This facility operates with a One Star License issued August 25, 2023. The last annual compliance visit was conducted by Child Care Consultant Shereen Pickett on April 23, 2024. Prior to this visit, the program’s 18-month compliance history was 97%. The center is owned by Topsail Montessori, LLC. As of today, the corporation is current and active with the NC Secretary of State. K. Carter, Director, was present and available for consultation. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. A variety of age-appropriate learning materials was observed in each classroom. Children in spaces 1, 3 and 5 were observed during outdoor play. Children in space 4 were engaged in free choice activities, accessing a variety of age-appropriate materials. Space 2 is no longer used as a child care space. All children were adequately supervised, and all staff interacted with children in a positive and appropriate manner. All meals are provided from home for each child. One violation was observed and corrected during the visit. A compliance letter is not due at this time. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Two outlets in space 1 and one outlet in space 5 were observed uncovered. 10A NCAC 09 .0604(c) Technical assistance regarding violation cited: For the safety of all children, ensure all unused electrical outlets accessible to children remain covered at all times. Reminders: G. Blanchard, hired on March 10, 2025, must complete the maltreatment training, and CPR/FA training by June 10, 2025, to maintain compliance. R. Darnell, hired on February 16, 2025, must complete the maltreatment training, and CPR/FA training by May 16, 2025, to maintain compliance. A fire and playground monthly inspections are due in April. As discussed, the climbing dome on the playground for building one may not be used with children until it is properly spaced and the required six inches of fall zone surfacing is added. The new play structure added to the playground for building two comes with a balancing bar. Today, children were observed using that bar as a seated swing, placing them at danger for hitting their head on the top metal bar and/or falling from a critical height. We discussed today the need for closer supervision and placing safety reminders on utilizing the bar to balance only. Staff members must remain close by while children are using that set. A new fire inspection is due by June 19, 2025. Ensure you contact the fire marshal's office soon as a reminder that a new inspection is due soon. In space 4, an epi-pen prescribed for use with an enrolled child expired in January 2024. 10A NCAC 09 .0803(1)(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 02, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Rated License Information: The program contacted me via email and requested to have the Environment Rating Scales conducted. They are interested in increasing to a 3-star license. We discussed the process of the star rated license in detail. Ms. Harris explained that she had previously spoken with Ms. Ross regarding her interest to increase the program’s star rating. She also stated the program completed an outreach assessment through the North Carolina Rated License Project (NCRLAP) in June 2023. The completed application for an initial assessment was received via email on April 16, 2024. An assessment was ordered by me the same day. The North Carolina Rated License Assessment Project (NCRLAP) contacted the program, and a 30-day window was scheduled for June 03 – June 28, 2024. In preparation for the upcoming ERS, I recommended that you contact your Quality Consultants at your local Smart Start very soon. They can give you resources, conduct mock assessments, and provide other technical assistance. Also, refer to the ncrlap.org website for 1) webinar information, 2) Additional Notes for Clarification, 3) frequently asked questions, 4) resource list documents, 5) training videos & video supplements, 6) and other pertinent updates. Utilizing the resources and online trainings provided through NCRLAP’s website was also advised. Regarding Education standards, we reviewed the Staff and Training Worksheet and draft Education worksheets to assist you in determining which staff members need DCDEE WORKS evaluations and how many points you may be eligible for in this component. As discussed, you must ensure all key staff including you have complete WORKS evaluations so appropriate Education points may be awarded during the rated license reassessment process. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years and to have inspections for lead paint and asbestos if applicable. This facility operates with a One Star License issued August 25, 2023. The last annual compliance visit was conducted on May 4, 2023. As of November 22, 2023, the 18-month compliance history for your facility was 100%. The center is owned by Topsail Montessori, LLC. As of today, the corporation is current and active with the NC Secretary of State. Today’s visit was conducted by Child Care Consultant April Bosse on behalf of your Child Care Consultant Tammy Ross. Fire Inspection – 06/19/2023 - Approved for day time care only Sanitation Inspection – 05/17/2023-Superior Staff member Jessica Johnson-Smith was present and available for consultation and accompanied me as I visited each classroom. The owner, Julie Ayala, was also present during the visit; she was in the office area of building #1. Ms. Ayala and I discussed the new fence on the playground. The big playground was divided into two with a wooden fence with two swinging gates. The space was measured today to determine the new capacity. The area that has been fenced off has a capacity of 21 children at 75 sq. feet per child. The new space capacity worksheet for your outdoor area was printed and emailed to you for your files. There are seventy-six children enrolled and sixty-nine children, ages 2-6, were present today. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. A variety of age-appropriate learning materials was observed in each classroom. Two-year-old children in Space 3 were observed eating snacks and completing routines. Preschool children in Space 1 were observed participating in free choice activities such as sorting balls with tongs, using droppers for art, and reading books. Preschool children in Spaces 4 and 5 were engaging in activity areas with a flour sensory table, jingle bell counting activity, dreidel game, writing letters to Santa and participating in group time as the caregiver demonstrated a new lesson. All children were adequately supervised, and all staff interacted with children in a positive and appropriate manner. Lunch is provided from home for each child. New staff files were monitored today. The following violations were observed today and must be corrected immediately: Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Three new staff members hired on 8/24/23, 9/25/23, and 6/8/23 did not receive at least 16 hours orientation within the first 6 weeks of employment. .1101(a) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical Assistance: Today we discussed ensuring that as children move rooms, their diaper creams, or other ointments such as bug spray go with them. Ensuring these ointments move with children to the new classroom will help the caregivers monitor the permission to apply forms for these children and ensure the ointments are applied as parents have requested. As you move forward with any renovations, please ensure that you are keeping your Child Care Consultant Tammy Ross informed. Adding any permanent fixtures such as sinks, or counter space could change the capacity for a room and will need to be remeasured. Any new spaces you may create, not previously approved for care of children, will need to be approved for use before children can be in those spaces. Reminders: Child Care Rules were revised July 1, 2023, and a new rule book was posted to the DCDEE website. As discussed, you should discard any old versions of the rules and ensure you are referring to the most current rule book when reviewing or researching requirements. Sanitation rules were also readopted July 1, 2023. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always whether they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. There have been changes to the 2023 licensing fees this year. License fee collection has moved to December 2023. • All invoices will be emailed, and no paper copies will be sent through the U.S. Postal Service. • All invoices will be emailed by December 1, 2023, and will include the invoice number and total amount due. • All payments are due no later than December 31, 2023 Payments must be submitted online only through the Division’s website using a Mastercard or Visa debit or credit card, or by e-check.
