The last annual compliance visit was conducted on 10/10/25. The 18-month compliance history is currently 100%. This facility currently operates with a 5-star license via the accreditation/head start pathway issued 2/24/2026. I completed a walkthrough of the indoor and outdoor spaces used by children and monitored for the following: supervision, staff/child ratios, storage of hazardous products and medication, adequate and approved space and program records. The staff and training worksheet for the annual compliance visit was used to monitor the current required information for the existing staff and some staff files were monitored for required information. All staff have current CPR and First Aid certification, and current criminal background check qualifying letters on file. The last documented fire drill was conducted on 4/22/26. The last documented shelter-in-place drill was completed in March 2026. A superior sanitation inspection was completed on 5/4/26 with 6 demerits. A fire inspection was completed on 3/31/26. The facility was found to be Satisfactory and approved for day time care only. The playground inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, menu, staff/child ratio chart, emergency telephone numbers, emergency procedures form, tobacco free signage and emergency medical care plan. During the visit there were twenty-nine (29) children present. I observed the children in space #3 participating in free play in activity areas, and completing bathroom/handwashing routines in preparation for lunch. The interactions and language observed were positive. The children in space #4 were observed participating in outdoor free play. The teachers were engaged with the children and moved about the playground providing guidance when needed. The interactions and language observed were positive. The children in space #1 were observed transitioning to the outdoor play area. There was one (1) violation of childcare requirements observed during the visit. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. Several areas in space #3 to include the wall corners were observed with peeling/chipped paint. Corners of walls in space #4 were observed with chipped paint. 15A NCAC 18A .2825(a) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. Technical Assistance: *Ensure that a lockdown or shelter-in-place drill is conducted prior to June 29, 2026, when the facility will close for summer break. *A discussion took place regarding the use of a timer to help the children and teachers monitor screen time. Upon arrival, I met with Jean Douglas, Site Director. We completed a walkthrough of the facility and outdoor learning environments. The facility has a 5-star license with seven (7) points in program standards, seven (7) points in education and one (1) quality point. The last rated license assessment was conducted in January 2019. The facility had a 94% compliance history prior to this visit. The “Annual Compliance Monitoring Checklist for Child Care Centers” was used to note the requirements monitored today. Prior to the visit I sent an email requesting the staff and training worksheets to Jean Douglas, Site Director. The worksheets were received prior to the visit. Additional staff were hired since the worksheets were received; therefore, I completed completed the worksheet with the new staff member's information during today's visit. Additional notes on the worksheets were made during the visit as well. Staff and children's records were reviewed during the visit. There have been three (3) new staff hired since the last monitoring visit was conducted. During the visit I monitored the files of the new staff members and one (1) existing staff member file. I selected four (4) children's records for review. During the visit there were twenty-five (25) children present. The children were observed during arrival, bathroom/handwashing routines, outdoor free play, and preparation for lunch. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, current activity plans, First Aid posters, menu, applicable staff/child ratio charts, tobacco free signage, and evacuation routes. The following centers are available: books, blocks, art, dramatic play, manipulatives, science, music, and math. The last fire inspection was conducted on 3/19/24, the facility was found to be satisfactory and approved for daytime care only. The last sanitation inspection was conducted on 4/30/24. The facility received a Superior rating with 2 demerits. The centers Emergency Drill Log report form was reviewed during the visit. Emergency Drills were completed as required. The last fire drill was conducted on 9/18/24. A shelter-in-place drill was conducted on 8/6/24. The monthly outdoor inspections have been completed as required. Per the site director transportation is not provided. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, medication administration, program records and management. There was one (1) violation of childcare requirements observed during this visit. Violation Number Comment Rule 1329 Application for enrollment did not include all required information. Two (2) of four (4) children's records reviewed did not contain all required information on the application. .0801(a)(1-7) The violations documented during the visit must be corrected immediately. The teachers were attentive to the needs of the children. The children in space #4 were observed completing bathroom/handwashing routines and engaging in free play in activity areas. One teacher assisted children with bathroom/handwashing routines, while the other teacher engaged in free play. The interactions and language observed were positive.
