This visit was conducted by Jennifer Davis , child care consultant with Roberta Mickel and Maria Hernandez, lead teachers. Currently this program operates with a FOUR (4) Star License issued 3/22/19. License restrictions include daytime care only; meets enhanced ratios, meets enhanced space; enhanced ratios minus 1. This program is operated by Yadkin County Schools. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. There are two licensed classroom spaces this school year. I observed children participating in center free play, art activities and routine care. I observed many developmentally appropriate materials and activities were available to the children in the classrooms. The last annual compliance visit was conducted on 1/9/25. A sanitation inspection was completed 4/30/2025 with a superior classification. The last fire inspection was completed 9/2/2025. Program records and required postings were monitored. A fire drill was documented on 11/21/2025. An emergency drill has not been conducted this school year. An outdoor inspection was documented on 12/5/2025. Staff files and children’s records were monitored per DCDEE procedures. DPI off-site verification forms were observed. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. I observed one emergency medication. NCPK Requirements Currently at this school, there are two NCPK classrooms with enrollment. The program's instructional day is 7:15 a.m. to 2:15 p.m. Attendance is taken daily and if a child is absent for three consecutive days either the teachers or school administration will contact the family. Examples of family engagement include a newsletter, take home folders, DOJO app, parent conferences when requested, parent volunteer opportunities and take home projects. Facility Requirements: This program currently holds a 4-star license issued 3/22/19. An ECERS-R assessment was conducted on 3/5/19 earning a score of 5.19. Child Health Assessment: Records of the children enrolled in the NCPK classroom were reviewed today. Records monitored were observed to contain health assessments which included physical examinations, updated immunizations, and vision, hearing and dental screenings. The teachers stated that regarding the on-site screenings held at the school, the screening agent communicated all results with the parents, as required by Rule .3005(b). The results were shared via verbal/written communication to the parents. Developmental Screening: Records of the children enrolled in the NCPK classroom were reviewed today. Children’s files reviewed contained developmental screenings (Dial 4 Assessment), administered within 90 days of the first day of attendance in the program or 6 months prior. The teachers stated that all developmental screening results were shared with families by the child’s teacher as required (Rule .3006(b), via verbal/written communication with parents. Curricula: The teachers stated Creative Curriculum and NC Foundations are used at this program. During the observation, children were engaged in free play and in group activities in each classroom. Throughout each classroom, items in the activity areas were consistent with the lesson plan. Formative Assessments: Children's files monitored today contained formative assessments regarding each child's growth and skill development. The teachers stated that the formative assessment used by the program is Teaching Strategies Gold. I observed assessments on file. They will be conducting winter assessments in February. A spring assessment will be conducted prior to the end of the school year. Parent conferences are offered. Staff-to-Child Ratio and Class Size: The class size is 18 children or less and the staff/child ratio is 1:9 or less for both classrooms. A lead and an assistant are assigned to each NCPK classroom. Instructional Staff Standards (Rule .3015): The rule states instructional staff shall work in direct contact with children in the Pre-K program for at least a 32.5-hour work week. In addition to these direct, day-to-day instructional experiences, the Pre-K program must provide adequate additional time for the instructional staff for related instructional activities, including time for planning, scheduling and conducting home visits, meeting with children's families, or attending required professional development activities. These related activities shall take place outside of the six-and-a-half-hour day of direct teacher-child contact. Planning time occurs after the instructional day or during the day when opportunities allow. Parents attended an open house in August prior to the beginning of the school year. Professional Development Requirements (Rule .3016) Professional Development opportunities such as training are logged on an electronic training transcript and/or a training log and kept in each staff’s file. Staff training hours were monitored during today’s visit. This visit was conducted by Jennifer Davis , child care consultant with Roberta Mickel, lead teacher. A. Mondragon and B. Moxley, lead teachers, also assisted with the visit. Currently this program operates with a FOUR (4) Star License issued 3/22/19. License restrictions include daytime care only; meets enhanced ratios, meets enhanced space; enhanced ratios minus 1. This program is operated by Yadkin County Schools. