This visit was conducted by Jennifer Davis, child care consultant with Kendall Irvin, on-site program coordinator. Currently this program operates with FIVE (5) star license issued 5/10/2024. Restrictions include daytime care only; school age children only; meets enhanced space; meets enhanced ratios; reduced group size by 1 in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. The program’s compliance history was 99% prior to today’s visit. All programs are required to maintain at least 75% compliance The license was observed, and the restrictions were met. This program is operated by Yadkin County Schools. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today between staff and children. I observed children in free play in centers today. Files were monitored for staff and children according to DCDEE procedures. No new staff were reported at this program. Storage and administration of medication were monitored. No medications were reported, nor observed. Storage hazardous items was monitored today. Program records and required postings were monitored. A playground inspection was recorded 2/9/26. A fire drill was recorded 2/23/26. A shelter in place drill was recorded 12/17/25. A fire inspection was conducted 9/2/2025. The program does not provide transportation during the school year. They do participate in field trips and use Yadkin County Schools buses. Off-site verification forms were current. The following violation was cited during the visit today. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid was not completed with an approved training course. .1102(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before 3/23/2026. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation& You may email the letter to jennifer.davis@dhhs.nc.gov : If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Restrictions include daytime care only; school age children only; meets enhance space; meets enhanced ratios; reduced group size by 1 in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. The license was observed, and the restrictions were met. This program is operated by Yadkin County Schools. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Supervision was adequate during the visit. I observed positive interactions today. I observed children in routine care and gross motor play. There is a new principal at the school this year with a current qualification letter. No medications were reported, nor observed. Storage of hazardous items was monitored today. Program records and required postings were monitored. A fire drill was recorded 10/20/2025. A shelter-in-place drill was recorded 9/23/2025. A fire inspection was conducted 9/2/2025 . Off-site verification forms were current for staff, children, and transportation. The program does not provide routine transportation but provides transportation for field trips. The follow violations were cited: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The administrator had an expired qualification letter. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. A current qualification letter was not on file for the administrator. Informed by national research, trends and outcomes, and input from more than 1,800 NC early childhood education professionals, Pathways to the Stars incorporates recommendations for change, such as: • Offering a choice of three pathways to demonstrate quality and earn a 2-5 Star Rated License. • Updating education standards for early childhood educators to expand options; and • Adding standards for both educators and facilities that support continuous quality improvement and family/community engagement and leadership best practices. • DCDEE has updated the Child Care Rule Rollout Course in DCDEE Moodle to include the July 2025 New Rule/Rule Amendments Overview covering the new QRIS Modernization rules.
