You, Malena Belcher, Teacher Assistant, assisted me with today’s visit. Mandy Stikeleather, Administrator, gave permission for Mrs. Belcher to sign the visit summary at the conclusion of today's visit. The last annual compliance visit was conducted on January 12, 2026. The last sanitation inspection was completed on October 24, 2025, with a “Superior” classification. The last fire inspection was completed on October 31, 2025, and your facility was approved for daytime care only. The program’s compliance history was ninety-nine percent prior to today’s visit. The center's five-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the environment was completed. Children were observed engaged in free choice activities, toileting and handwashing routines, lunch, and outdoor play. Fire drills were conducted monthly as required. Emergency drills are being conducted quarterly as required. Please conduct an emergency drill in the Month of May prior to school being out for the summer and again in August when school returns. This will ensure that compliance is maintained with emergency drills being conducted quarterly. Your outdoor play area meets child care safety standards. The last outdoor playground inspection was completed on March 31, 2026. No concerns were observed. Materials, toys, and equipment throughout the facility were of sufficient quantity, developmentally appropriate, and in good repair. You stated that no children currently enrolled have medications or Medical Action Plans. All staff files were monitored for a current Criminal Background qualification letter, current First Aid and CPR training, and special training during today’s visit. Kelsey Greer, Substitute, was present today and stated that she has been present in the classroom at least three times. A file was not available for review. The ABCMS criminal background portal was monitored during today’s visit. Ms. Greer has a current qualification letter and is qualified for employment through May 12, 2028. The facility does not provide transportation. The following violations were documented today: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. A substitute present during today's visit, did not have a file available for review. G.S. 110-91( 9) Technical Assistance was provided on the following: • We discussed the requirements for all substitutes that will be working in the NC Pre-K classroom. Administrative staff should ensure that all substitute staff have the required documents on file for review. It is recommended that substitutes that will be utilized create a personal file to carry to each location so that they are in compliance with child care rules. • Please reference the staff and training worksheet that was provided to administrative staff during a meeting held on February 5, 2026, to reference what documentation is required for substitute staff. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. You, Shelley Fago, Lead Teacher, assisted me with today’s visit. The last sanitation inspection was completed on September 19, 2024, with a “Superior” classification. The last fire inspection was completed on October 4, 2024, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 100 percent prior to today’s visit. The center's five-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed during teacher directed art activities, free choice activities, lunch, indoor gross motor, and rest time. Fire drills were completed monthly as required. The last fire drill was completed on January 17, 2025. Emergency drills are being completed every three months as required. The last emergency drill was completed on December 2, 2024. Your playground meets child care safety standards. The last playground inspection was completed on January 14, 2025. No concerns were observed. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. You stated that no children currently enrolled have medications or medical action plans. A selection of staff files and children’s files were monitored during today’s visit. Health and Safety standards were monitored during today's visit. The program does not provide transportation. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC PreK staff are lead teacher, Shelley Fago and teacher, Malena Belcher. A selection of files was monitored for completed health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 7:15am to 1:45pm. Parent conferences are held three times per year. You stated that you use the remind app, emails, and face to face to communicate with parents. In addition, you use communication folders, weekly newsletters, and complete parent engagement activities. The environment rating scale score received on May 11, 2023 was 6.27. The following violation was documented during today’s visit. Violation Number Comment Rule 1898 Staff did not complete the health and safety training within one year of employment. A staff member hired on 8/7/23 completed four health and safety trainings on 10/31/24. .1102(a) Technical Assistance was provided on the following: 1. We discussed ensuring that staff complete required trainings within the specified timeframe. Health and Safety Trainings should be completed within one year of hire and every 5 years thereafter. All Health and Safety Trainings have been completed and are available for review. This violation was corrected during today’s visit. Consultation was provided on the following: 1. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The last emergency drill was completed on December 2, 2024. Your playground meets child care safety standards. The last playground inspection was completed on January 14, 2025. No concerns were observed. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. You stated that no children currently enrolled have medications or medical action plans. A selection of staff files and children’s files were monitored during today’s visit. Health and Safety standards were monitored during today's visit. The program does not provide transportation. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC PreK staff are lead teacher, Shelley Fago and teacher, Malena Belcher. A selection of files was monitored for completed health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 7:15am to 1:45pm. Parent conferences are held three times per year. You stated that you use the remind app, emails, and face to face to communicate with parents. In addition, you use communication folders, weekly newsletters, and complete parent engagement activities. The environment rating scale score received on May 11, 2023 was 6.27. The following violation was documented during today’s visit. Violation Number Comment Rule 1898 Staff did not complete the health and safety training within one year of employment. A staff member hired on 8/7/23 completed four health and safety trainings on 10/31/24. .1102(a) Technical Assistance was provided on the following: 1. We discussed ensuring that staff complete required trainings within the specified timeframe. Health and Safety Trainings should be completed within one year of hire and every 5 years thereafter. All Health and Safety Trainings have been completed and are available for review. This violation was corrected during today’s visit. Consultation was provided on the following: 1. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. We discussed that staff members annual on-going training hours are calculated from each staff members individual hire date. 3. We discussed when completing the staff and training worksheet, it is recommended for lines 22,23, and 27 to keep the original review dates and add the annual date that these items are reviewed/renewed with staff. 4. https://www.positivechildhoodalliancenc.org/online-trainings/ is the new website to complete the Recognizing and Responding to Suspicions of Child Maltreatment training. Please print a copy of the training certificate and place it in each staff members file. 5. Please ensure that staff and training worksheets are completed for each visit. Consultants are not permitted to complete or make changes to the worksheet. Please ensure that the new administrator has a copy of their current information on file at the facility and is added to the staff and training worksheet. 6. Please ensure that all staff have a WORKS account and that they send in official transcripts to the Workforce Unit for evaluation. Once staff have received a status letter they should place a copy of their letter in their staff file. 7. As of July 8, 2024, the Senate Bill 425/Session Law 2024-34 for Hold harmless has been extended until the new QRIS is implemented. At this time providers are not required to go any further with a rated license assessment unless they want to.
