I met with Jean Douglas, Site Director, and we conducted a walk-through of the facility. The facility operates with a 5-star license that was issued 10/25/19. The facility had a 92% compliance history prior to this visit. NC Pre-K monitoring was completed during today’s visit. There are 70 children currently enrolled and 55 were present during the visit. I observed the children present engaged in the following activities: lunch, free play in activity areas, large group shared reading, rest time, and bathroom/handwashing routines. Lunch consisted of chicken casserole, peas with carrots, mixed fruit, and milk. The "Annual Compliance Monitoring Checklist for Child Care Centers" was used as a monitoring tool during today's visit. Prior to the visit the staff and training worksheets were received. Staff and children's records were reviewed during the visit. No new staff have been hired since the last monitoring visit was conducted in March 2025. I reviewed two (2) staff files. I selected seven (7) children's records for review. Five (5) of the seven (7) children’s records monitored were those of enrolled NC Pre-K children. The last fire inspection was conducted on 3/25/25 and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted on 3/25/25. The facility received a Superior rating with 7 demerits. The emergency drill log was reviewed during today's visit. All drills have been completed as required. The last fire drill was conducted on 3/13/25. The last lockdown drill was conducted on 3/31/25. Playground safety inspections were documented as required. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, current activity plans, CPR and First Aid poster, menu, applicable staff/child ratio charts, tobacco free signage, allergies, mat lists, and evacuation routes. Medication forms and all documentation were completed as required. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, medication storage, program records and management. NC Pre-K: This NC Pre-K site serves fifty-four (54) children. The facility uses the Creative Curriculum with Smart Tech by Teaching Strategies. On-going developmental assessments/checkpoints are conducted throughout the school year. The tool used assesses the children in social, physical, cognitive language development, literacy, math, arts, social studies and science. The information gathered from the assessments is used to prepare information that is shared with parents during parent/teacher conferences that are held two (2) times per year. Both parent/teacher conferences have been held for the school year. During these conferences a developmental and learning report is shared with the parent. The facility meets the 1:9 staff/child ratio requirements. The classrooms operate from 7:45am until 2:15pm to meet the required 32.5 hours per week requirement. Family engagement activities occur weekly. Parents are encouraged to engage in their child's education and asked to complete weekly take home activities with their child. Parents are invited to attend the facility for on-site activities many times throughout the school year. The following activity centers are present in the classrooms: books/literacy, dramatic play, blocks, art, writing, science, music, sand/water, and manipulatives. There was one (1) violation of child care requirements observed during today's visit. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. During the visit the outdoor learning environment was observed with multiple pieces of trash in different areas. 10A NCAC 09 .0604(p) The cited violations were corrected during the visit. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history was 92% prior to this visit. Violations cited during the visit may negatively affect this score. There is a concern that staff did not interact with a child in a nurturing and caring manner. There is a concern that inappropriate discipline was used. I reviewed the allegation with the Administrator, J. Douglas, who submitted the self-report on December 5, 2024. Per the Administrator, J. Douglas, on December 2, 2024, a therapist who was onsite working with a child reported that the child told her a teacher hit him. The incident was reported by the therapist to T. Mack, office personnel, as J. Douglas was not onsite when the incident occurred. T. Mack informed J. Douglas of the conversation between herself and the therapist on December 4, 2024. The staff member involved in this complaint, J. Ingram, was not present today. Per Ms. Douglas, the staff member has been suspended until further investigation. I was not able to get a verbal statement, but I was able to obtain a written statement of the account for the staff. I also obtained a written statement submitted by the therapist. I interviewed staff member S. Wall, who was present and providing care in space #2 where the child is enrolled on the date of the incident. S. Wall stated that J. Ingram plays around with the children, picks them up and flips the children around, and the children hit J. Ingram. She stated on the day of the incident she saw J. Ingram playing with the children, but she did not witness him hit a child. I viewed the footage of the incident today with Ms. Douglas and determined that the child was hit on the head by staff member J. Ingram. Ms. Douglas stated that Mr. Ingram has been employed with the center since September 2024. There are no prior incidents involving his care of the children. The guardian of the child was notified of the incident, and the child continues to be enrolled at the facility. Based upon the recorded video footage observed today, the allegation that staff did not interact with a child in a nurturing and caring manner is substantiated. The allegation that inappropriate discipline was used is substantiated. The following violations of child requirements were cited during the visit. Violation Number Comment Rule 872 Appropriate discipline practices were not followed. Video footage viewed during this visit shows a child walk up