You, Karen McMillan, Director, assisted me with today’s visit. The last temporary time period visit was conducted on June 18, 2025. The last sanitation inspection was completed on August 7, 2025. The last fire inspection was conducted August 15, 2025, and your facility was approved for daytime care only. The program’s compliance history was 96% prior to today’s visit. The center's notice of compliance, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaging in free choice activities, routine care taking, meal time and rest time. Fire drills are being completed monthly as required. Emergency drills are being conducted every three months as required. Incident logs were monitored during today’s visit. Incident reports are being filed in children’s individual files. Playground inspections are being conducted monthly. Your playground meets child care safety standards. Materials, toys, and equipment throughout the facility were developmentally appropriate and in good repair. No emergency medications and permission to administer forms are currently on site. Two staff files and two children’s files were monitored during today’s visit. This program does not currently offer transportation. The following violations were cited today: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member hired on 1/6/25 did not have verification of completion of First Aid training from an approved training organization on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member hired on 1/6/25 did not have verification of staff completion of the CPR course from an approved training organization on file. .1102(d) 1827 The Center did not have a person on staff who completed the EPR in Child Care training within the required timeframe and/or documentation of completion of the training was not on file or in a file designated for emergency preparedness and response plan documents. The Center did not have documentation of a person on staff who completed the EPR in Child Care training within the required timeframe. .0607(b) Technical Assistance: Please ensure First Aid and CPR certifications are completed every two years, and certifications are obtained prior to previous certifications expiring. Please complete an EPR plan as soon as possible. Today the plan for this facility was not on file and available for review. Consultation was provided on the following: Please continue ensuring all staff continue practicing active supervision during nap time. Please continue ensuring that all substitutions and corrections to menu items is posted prior to meals being served to children, preferably in the morning so parents can see the updated menu items during drop off. Please continue ensuring all documentation, including but not limited to applications for employment and staff orientation documents are completed in their entirety and are on file and available for review at all times. We discussed the QRIS process today and reviewed each pathway. I encouraged you to continue reviewing the QRIS Modernization information and documents on the DCDEE website. Also, please continue reviewing section .3200 in child care rules, which contains all the new rules related to QRIS. You stated at this current time you are interested in pursuing pathway #3 in the future. The violation(s) documented must be corrected immediately. Upon arrival, I was greeted by you, Karen McMillan, Owner and Director, and I stated the reason for my visit. This facility was issued a temporary license on January 6, 2025, with a restriction of first and second shift care and children under 2 ½ years old are in rooms with direct exits. The following were posted: Temporary License, Classroom Staff to Child Ratio charts, Safe Arrival and Departure Procedures, NC Summary of Child Care Law poster (this has been revised), Emergency Medical Care Plan and Evacuation Plans. The children were observed during a coloring activity at the table, music and movement during group time, arrival procedures, hand washing routines, free choice activities outdoors, departure procedures, lunch and preparing for nap time. Supervision, staff/child ratio, grouping of children, the use of approved space, and permit restrictions were monitored during today’s visit. The last monthly fire drill was conducted on March 31, 2025. The last playground inspection was conducted on March 18, 2025. A fire inspection was conducted on July 12, 2024. A sanitation inspection was conducted on February 3, 2025, with no demerits. I monitored a portion of the children’s files. There was one new staff person/substitute from “PURE Excellence” that began yesterday, April 2, 2025. I monitored her file, and she did not have a medical statement. I also did not see documentation that she has received and you stated that you have not reviewed the Shaken Baby Head Trauma Policy. You did not have the Staff and Training Worksheet completed for me to review. You will want to include substitutes, volunteers, uncompensated providers. Please complete the worksheet and submit it to me no later than the end of the business day April 7, 2025. Rated License Discussion – I gave you the “Initial Application for Assessment for a Two-Through Five Star Rated License” and the “Rated License Assessment Request Review” form to complete during the visit conducted on February 11, 2025. We discussed that you would complete the Review form if you would like to have the Environmental Rating Scale 3 (ERS3) assessment completed. I received the completed documents today. -Education Standard points: You and staff need to have current