Your program currently operates with a 5-Star license effective February 20, 2022. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97) percent as of January 22, 2025. The NC Secretary of State website was reviewed on January 21, 2025, and ELA KidsNet, Holdings, LLC was listed as current-active. The license was posted, and the restrictions were in compliance. Director, Wendy Simpson, was available for consultation during the visit. There are six (6) approved indoor spaces and three (3) approved outdoor spaces. Seventy-six (76) children, between the ages of zero (0) and twelve (12) years old are enrolled and forty-four (44) were present today. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. A walk-through of the facility was completed today. All indoor and outdoor areas were monitored. Infants assigned to indoor space #1 were receiving care based on their individual needs. One (1) infant was being held by a staff member and the other infant was seated in a bouncy seat. Safe sleep checks were monitored, and requirements were met. Younger preschool children assigned to indoor spaces #2, #3, and #4 were observed during nap time. The children were on individual cots with individual linens. Mat spacing, music, and room lighting, were monitored and found to be in compliance. Indoor space #5 was closed due to the scheduled NC Pre-K teacher workday. School aged children assigned to indoor space #6 were engaged in a variety of free play activities which included tracing images, magnetic shapes, bristle blocks, dot markers, brickyard construction engineering, handwriting, and legos. Indoor spaces #1 and #6 did not have current activity plans posted. The outdoor spaces had a variety of gross motor equipment including tricycles, basketball goals, push toys, riding toys, small, covered sand boxes, large abacus, large chalkboard, bike trails, and dramatic play toys. Health and Safety trainings, the Shaken Baby and Abusive Head Trauma Policy, and new staff files were monitored today and found to be in compliance. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in indoor space #6. The activity plan posted was dated January 13, 2025, through January 17, 2025. GS 110-91(12); .0508(a) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. A thermometer was not in the refrigerator in the kitchen; therefore, a temperature of 45 degrees F. or below could not be verified. 15A NCAC 18A .2806(j)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation documented may impact the compliance history score. The violations documented must be corrected immediately. The program follows the Craven County school calendar. The length of the day, 8:00am until 2:45pm, meets the instructional day requirement of 6.5-10 hours. Child Assessment and Required Screenings: The assessment tool, GOLD, Teaching Strategies 2010-2011, is in place. Portfolios for each child are maintained in both indoor spaces and electronically on Teacher Strategies Gold online application. The facility uses the developmental screening tool, DIAL-4 for pre-screening and ongoing assessments. Documentation of health assessments, vision, hearing, dental screenings, and DIAL-4 pre-screenings were on file for each child. Curriculum: The Creative Curriculum for Preschool, 6th Edition, Teaching Strategies, 2016 has been implemented and the classroom has the required resources available for use. Materials to support the current activity plan were displayed throughout the classroom. Family Engagement: Parent involvement included open house, monthly newsletters, Thanksgiving friends giving, parent teacher conferences, and classroom volunteer readers. Staff Requirements: All staff met NC Pre-K requirements. Technical Assistance: Criminal Background Check Information - DCDEE launched the Provider Portal for ABCMS on February 1, 2024. This access will allow providers to see applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster, see the real-time background check status of staff/household members, and run a printable report of the staff roster to assist with compliance visits. To gain access to the Provider Portal complete the ABCMS Child Care Provider Portal Training on DCDEE's Moodle. The training consists of watching a short video then taking a test about the video. A certificate will be issued upon successful completion of the test. Email the certificate along with your full name, position, and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. Access will then be granted within 2 business days. If you have general questions, please continue to use DHHS.CBC.Unit@dhhs.nc.gov. This program currently operates with a Five Star License, issued February 20, 2022, earning five (5) points in Staff Education, seven (7) points in Program Standards, and 1 quality point for choosing a Programmatic Option. The last annual compliance visit was conducted July 17, 2024. Prior to this visit, this facility’s eighteen-month compliance history was 99%. According to the Secretary of State’s website, ELA KidsNet Holdings, LLC corporation’s status is current and active. I visited all spaces used with the children unaccompanied. One (1) infant was present and receiving routine care based on his individual needs. Toddlers were observed transitioning from naptime to afternoon snack. Afternoon snack consisted of mangoes, milk, water, and sugar cookies. Today, all preschool children were decorating Christmas cookies during afternoon snack. Younger and older preschool children were on individual cots with individual linens. Food service, room lighting, and cot spacing were monitored, and requirements were met. A variety of gross motor equipment was available for the children. Caregivers and children interacted in a positive and appropriate manner. Staff/child ratios and supervision were monitored and found to be in compliance today. Allegation: There are concern that a child was left unsupervised on the bus. Selected staff members were interviewed separately and privately. Staff members reported that children were adequately supervised in both indoor and outdoor spaces. The allegation of this report was discussed with Wendy Simpson, Director. You were given the opportunity to state your perceptions of the allegation and to share any pertinent information. You