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed children participating in group activities, center activities and having lunch. I observed many developmentally appropriate materials and activities were available to the children in the classrooms. The last annual compliance visit was conducted on 2/3/23. A sanitation inspection was completed May 16, 2023 with a superior classification. The last fire inspection was completed 9/12/2023. Program records and required postings were monitored. A fire drill was documented on 12/19/23. A shelter-in-place drill was documented on 12/1/23. An outdoor inspection was documented on 12/8/23. Staff files and children’s records were monitored per DCDEE procedures. DPI off-site verification forms were observed for staff, children, and transportion records (8/23). Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. No medications were reported nor observed. The program does provide transportation. A DPI off-site verification form for transportation (8/29/22) was observed. NCPK Requirements Currently at this school, there are three NCPK classrooms with enrollment. (NCLB) Ann Bates- AAS ECE Maria Hernandez- AA/AAS ECE The program's instructional day is 7:15 a.m. to 2:15 p.m. Attendance is taken daily and if a child is absent for three consecutive days either the teachers or school administration will contact the family. Examples of family engagement include a newsletter, take home folders, DOJO app, parent conferences when requested, parent volunteer opportunities and take home projects. Facility Requirements: This program currently holds a 4-star license issued 3/22/19. An ECERS-R assessment was conducted on 3/5/19 earning a score of 5.19. Child Health Assessment: Records of the children enrolled in the NCPK classroom were reviewed today. Records monitored were observed to contain health assessments which included physical examinations, updated immunizations, and vision, hearing and dental screenings. The teachers stated that regarding the on-site screenings held at the school, the screening agent communicated all results with the parents, as required by Rule .3005(b). The results were shared via verbal/written communication to the parents. Developmental Screening: Records of the children enrolled in the NCPK classroom were reviewed today. Children’s files reviewed contained developmental screenings (Dial 4 Assessment), administered within 90 days of the first day of attendance in the program or 6 months prior. The teachers stated that all developmental screening results were shared with families by the child’s teacher as required (Rule .3006(b), via verbal/written communication with parents. Curricula: The teachers stated Creative Curriculum and NC Foundations are used at this program. During the observation, children were engaged in free play and in group activities in each classroom. Throughout each classroom, items in the activity areas were consistent with the lesson plan. Formative Assessments: All children's files monitored today contained formative assessments regarding each child's growth and skill development. The teachers stated that the formative assessment used by the program is Teaching Strategies Gold. I observed assessments on file. They are currently conducting winter assessments. A spring assessment will be conducted prior to the end of the school year. Parent conferences are offered. Staff-to-Child Ratio and Class Size: The class size is 18 children or less and the staff/child ratio is 1:9 or less for both classrooms. A lead and an assistant are assigned to each NCPK classroom. Instructional Staff Standards (Rule .3015): The rule states instructional staff shall work in direct contact with children in the Pre-K program for at least a 32.5-hour work week. In addition to these direct, day-to-day instructional experiences, the Pre-K program must provide adequate additional time for the instructional staff for related instructional activities, including time for planning, scheduling and conducting home visits, meeting with children's families, or attending required professional development activities. These related activities shall take place outside of the six-and-a-half-hour day of direct teacher-child contact. Planning time occurs after the instructional day or during the day when opportunities allow. The teachers stated that home visits were not required this school year. Parents attended an open house in August prior to the beginning of the school year. Professional Development Requirements (Rule .3016) Professional Development opportunities such as training are logged on an electronic training transcript and/or a training log and kept in each staff’s file. Staff training hours were monitored during today’s visit. The following violations were cited during today’s visit: Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff did not have a current emergency information form on file. .0701(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One new staff did not complete the training within 90 days of employment. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Compliance Documentation: You must correct the violations cited during today's visit immediately and documentation verifying compliance must be received on or before 1/31/2024.