education evaluated by the workforce unit in Raleigh. The education points will be determined upon the receipt of evaluation results from the workforce unit. If staffs' education is not evaluated by the fourth month of the temporary license, the education points will be calculated with the current education showing in WORKS. We discussed that a lead teacher will need to be at least: *18 years of age, have a HS/GED diploma and have earned the NCECC OR *18 years of age, have a HS/GED diploma and one year experience in a licensed early childhood program or two years of experience in a licensed FCCH. OR *Two years working in a Head Start Program. To be eligible to qualify for minimum lead teacher education standard points. They will also need to have a WORKS account and a status letter in their file. We discussed that a teacher will need to be at least 18 years of age and have one year experience in a licensed early childhood program to be eligible to qualify for minimum teacher education standard points. They will also need to have a WORKS account and a status letter in their file. I monitored WORKS during today’s visit, and you will need to upload your BSAC certificate into your WORKS account. -Program Standard Points: A request will be submitted to have the ERS-3 assessments conducted. -Quality point: You stated that the quality point options are still being reviewed, and you have not decided which option this program is eligible for. Star Rated License Child Care programs are expected to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent at all times [NC GS 110-90(4)(d)]. During today’s visit I gave you a Foundations Book to use as a resource. The following violations were observed today. Violation Number Comment Rule 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. There was no record now was it reviewed by with the provider prior to working with the children. .0608(d)(1-4) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. The substitute provider's file had her TB results in her personnel file and was not located in a separate file. .0701(d) TECHNICAL ASSISTANCE: -Use the Substitute Staff File Checklist to ensure that you have all the required documents before a substitute staff member begins working with the children. The Checklist can be found on the DCDEE website under “Provider Documents”. I gave you a copy during today’s visit. -We discussed that all staffs’ health information needs to be in a sperate file and not placed the general personnel file. CONSULTATION: -We discussed that even when the kitchen is locked the sanitizer and disinfectant sprays are required to be at least five feet from the floor or in locked storage. -The NC Summary of Child Care Law brochure and poster were both updated in February 2025 and are available on the DCDEE website at https://ncchildcare.ncdhhs.gov/Provider/Provider-Documents-and-Forms. Please begin using the February 2025 version of the NC Summary of Child Care Law brochure in your enrollment packet. Please be reminded that any ERS assessments will be assessed using the ERS-3 versions. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 16, 2025, We must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. While the Division allows you time to explain how you have corrected violations when violations are cited, it is expected you correct all violations immediately. When you are asked to send a compliance letter to me, it is expected and required that your compliance letter be sent to me no later than the date indicated in this documentation. Upon arrival, I was greeted by you, Karen McMillan, Owner and Director, and I stated the reason for my visit. This facility was issued a temporary license on January 6, 2025, with a restriction of first and second shift care and children under 2 ½ years old are in rooms with direct exits. The children were observed during free choice activities indoors and outdoors, hand-washing routines, “Going on a Bear Hunt” activity, table toy manipulatives while waiting for lunch, eating lunch and naptime . Positive interactions between the staff and children took place throughout the visit today. A portion of children files were monitored. We discussed that you would need to complete the Staff and Training Worksheet for all staff; including yourself, and have it current and available for review by DCDEE staff. I gave you a copy of the worksheet today. It can also be found under “Provider Documents” on the DCDEE website. You are the only staff member at this time. A fire inspection was conducted on July 12, 2024 . A sanitation inspection was conducted on February 3, 2025, with no demerits. You stated that this facility does not provide transportation. The outdoor play area was monitored. We discussed the rated license process. You were reminded that a compliance history score of seventy five percent or above must be maintained to receive a rated license. I gave you the “Initial Application for Assessment for a Two-Through Five Star Rated License” and the “Rated License Assessment Request Review” form to complete. We discussed that you will complete the Review form if you would like to have the Environmental Rating Scale 3 (ERS3) assessment completed. Please submit the paperwork to me no later than March 11, 2025. -Education Standard points: The administrator, lead teachers and teachers must have their current education evaluated by the workforce unit in Raleigh. The education points will be determined upon the receipt of evaluation results from the workforce unit. If staff’s education is not