The facility currently operates with a 3-star license. The facility had a 99% compliance history prior to this visit. The last rated license assessment was conducted in November 2022. There are sixty (61) children enrolled in the facility and thirty-six (36) were present today. The children were observed during indoor free play, outdoor free play, preparation for lunch, and bathroom/handwashing routines. The facility does participate in the child and adult food program. I monitored the menu and the applicable requirements of the food components were being met. Prior to the visit the staff and training worksheets were received via email from Lorie Tew, Director. There are a total of seven (7) staff employed. Since the last monitoring visit no new staff have been hired. I monitored the files of two (2) existing staff. I monitored six (6) children's records for 10% of children’s files. I monitored staff files for the completion of the annual in-service hours and the required health and safety training. I monitored the regulatory system to verify current criminal record qualification letters for staff. All staff have current CPR and First Aid training. All staff scheduled to work in the infant classroom have valid ITS-SIDS certification. Fire drills have been documented and completed as required. The last fire drill was documented as being conducted on 5/28/26. The last shelter-in-place drill was documented as being conducted on 5/11/26. Monthly playground inspections have been completed as required. A superior sanitation inspection was conducted on 4/28/26 with 4 demerits. The last fire inspection was completed 5/28/26, the facility was found to be Satisfactory and approved for day-time care only. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily classroom schedules, activity plans, Safe Sleep policy, First Aid poster, menu, applicable staff/child ratio charts, evacuation routes, infant feeding schedules, cot lists, allergies and tobacco free signage. The provider stated that transportation is not being provided. Rated License: The facility has chosen pathway 2: classroom and instructional quality. During today’s visit the following were reviewed: A Continuous Quality Improvement Plan for each staff member, facility CQI plan, the family and community engagement form with supporting documents, the curriculum for each age group, formative assessment tool, training certificate for the administrator, L. Tew, verifying completion of training on the use and implementation of a curriculum and formative assessment. The facility uses the Funny Daffer curriculum for all ages and the Funny Daffer formative assessment. The application for assessment for a rated license, facility CQI plan, the family and community engagement form, and staff information and education sheets will be sent to the child care consultant via email. Staff education was reviewed via WORKS prior to this visit. Upon receipt of the required application and other documents the child care consultant will move forward with transitioning the facility to a new license via the classroom and instructional quality pathway. It is proposed that the facility will be issued a 4-star license, if changes are needed and/or an error in determination is found, a written correspondence will be sent to provide justification for the reduction in stars. There was one (1) violation of child care requirements observed during today's visit regarding staff training hours. Violation Number Comment Rule 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. B. Martin completed ten (10) of the twenty (20) required on-going training hours. .1103(a) The violation documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 6/26/26. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during the visit may negatively affect this score. The facility has a 3-star license with three (3) points in education, four (4) points in program standards and one (1) quality point. The facility had a 100% compliance history prior to this visit. The last rated license assessment was conducted in November 2022. There are sixty (60) children enrolled in the facility and thirty-seven (37) were present today. The children were observed during indoor free play, outdoor free play, and bathroom/handwashing routines. The facility does participate in the child and adult food program. I monitored the menu and the applicable requirements of the food components were being met. Prior to the visit the staff and training worksheets were received via email from Lorie Tew, Director. The facility operates with a 3-star license. The facility has a current compliance history of 89%. There were twenty-four (24) children present today. The children were observed during indoor and outdoor free play, and bathroom/handwashing routines. The last annual compliance visit was conducted on 6/21/23. A superior sanitation inspection was completed on 8/30/23. A fire drill was last conducted on 1/31/24. A lockdown drill was last conducted on 2/2/24. The indoor areas used by children were monitored today. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, daily schedules, First Aid posters, applicable ratio charts, emergency medical care plan, emergency telephone numbers, SIDS policy, evacuation routes, tobacco policy, cot lists, and menus. The following items were monitored today: supervision, staff/child ratios, storage of hazardous products and medications, adequate/approved space, program files, posted license and license restrictions. There have been two (2) new staff hired since the annual compliance visit was conducted. I monitored the files of the two (2) new employees. I monitored the current criminal record qualifications and specialized training for the staff using the staff and training worksheets that were completed for the last annual compliance visit. All staff have current CPR and First Aid training. The staff have completed the required health and safety training except for the new staff. I observed the children from both space #1a and space #2b engaged in free play on the outdoor learning environment. The teachers were engaging with the children as they were playing. The interactions observed between the teachers and children were positive. I observed the children in space #2a sleeping and engaged in free play in activity areas. The teachers were sitting on the floor engaging with the children and providing positive guidance. All language and interactions were positive. There was one (1) violation of child care requirements observed during today’s visit. Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months.
