with the visit. Your program currently operates with a four-star license, issued 1/9/2020, earning 4 points in the education component, 5 points in the program standards component and 1 quality point for having a staff benefits package and an infrastructure of parental involvement. The last annual compliance visit was conducted on 4/9/25. The sanitation inspection was completed 5/29/25 with a “Superior” classification. The last fire inspection was conducted on 2/26/26 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety nine percent as of 3/3/26. The NC Secretary of State website was reviewed on 3/3/26 and LSG Enterprises, LLC was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. All indoor and outdoor spaces were monitored. Children throughout the center were engaged in free play in activity centers, personal care routines, and transitions. In space #1, a staff member sat on the carpet and read The Very Busy Bugs book to children. Infants in space #2 were being cared for according to individual needs including being fed and napping. The outdoor space was observed to have portable climbers, riding toys, and balls for the children to engage in gross motor activities. Lunch was observed and consisted of breaded chicken sandwiches, green beans, peaches, and milk. The following violations were documented. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. In space #1, a feeding plan was not posted for a child under (fifteen) 15 months of age. 10A NCAC 09 .0902(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. A child enrolled in space #1 had an Albuterol inhaler on file without written permission from the parent. 10A NCAC 09 .0803(1)(a & b) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff member hired on 6/2/25, had a qualification letter that expired on October 15, 2025, and the five-year renewal qualification letter was issued on October 21, 2025. A staff member hired on 4/10/25, had a qualification letter that expired on August 10, 2025, and the five-year renewal qualification letter was issued on August 19, 2025. G.S. 110-90.2(b) & .2703(n)&(o) 1887 Each infant was not served only bottles labeled with their individual name. In space #2, four (4) bottles were not labeled with the child’s name. .0902(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 3/25/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Background screenings are needed to provide a safe program environment, support children’s health and safety, and are important for preventing child maltreatment in early care and education programs. A CBC must be completed prior to employee hire and every five years thereafter. An expiration date is now listed in each Qualification Letter. Submissions can also be made six months before the expiration date listed on the current Qualification Letter. U. Murphy, qualification letter expired on October 15, 2025, and the five-year renewal qualification letter was issued on October 21, 2025. A. Murphy, qualification letter expired on August 10, 2025, and the five-year renewal qualification letter was issued on August 19, 2025. You stated that you didn’t start the application process in time for the letter to come back before it expired. We discussed adding expiration dates of criminal background checks in the digital calendar that will send reminders months before they expire. Regularly reviewing your files and staff and training worksheet will help keep you informed about upcoming expiring dates. Medication: Administering medication requires skill, knowledge, and careful attention to detail. All medications for children present at the center must have a current, completed medication permission form even if there is a Medical Action Plan. The medication permission forms provide additional information that could be critical to the administration of the medication and the care of the child. A child enrolled in space #1 had an Albuterol inhaler on file without written permission from the parent. You stated that the parent had the medical action form completed by the doctor and were unsure about the medication permission form. We reviewed the children’s file checklist and where to document when the medical action form and medication authorization are on file. During the visit you printed a medication permission form and stated that you will have the parents complete it this afternoon. Check children’s files regularly to ensure that documents are received on time and documented on the file checklist. Infant Needs: Growth and development during infancy require that nourishing, wholesome, and developmentally appropriate food be provided, using safe approaches to feeding. All children under 15 months of age must have a signed feeding schedule/plan posted for reference by the caregivers. In space #1, a feeding plan was not posted for a child under (fifteen) 15 months of age. You immediately posted the feeding plan to correct the violation. We discussed that when children transition to the toddler room from the infant room that the plan must be posted until the child is over 15 months. Each infant shall be served only bottles labeled with their individual name. In space #2, four (4) bottles were not labeled with the child’s name. You immediately added the child’s name to the bottle to correct the violation. We discussed checking bottles each morning when the parents bring them in to ensure that they are labeled and to use tape to add the name if they are not labeled. You, L. Gatling, Administrator, assisted me with the visit. This facility was issued a temporary license on April 10, 2025. During this visit, we walked through the entire facility observing the indoor and outdoor environment, but a full assessment was not completed for each space. The facility was assessed for supervision, staff/child ratio, and health and safety requirements. Children were observed engaged in free play activities, personal care routines, and group time. Program records, including fire drills and monthly outdoor area inspections were complete and current. New staff records were reviewed to ensure compliance. One electrical outlet was observed uncovered in the hallway leading to the child care spaces. You indicated the outlet was uncovered because it was in use the previous day and you unintentionally forgot to check it. Tamper-resistant electrical outlets or securely attached safety covers prevent children from placing fingers or sticking objects into exposed electrical outlets and reduce the risk of electrical shock, electrical burns, and potential fires. We reviewed the Facility Summary Report for your ITERS-3 assessment. Items scoring less than three could constitute a violation of child care requirements and therefore were monitored and noted below. You, L. Gatling, Administrator, assisted me with the visit. This facility was issued a temporary license on April 10, 2025. During this visit, we walked through the entire facility observing the indoor and outdoor environment, but a full assessment was not completed for each space. The facility was assessed for supervision, staff/child ratio, and health and safety requirements. Children were observed engaged in free play activities, personal care routines, and group time. Program records, including fire drills and monthly outdoor area inspections were complete and current. New staff records were reviewed to ensure compliance. The NC Secretary of State website was reviewed on September 8, 2025 and LSG Enterprises, LLC was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed and the information is current. It is important to frequently check your email to remain informed about any new changes that have or may occur. The following violation was documented and corrected. Any violation(s) documented may impact the compliance history score. Compliance History - The compliance history for this facility prior to today’s visit is 100%. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. 10A NCAC 09 .0604(c) * Technical assistance with documented violations: Electrical Outlets- Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. One electrical outlet was observed uncovered in the hallway leading to the child care spaces. You indicated the outlet was uncovered because it was in use the previous day and you unintentionally forgot to check it. Tamper-resistant electrical outlets or securely attached safety covers prevent children from placing fingers or sticking objects into exposed electrical outlets and reduce the risk of electrical shock, electrical burns, and potential fires. We reviewed the Facility Summary Report for your ITERS-3 assessment. Items scoring less than three could constitute a violation of child care requirements and therefore were monitored and noted below. These items are noted below: ITERS-3 Assessment – August 5, 2025 – (J. Whitman; A. White; K. Oullette) Total score – 5.03 Item 5: Indicator 3.4 (Score 2pts) Assessor Comments: During outdoor play, children were provided with sippy cups of water. As the children drank their water, they walked around the playground and used the slides and riding toys. The practice of children drinking while not closely supervised prevented credit from being earned. Observations today: I did not observe children walking around drinking or eating. One child was held for a bottle feeding in space 2. The operator has implemented the use of the child sized picnic table for drinking water during outdoor play. Program Standards- 6 points Staff Education- 3 points Quality Point - The child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. Total – 6 + 3 + 1 = 10 = 4 Star License. A new license effective October 11, 2025 will be mailed directly to you and must be posted upon receipt. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated.