evaluated by the fourth month of the temporary license, the education points will be calculated with the current education showing in WORKS. Quality point: The quality point options were reviewed, and a copy of options was given to you. You can inform me during the 2nd TTP what quality point option you would like to be approved for, to earn one extra point towards your star rated license. The following violations were observed: Violation Number Comment Rule 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The railing that leads out to the outdoor play area had areas in the corner by the door and at the end of the ramp that had spaces greater than 3 1/2 inches between railing spindles. .0605(g) 807 A safe indoor and outdoor environment was not provided for the children. The disinfectant and sanitizer spray was located on top of a shelf that housed children activity materials and was not at least five feet from the floor. Sanitizing wipes and all purpose spray were located on the kitchen counter between the classroom and kitchen and were not stored in locked storage. 10A NCAC 09 .0601(a) 1314 Emergency information did not name child's health care professional. A child's file monitored did not have the this information listed on their application. .0802(c)(2) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. There was no documentation in your file that the above policy was reviewed, signed and dated. .0608(d)(1-4) TECHNICAL ASSITANCE: -Review all paperwork that you receive from the families to ensure that all spaces/blanks have something written in them. If it does not pertain to their children they can write “NA”. It is important that all dates and signatures are listed. -You will need to review the application to make sure parents have listed the medical professional and contact information. -The Sanitizer and disinfectant spray need to be at least five feet from the floor. You stated that you have a shelf that you will install at least five feet from the floor to store the sprays. All other cleaning supplies need to be kept in locked storage. -All staff, including yourself, will need to review, sign and date acknowledging that you have received the Shaken Baby Prevention Policy. Make sure to place it in your file. -There should no spaces greater than 3 ½ inches and less than 9 inches on equipment and railings. This is considered an entrapment hazard. CONSULTATION: -Any documents that pertain to staffs’ health information (health questionnaire, TB results, medical report, doctor notes, etc.) need to be placed in separate individual files. -Please visit The Center on the Social and Emotional Foundations for Early Learning at https://csefel.vanderbilt.edu/. The Center develops training and technical assistance (T/TA) and materials that reflect evidence-based practices for promoting children's social and emotional development and preventing challenging behaviors. -Another resource to visit is https://www.childcarerrnc.org/special-projects/healthy-social-behaviors/ -We discussed and reviewed the Volunteer File Checklist so you can ensure that any volunteer at the facility has the required information in their file. -As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. -Health and safety trainings need to be taken over if you are hiring staff that have health and safety trainings that are older than 12 months. -For continued eligibility in the subsidized child care program, you must achieve a three star or higher rated license at the end of the temporary licensed period. -A lead teacher must already have earned the NC Early Childhood Credential (NCECC) per Child Care Rule .0703(c) or meet the Preservice Lead Teacher requirements per Child Care Rule .0710(a) while working on the EDU 119 college course within six months of assuming the position. Once the EDU 119 course is complete, staff need to mail an unopened official transcript to the workforce unit in Raleigh, and request in WORKS for their transcript to be evaluated as a lead teacher. CC RULE REFERENCES: *.0710(b) A lead teacher or a teacher shall be 18 years of age, have a high school diploma or its equivalent, and have one of the following: (1) One year of child care experience working in a child care center or two years of verifiable experience as a licensed family child care home operator; or (2) Completion of a two-year high school program of Early Childhood Education in Family and Consumer Sciences Education; or (3) Twenty hours of training in child development, which shall include the North Carolina Early Childhood Credential coursework, within the first six months of employment in addition to the number of on-going training hours required in Rule .1103 of this Chapter. *.0703(c) Within six months of an individual assuming lead teacher or child care administrator duties, each center shall maintain the following information in the individual's staff record: (1) a copy of the credential certificate; (2) a copy of notification from the Division that the individual meets the equivalency or that the individual does not meet the equivalency and must enroll in coursework; (3) a dated copy of the request submitted by the individual to the Division for the assessment of equivalency status; or (4) documentation of enrollment in credential coursework. -All staff must have their current education evaluated by the WORKS Unit in Raleigh. Upon arrival, I was greeted by you, Karen McMillan, Owner and Director, and I stated the reason for my visit. This facility was issued a temporary license on January 6, 2025, with a restriction of