and hit teacher, J. Ingram, from behind, and J. Ingram turns around and hit the child on the head. .1803 The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 1/1/2025. You are required to maintain compliance with all applicable childcare rules and regulations at all times. I made an unannounced visit today to investigate the complaint allegation of violating child care requirements as listed above. I reviewed the allegation with the Administrator, J. Douglas, who submitted the self report on 11/6/24. J. Douglas stated she contacted DCDEE on 11/5/24, to make the report but had to leave a voicemail, she received a return phone call on 11/6/24. Per the Administrator, J. Douglas, on November 4, 2024, at 12:08 p.m., a child enrolled in space #4 activated the fire alarm station setting off the fire alarm across the entire building. Ms. Douglas stated that she was in the parking area preparing to leave the site when she heard an alarm going off. She returned to the building and noticed all staff along with the children exiting the classrooms. She was informed that a child set off the alarm. Ms. Douglas went to her office and contacted the fire department. She later learned that a sleeping child had been left unattended in space #4. Upon reviewing the video footage of the incident she determined that the child was unsupervised for approximately eighteen (18) seconds. There was nothing in the classroom that put the child in harms way. The video footage of the incident was reviewed during this visit with J. Douglas. The footage shows a teacher, A. Revels, getting up from a chair, going to the primary exit door of the classroom where she opens the door and begins to exit with the children and the other staff member who was present in the classroom following behind her. As they are exiting the classroom space a third staff member, J. Robinson, is seen approaching the classroom and she proceeds to hold the exit door open as the children leave the space. Approximately eighteen (18) seconds pass when J. Robinson is seen entering space #4. As she enters the space she is seen looking in the classroom bathroom as if she is looking for someone. She is then seen picking sleeping child up and exiting the classroom with the child. In the video footage the cozy area where the child was sleeping isn't visible. The staff involved in this complaint, A. Revels, R. Jacobs, and J. Robinson were not present today. Per Ms. Douglas, the three(3) staff have been placed on investigatory suspension until further investigation. I was not able to get their verbal statements but I was able to obtain written statements of the accounts for each of the three staff. Based on the video footage of the incident I determined that the child was left unsupervised for approximately eighteen (18) seconds before J. Robinson returned to the classroom to retrieve the child. Ms. Douglas provided the following information as the measures that she and the facility has taken to correct this incident and to prevent any further occurrences. Ms. Douglas stated that since the incident occurred, a mandatory meeting was held in which policies and procedures regarding the supervision of children were reviewed. Training on active supervision and strategies to put active supervision in place was administered during this meeting. Ms. Douglas obtained a signed acknowledgement form for this review as well. The parent of the child was notified of the incident on 11/6/24. The child continues to be enrolled at the facility and has been in attendance each day since the incident occurred. Based upon the interview with the Administrator, the written statements from the staff and the recorded video footage observed today, the allegation that a child was left unsupervised inside the classroom is substantiated. It was determined that staff A. Revels, R. Jacobs, and J. Robinson were responsible for supervising the children and failed to provide adequate supervision during an accidental fire alarm. The following violation of child care requirements was cited. Violation Number Comment Rule 303 Children were not adequately supervised at all times. A child was left unattended in space #4 during an accidental fire drill on 11/4/24. .1801(a)(1-5) The violation documented during the visit must be corrected immediately. The facility had a 95% compliance history prior to this visit. NC Pre-K monitoring was completed during today’s visit. There are 71 children currently enrolled and 50 were present during the visit. I observed the children present engaged in the following activities: indoor free-play, outdoor free-play, bathroom/handwashing routines, lunch and rest time. The "Annual Compliance Monitoring Checklist for Child Care Centers" was used as a monitoring tool during today's visit. Prior to the visit I sent an email requesting submission of the staff and training worksheets to Jean Douglas, Site Director. The worksheets were received prior to the visit. Staff and children's records were reviewed during the visit. Four (4) new staff have been hired since the last monitoring visit was conducted in October 2023. I reviewed the staff files of the four (4) new employees and two (2) staff files of existing staff for 10% of staff files monitored. I selected nine (9) children's records for review. Six (6) of the nine (9) children’s records monitored were those of enrolled NC Pre-K children. The last fire inspection was conducted 3/19/24 and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted on 3/14/24. The facility received a Superior rating with 5 demerits. The emergency drill log was reviewed during today's visit. All drills have been completed as required. The last fire drill was conducted on 4/17/24. The last lockdown drill was conducted on 2/16/24. Playground safety inspections were documented as required. I met with Jean Douglas, Site Director and I conducted a walk-through of the facility. The facility operates with a 5-star license that was issued 10/25/19. The facility