reported on Thursday, December 12, 2024, a staff member entered her classroom from the parking lot with her after school children that she transported to your facility via bus. You greeted the staff member in her classroom. You asked both staff members that care for the school aged children, how many children did they have. They reported that there were fifteen (15) children present according to the Transportation Roster. You clocked the children in on the Line Leader online application. You went back to double check the number of children present, as you do daily. When the staff members counted, they only counted fourteen (14) children present. You verified by name face recognition the children that you clocked in the Line Leader online application with the children present in the classroom, and one (1) child was missing. Both staff members checked the classrooms and bathrooms for the child. Another older child reported that the missing child was on the bus, because she sat with her. You asked the older child if she saw the missing child get off the bus, and she said no. You asked the child’s teacher to go check to see if the child was still on the bus. The staff member found the child asleep on the bus and carried her into the classroom. You asked the child if she was ok and checked her out. The child reported that she was just asleep, and she was fine. You reported the incident to the child’s parent. The staff member that left the child on the bus was terminated from your facility on Monday, December 16, 2024. This incident was self-reported by Wendy Simpson, Director. Based on observations, information received, and my investigation, this allegation is substantiated. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On December 12, 2024, one (1) child, aged five (5) years old, was left on the bus in the facility’s parking lot unsupervised for approximately three (3) to four (4) minutes. .1801(a)(1-5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation documented may impact the compliance history score. The violation documented must be corrected immediately. On or before Friday, December 27, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violation was corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: Supervision: Supervision is basic to the safety and the prevention of injury and maintaining quality childcare. Parents have a contract with the facility and its staff to supervise their children. The importance of supervision is not only to protect children from physical injury, but from harm that can occur from teasing, bullying, or inappropriate behavior. It is the responsibility of staff to regularly count children (name to face recognition) on a routine basis, at every transition, and whenever leaving one area and arriving at another. Active and positive supervision involves knowing each child’s abilities, establishing clear and simple safety rules, being aware of potential hazards, standing in strategic positions, and scanning play areas and circulating around the space. Focus on the positive rather than the negative to teach a child what is safe. Teach children the appropriate and safe use of each piece of equipment. It is imperative that caregivers supervise children at all times, not just for the safety of children, but to ensure children get the attention they need for optimal growth and development. Children like to test their skills and abilities. Serious injuries can occur in indoor and outdoor play areas if constant supervision is not maintained. Caregivers who are actively involved, aware, and appreciative of young children’s behaviors are in the best position to keep children safe. Low staff-child ratios make it easier for caregivers to supervise children adequately while interacting. Adequate supervision means that staff interact with the children while moving about the indoor and outdoor area and can hear and/or see the children at all times, except when emergencies necessitate that direct supervision is impossible for brief periods of time. A PLAN OF CORRECTIVE MEASURES: This facility made a self-report. The facility’s Transportation and Safety Policy was reviewed with all staff that transport children on Friday, December 13, 2024. Your program currently operates with a 5-Star license issued February 20, 2022. The last annual compliance was conducted on August 3, 2022. The sanitation inspection was completed December 16, 2022, with a “Superior” classification. The last fire inspection was conducted September 21, 2022. Your facility was approved for 1st shift care, enhanced ratios, enhanced space, children under 2 ½ years old cared for in rooms with direct exits only, and the facility meets reduced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three (93%). The NC Secretary of State website was reviewed on July 20, 2023, and ELA KidsNet Holdings, LLC was listed as current-active. Fifty-four (54) children between the ages of zero (0) and eleven (11) are enrolled and thirty-eight (38) were present today. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in all classrooms. Age-appropriate materials and activities were provided for all children. All indoor and outdoor spaces were monitored. In indoor space one (1), two (2) staff members were seated on the carpet playing with two (2) infants. Safe sleep checks, infant feeding plans, and nutrition requirements were monitored and found to be in compliance. Infants received routine care based on their individual needs. Toddlers assigned t indoor space two (2), were seated on the carpet as a staff member read a book. Young preschool children assigned to indoor space three (3) were putting puzzles together at a table. Older preschool children assigned to indoor space four (4) were participating in indoor large gross motor activities including jumping on the carpet and a Pinocchio Musical Body Movements Game. School age children were observed in the outdoor space playing kickball. Water was offered to the children while outside. All staff members were actively engaged with the children. All outdoor spaces had a variety of gross motor equipment including tricycles, basketball goals, push toys, riding toys, small, covered sand boxes, large, covered sandboxes, bike trails, and dramatic play toys.