first and second shift care and children under 2 ½ years old are in rooms with direct exits. The children were observed during free choice activities indoors and outdoors, hand-washing routines, “Going on a Bear Hunt” activity, table toy manipulatives while waiting for lunch, eating lunch and naptime . Positive interactions between the staff and children took place throughout the visit today. A portion of children files were monitored. We discussed that you would need to complete the Staff and Training Worksheet for all staff; including yourself, and have it current and available for review by DCDEE staff. I gave you a copy of the worksheet today. It can also be found under “Provider Documents” on the DCDEE website. You are the only staff member at this time. A fire inspection was conducted on July 12, 2024 . A sanitation inspection was conducted on February 3, 2025, with no demerits. You stated that this facility does not provide transportation. The outdoor play area was monitored. We discussed the rated license process. You were reminded that a compliance history score of seventy five percent or above must be maintained to receive a rated license. I gave you the “Initial Application for Assessment for a Two-Through Five Star Rated License” and the “Rated License Assessment Request Review” form to complete. We discussed that you will complete the Review form if you would like to have the Environmental Rating Scale 3 (ERS3) assessment completed. Please submit the paperwork to me no later than March 11, 2025. -Education Standard points: The administrator, lead teachers and teachers must have their current education evaluated by the workforce unit in Raleigh. The education points will be determined upon the receipt of evaluation results from the workforce unit. If staff’s education is not evaluated by the fourth month of the temporary license, the education points will be calculated with the current education showing in WORKS. Quality point: The quality point options were reviewed, and a copy of options was given to you. You can inform me during the 2nd TTP what quality point option you would like to be approved for, to earn one extra point towards your star rated license. The following violations were observed: Violation Number Comment Rule 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The railing that leads out to the outdoor play area had areas in the corner by the door and at the end of the ramp that had spaces greater than 3 1/2 inches between railing spindles. .0605(g) 807 A safe indoor and outdoor environment was not provided for the children. The disinfectant and sanitizer spray was located on top of a shelf that housed children activity materials and was not at least five feet from the floor. Sanitizing wipes and all purpose spray were located on the kitchen counter between the classroom and kitchen and were not stored in locked storage. 10A NCAC 09 .0601(a) 1314 Emergency information did not name child's health care professional. A child's file monitored did not have the this information listed on their application. .0802(c)(2) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. There was no documentation in your file that the above policy was reviewed, signed and dated. .0608(d)(1-4) TECHNICAL ASSITANCE: -Review all paperwork that you receive from the families to ensure that all spaces/blanks have something written in them. If it does not pertain to their children they can write “NA”. It is important that all dates and signatures are listed. -You will need to review the application to make sure parents have listed the medical professional and contact information. -The Sanitizer and disinfectant spray need to be at least five feet from the floor. You stated that you have a shelf that you will install at least five feet from the floor to store the sprays. All other cleaning supplies need to be kept in locked storage. -All staff, including yourself, will need to review, sign and date acknowledging that you have received the Shaken Baby Prevention Policy. Make sure to place it in your file. -There should no spaces greater than 3 ½ inches and less than 9 inches on equipment and railings. This is considered an entrapment hazard. CONSULTATION: -Any documents that pertain to staffs’ health information (health questionnaire, TB results, medical report, doctor notes, etc.) need to be placed in separate individual files. -Please visit The Center on the Social and Emotional Foundations for Early Learning at https://csefel.vanderbilt.edu/. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. -Health and safety trainings need to be taken over if you are hiring staff that have health and safety trainings that are older than 12 months. -For continued eligibility in the subsidized child care program, you must achieve a three star or higher rated license at the end of the temporary licensed period. -A lead teacher must already have earned the NC Early Childhood Credential (NCECC) per Child Care Rule .0703(c) or meet the Preservice Lead Teacher requirements per Child Care Rule .0710(a) while working on the EDU 119 college course within six months of assuming the position. Once the EDU 119 course is complete, staff need to mail an unopened official transcript to the workforce unit in Raleigh, and request in WORKS for their transcript to be evaluated as a lead teacher. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. While the Division allows you time to explain how you have corrected violations when violations are cited, it is expected you correct all violations immediately.