had a 95% compliance history prior to this visit. NC Pre-K monitoring was completed during today’s visit. There are 71 children currently enrolled and 50 were present during the visit. I observed the children present engaged in the following activities: indoor free-play, outdoor free-play, bathroom/handwashing routines, lunch and rest time. The "Annual Compliance Monitoring Checklist for Child Care Centers" was used as a monitoring tool during today's visit. Prior to the visit I sent an email requesting submission of the staff and training worksheets to Jean Douglas, Site Director. The worksheets were received prior to the visit. Staff and children's records were reviewed during the visit. Four (4) new staff have been hired since the last monitoring visit was conducted in October 2023. I reviewed the staff files of the four (4) new employees and two (2) staff files of existing staff for 10% of staff files monitored. I selected nine (9) children's records for review. Six (6) of the nine (9) children’s records monitored were those of enrolled NC Pre-K children. The last fire inspection was conducted 3/19/24 and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted on 3/14/24. The facility received a Superior rating with 5 demerits. The emergency drill log was reviewed during today's visit. All drills have been completed as required. The last fire drill was conducted on 4/17/24. The last lockdown drill was conducted on 2/16/24. Playground safety inspections were documented as required. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, current activity plans, CPR and First Aid poster, menu, applicable staff/child ratio charts, tobacco free signage, allergies, mat lists, and evacuation routes. Medication forms and all documentation were completed as required. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, medication storage, program records and management. NC Pre-K: This NC Pre-K site serves forty-three (43) children. The current NC Pre-K staff were the same staff indicated in the NC Pre-K Plan System. The facility uses the Creative Curriculum with Teaching Strategies Gold. On-going developmental assessments/checkpoints are conducted throughout the school year. The tool used assesses the children in social, physical, cognitive language development, literacy, math, arts, social studies and science. The information gathered from the assessments are used to prepare information that is shared with parents during parent/teacher conferences that are held two (2) times per year. Both parent/teacher conferences have been held for the school year. During these conferences a developmental and learning report is shared with the parent. The facility is meeting the 1:9 staff/child ratio requirements. The classrooms operate from 7:45am until 2:15pm to meet the required 32.5 hours per week requirement. Family engagement activities occur weekly. Parents are encouraged to engage in their child's education and asked to complete weekly take home activities with their child. The following activity centers are present in the classrooms: books/literacy, dramatic play, blocks, art, writing, science, music, sand/water, and manipulatives. There was one (1) violation of child care requirements observed during today's visit. Violation Number Comment Rule 1769 The health assessment did not include a dental screening. One (1) of six (6) NC Pre-K children's records monitored did not contain a dental screening. .3005 (a)(5) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 5/8/24. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. The last monitoring visit was conducted on 5/23/23. The 18-month compliance history prior to this visit was 97%. A walkthrough of the indoor space used by children was conducted and monitored for the following: supervision, staff/child ratios, current CPR and First Aid certification, storage of hazardous products and medication, adequate and approved space and program records. The staff and training worksheet from the annual compliance visit was used to monitor for current required information for the existing staff. Six (6) new staff have been hired since the last monitoring visit was conducted. All new staff were added to the staff and training worksheet today and files of all new staff were monitored during today's visit. All staff have current criminal background check qualifying letters on file. All emergency drills were conducted and documented as required. A superior sanitation inspection was completed on 2/15/23 with 8 demerits. The playground inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, menu, staff/child ratio chart, emergency telephone numbers, emergency procedures form, tobacco free signage and emergency medical care plan. During the visit there were sixty-five (65) children present. During the visit the children were observed participating in outdoor free play, bathroom/ handwashing routines, large group activities, and free-play in activity centers. I observed the children enrolled in space #5 and space #3 participating in outdoor free play. The children were participating in relay races led by the teachers. The teachers were traveling around the outdoor learning environment interacting with the children and providing assistance when needed. All interactions observed between the teachers and children were positive and nurturing. During the visit the children in space #4 were observed participating in free play in activity areas. The teachers were attentive to the needs of the children. Positive guidance was used as needed. The interactions between the teachers and children were positive. The teachers were observed engaging with the children during free-play, participating in small group opportunities and offering assistance as needed. The children in space #1 and space #2 were observed participating in bathroom/handwashing routines and transitioning to the outdoor learning environment. There was one (1) violation of childcare requirements observed during the visit.
